Employee was attempting to slide a drain grate with EE's hands reaching into the grate. The grate moved and EE's left index finger was pinched between the grate and a metal tab holding the grate down resulting in a laceration requiring stitches.

Miner was throwing away empty grease barrels into dumpster. Barrel ended up slipping out of grip of miner, which resulted in a cut to the finger. Miners tried to cover to stop bleeding for 30 mins. Resulted in taking to Hospital.

Several screens decks and screens for cleaning the sand were removed and reinstalled and EE noted some soreness after the work ended. Injury was not reported to Operator until 18 Jan 2020 after employee visited the doctor.

Employee attempted to remove sand from a 1" pin hole by sticking a finger in the hole while another employee was sliding the tow bar into the equipment. The employee's pointer finger was subsequently pinched in the hole causing a laceration that required stitches.

Employee opened a cap on a radiator while it was still hot. Steam and coolant sprayed out on the left arm and face. Employee was taken to the hospital. Employee has 1st degree burn on the face and 2nd degree burn on the left wrist.

Employee had hook from lanyard hooked to the shoulder strap of a harness while not in use. While attempting to remove hook from shoulder strap to put in use the employee removed the hook and hit themselves in the mouth with the hook.

While installing speed sensor portion of BC-9A conveyor tail pulley guard, individual hit guard against frame of conveyor to remove frozen sand (guard was placed on ground during maintenance) and cut right index finger approximately 1/2" in length. *Received 4 stitches along joint of index finger.

Employee was attempting to get a strap around a broken blow bar when the bar dislodged from the backing and pinched left thumb against a rotor. EE went to have an X-Ray and shows a potential hairline fracture of thumb.

Employee told supervisor the next day and then informed manager 2 days later that ee wanted to go to the doctor to get out of guard duty. Employee went to the doctor 3 days later and received restrictions.

Initial injury occurred on 6 Jan 2016 and not reported to operator supervisor until 11 Jan 2016 after employee felt they needed to see physician for groin pain. Individual was removing sand from work area with a large shovel on main floor of 320 silo when initial discomfort was noticed. After the 5 days, the individual felt they needed to visit doctor.

Injury was reported to operator on 30 Sep 2015. Individual was working in the Rail Load Out area and was opening a grain style topped railcar door. The door was 12 to 14 feet long and when opening the door he hurt his lower back muscles.

EE was lifting on a board that was being used as a form for a small concrete pad that was to be poured. EE noticed discomfort in his shoulder. EE was seen by a doctor on 5-13-2015 and has a follow up appointment on 6-18-2015.

Employee sustained a laceration to right pinky finger from a sharp edge on a steel chute. Employee was treated with first aid. 2 days later laceration was showing signs of infection. Employee was seen by physician and given an antibiotic for infection.

While sliding a small piece of 1/8" steel plate across shop floor the plate all at once stopped and the force used to push the steel continued through causing individuals left thumb to bend backwards. No open wound occurred.

Equipment 170A was locked out and staff was wearing the proper PPE. Employee was replacing screens with the assistance of another EE. Employee had his finger on the door cover as other EE shut the door. I took employee to the local hospital where he received five stitches. The doctor placed no restrictions on employee.
