Grabbed hood and put feet on bumper of Kenworth truck by himself to try to lift hood. As he pulled he felt pain in lower to mid left side of stomach. Taken to ER and CT was done. Showed negative and doctor said he had pulled a muscle.

The employee continued to work until further medical examinations revealed a tear in his right shoulder that must be surgically repaired. This injury become a lost time injury on 02/06/2012.
EE was installing an alternator when he developed leg cramps and as he was dismounting the truck he struck his hand on the fender mirror resulting in a laceration to the Right hand.

Injury initially sprained his right knee on 2/15/10 lifting mine belt for set up and has worked until today, date (3-2-10) he has become a reportable lost time injury after an MRI revealed some tissue damage in his right knee.
The employee was attempting to hang an electrical cable with a cable hanger with a metal hook when the cable slipped, puncturing the employee in the right middle finger with the metal hook which required 3 stitches.

Employee was repairing a steering clutch in a bent over postion. As he raised up. He felt a sharp pain in his lower back. He went to dr and was placed off work on 7/8/09.
EE stated that as he was dismounting his equipment at quit time, his shoulder felt as if it popped out of socket. EE was taken to Emergency room and results were negative. EE went to his personal DR. on 06/24/09 and we are waiting results from test. EE had surgery on this shoulder in 2008.
Employee was mounting his drill when he developed a pain in his lower abdomen.
EMPLOYEE WAS ATTEMPTING TO SUPPLY HIS ROOF BOLTING MACHINE, WHILE STOOPED OVER IN LOW COAL HE TURNED AROUND WITH SUPPLIES AND TWISTED HIS RIGHT KNEE.
Initially employee injured his back on 1-16-09. He worked thru 1-23-09. He went to the doctor on 1-23-09 and started missing work on 1-26-09. He strained his back when he bent over to pick up cable cross-over pads.

EE alleged while changing a piece of pipe in the prep. Plant his foot got stuck in magnetite and when he pulled his foot loose he felt a burn in his left knee. He was receiving treatment from his dr for gout in his feet an dknee prior to the alleged incident. foreman stated that he did not report anything until 8/13/07. this investigation is on going.
EE was removing a tooth from the ripper of a D-11 dozer when a piece of metal flew off striking him in his right leg. EE spent one night in hospital while metal was removed.

Employee stated that he bumped his left leg below the knee on 3/27/06 on a piece of belt structure and it made a blue place. He said that he did not think it was anything but it got worse over time. When he came outside after working the third shift on 4/10/06 he showed his leg to the mine superintendent and told him what had happened. He told him to go to the doctor.
While installing a fender on a D10 dozer, employee's hand was mashed when fender slipped.
While changing out grease pump employee felt pain in left shoulder. X-rays were negative. We are now awaiting approval of an MRI by insurance carrier.
Employee had hurt his shoulder on 9-12-05 but had continued to work. Employee Dr. decided on 12-15-05 employee needed some corrective surgery.
