The D-10R dozer was being disassembled in preparation to be moved. The push-arms that had been removed were sitting on the ground. Employee stepped over one of the push-arms and it fell, striking right leg causing a fracture.

EE was helping 2 other employees lift a shuttle car speed reducer on to a 4-wheeler when EE felt something pop in right knee. EE works a 4 on 4 off schedule and was scheduled to return to work on 6/26/2017 in which EE's doctor stated was unable to return to work.

Employee stated was removing a rock off of the left side continuous miner measuring 12" in length 13" in width and 10" in thickness and as EE was going to roll it into the conveyor chain the rock shifted and rolled back onto left hand.

Employee was lapping up approximately 50 feet of line curtain when they felt a sharp pain in their lower back. The curtain was covered with mud and water at the time of the incident.

EE (contractor) was assisting 3 co-workers to hang a section of belt and structure to a belt chain. As ee was lifting ee felt a sharp pain and something pop in lower back.

EE was moving the miner cable out of the roadway when the bolt machine was backing up the bolter cable caught the employee's left leg and caused it to bend backwards injuring EE's left knee. This employee did not seek medical attention at the time of the incident. EE went to the doctor on 3/14/2016 in which it was determined EE could not return to work pending on an MRI.

The injured employee was being assisted by a co-worker loading top structure into a scoop bucket and as EE turned to load the last piece of structure EE felt some pain in upper back between shoulder blades.

Employee changed the water filter on the continuous miner and was putting the shield back in place with help from a coworker. As they dropped the shield down employee caught right finger between the shield and frame.

Employee had changed out the collector bag in the roof bolting machine dust box. Employee slammed the door shut on the box and caught the middle and ring fingers of left hand between the door and frame.

Employee and a coworker were installing a bottom belt roller. One end of the roller was put in the hanger. To install the other end, the coworker lifted up on the belt using channel locks while injured lifted the end of the roller into place. Some weight of the belt was still on the roller and he felt pain in his back as he lifted it.

Employee was in the process of unloading trash from bucket of scoop. The scoop was loaded with mine trash and was parked near the dumpster on the surface of the underground mine. The employee was lifting trash into the dumpster. He pulling on piece of curtain it was stuck, resulting in twisting of lower back. Continued to work rest of shift then went for medical attention.

Employee stated he was unlapping belt for a section move when he felt some pain in his right knee. Employee continued to work and sought medical attention on 6/19/2015 where an x-ray was negative. He was referred to a specialist and an MRI results were discussed with the employee on 7/23/2015.

Employee was installing a rubrail on the left miner. He was using left hand to push the hoses over so the rubrail would close. As the rubrail closed against the miner it pushed a piece of rock against employees left ring finger and the hoses causing a laceration and a fracture to the tip of the finger

EE was cleaning top of power center. He picked up a bag of rock dust & twisted to his right to drop it on the ground & felt a sharp pain in right side of lower back. He went to the foreman & stated "I may have hurt my back." Foreman asked if he needed to go outside. He said "No, I'll be OK." He completed his shift. He went to the hospital the following day due to pain.

EE was assisting 3 others remove a shaft from a Mac 12 mantrip. They had one side of the shaft suspended in the air approx 26 off the mantrip with a pair of come-alongs. The IE grabbed the bearing end of the shaft to lift it over the rail of mantrip. Another EE continued to crank the come-along causing the shaft to shift, catching his middle/index fingers between trip and shaft.

Employee alleges that he lifted a pallet and he felt his left shoulder pop. This employee has had surgery on both shoulders for off job injuries. Management is waiting for conclusive medical evidence from MRI to substantiate whether or not a new injury has occurred.

Sub zero weather: An employee was boosting a welder/generator from a truck to get it started. After the unit started he reached down to the battery to detach the booster cable when one of the leads struck the fan blade causing it to disintegrate.

The employee misplaced a 6x8x16 solid cinder block on a stopping which fell striking the employee on the gloved left thumb. Employee finished his shift, sought medical treatment and returned to work. He did not miss any scheduled shifts.

Emp was cleaning around tailpiece when he felt a pull in his lower back.Employee finished shift.Emp continued to work until 5/18/12 and took 5 days vacation time through 5/25/12 during which time he scheduled an MRI. His 1st date of missed work was 5/29/12 awaiting results of MRI. We received notice of a positive MRI on 6/14/12. Employee remains off work at this time.

Employee had welded lug on top of lifting tube to secure a choker to be used to lift a platform out of the water, Lug broke after applying pressure and employee was struck by the lug on the left hand, Resulting in a laceration and fracture to the ring finger.

The employee was loading a hydraulic pump into a scoop bucket when the pump slipped and caught his index finger between the pump and the bottom of the scoop. The accident resulted in a severed tendon in his right index finger between the knuckle and the first joint.

EE was using a slate bar to pull pin from a rubrail on C/M. When the pin came out, the rubrail struck him in the bend of his leg (ankle/foot), knocking him to the ground. He was complaining of pain in his right leg, foot/ankle. The rubrail was bent just slightly making the pin difficult to pull.
