While installing lagging on the head pulley, the pulley was being rotated using the drive belt. During rotation, employee's fingers were positioned too far back on the drive belt and motor pulley and two fingers became pinched between the drive belt and motor pulley, the tension prevented immediate release of the fingers. Drive belt was cut.

Showing all 40EE strains right wrist when rear fender EE is holding with right hand suddenly sags while attempting to latch it back after checking oil level. Injury originally classified as First Aid by Physician / Restrictions were added by Orthopedic Specialist on 12/23/2025.
Employee lifted improperly and twisted while lifting; metal cover that was lifted weighs 35lbs roughly. Due to positioning, the tech needed to remove the cover to access the filter in the compartment of the 980G. Resulted in back strain.
EE was injured while washing out a dust bin when material buildup sloughed off and fell onto EE's left arm pushing it downward onto the steel opening causing a laceration to the bottom of the wrist.
During screen maintenance the sharp edge of a metal panel caused an open fracture to the right index finger of the employee.

150 ton track press was being used to press track pins out of a 390. Press back plate had to be removed first. 2 cranes were used to support track press about waist high. Tech was positioned at back plate. Back plate was removed by sliding off guide rods manually by tech. When back plate came off guide rods, the press section cantilevered up striking tech's lip causing laceration.
Employee used a wedge to relieve pressure in the primary jaw crusher in order to get a bolt out. When the employee was removing the wedge, the stored energy popped the wedge out, striking the employees right hand.
Employee was clearing catwalk when a rock fell and hit EE in the ribs.
An employee was on the dredge replacing the bolts for the cutter head coupling. The coupling slipped and severed the tip of EE's middle finger of the left hand.
Mechanic was hit by metal shard while replacing a bucket tooth on a loader.
EE was balancing electrical transformer on Unistrut. EE caught it as it shifted and felt discomfort in their back.
Miner was kneeling while performing maintenance. When miner stood up, EE felt pain in EE's right leg.
Employee stated while in the process of removing the mid axle of a portable mobile crusher, the axle linkage pivoted upward creating a pinch point resulting in injuries to the tips of three fingers.

EE was unreeling a water hose as EE walked up conveyor #2. When EE attempted to turn on the water, EE realized that there was pain in EE's arm. Update: Upon further review, right rotator cuff is torn.
Miner was completing maintenance work on cone crusher.
While unloading drill pipe from the drill truck, the miner was lowering the drill pipe to the ground using a chain. While lowering, the drill steel slipped within the chain causing EE's right little finger to get caught between the steel being lowered and another piece of drill steel on the ground.
EE lifted and carried 12"x12"x36" cribbing weighing approx. 160lbs
While removing accumulator on HP500, the accumulator fell unexpectedly. While attempting to catch the accumulator, ee's index finger became caught between the accumulator and side of the crusher mainframe. EE's finger was cut through gloves and required 7 stitches.
Employee was changing the configuration of wash screen diverter gates (hinged at the bottom) by hand. While repositioning one gate, the tips of the employee's middle and ring fingers were caught between the top of the gate and the discharge chute, resulting in injury.
Employee was shoveling in a surge tunnel. While attempting to loosen some spilled material, the material broke loose suddenly, causing the employee's right hand to travel further forward than expected, contacting the outside of a conveyor frame.
Employee pulling tire and felt pop in right shoulder.
While removing an impact bed from under the feeder, one of the EEs lost grip and it landed on the same EE's right foot, just behind the steel toe, resulting in a bruise.

Employee was picking up a hydraulic pump motor off of a waist high table to place in the back of a pickup truck. Once the pump was up the EE twisted at the waist and strained his back
Changing rollers on C-1 tail pulley. EE laid the roller flat and then pulled it out, felt a sharp pain in middle of his back. EE continued to hold roller to keep it from falling and then set the roller down. Pain was in the middle of his back, between shoulder blades. EE was sent to the clinic where he was x-rayed and diagnosed with a muscle strain and placed on light duty.
EE was lifting a wheel flange with another teammate and felt a sharp pain in his back while lifting the flange, then he finished the job and then filed the work injury.
EE MASHES RIGHT THUMB BETWEEN FRONT SPRING AND AXLE OF STOCKPILE TRUCK DURING MAINTENANCE CAUSING BROKEN BONE
Rock being transported to dump area was lodged in truck tailgate. When employee was attempting to remove the tailgate he injured his fingers.
EE was removing a drive pulley guard when it shifted and pinched his finger between the guard and the support leg causing a laceration. EE received several sutures.
EE's front tooth is broken when the hanging bin control push button box he reaches for slips from his hand and swings back striking the tooth.
It had been raining all day. At the time of the accident the rain was very heavy and oxygen bottle was slippery. The injury that resulted was a lacerated/smashed finger tip on right hand ring finger when attempting to load new oxygen bottle in mechanic's truck.
