Miner was working inside the Raw Mill on the seal air vertical pipe #3 roller. Employee was disconnecting the pipe and rolled the pipe around out of the way of the work. When they rolled the pipe, the pipe swung around so quickly that it pinned their left index finger between pipe and yoke of the #3 roller. The pipe mashed it at the nail and busted the end of the index finger.

When the PA had the piece of steel outside the plane of the door, PA attempted to remove the steel from the doorway when a rock came in contact with the piece of steel debris and PA's right thumb.

Employee was helping another employee replace the cutting edge on a loader bucket. They were both lifting one end of the cutting edge to get it into place to attach it to the bucket. When they were lifting the cutting edge, it raised up until it hit the bucket then it stopped abruptly. The employee felt pain in EE's back at that moment.

Two employees were installing a metal cover on top of a silo. While repositioning the cover, one of the employees pulled up on handle forcibly causing the handle on the opposite side to go down rapidly pinching the end of the employee's middle finger between the cover's handle and the silo roof. This caused a laceration to the finger and the loss of the fingernail.

While covering a pallet with a plastic wrap bag, the employee waved the bag in the air similar to the way one opens a trash bag. The bag caught the edge of the employee's headlamp that was mounted to EE's hardhat via elastic band. The waving process pulled the flashlight away and then snapped back in EE's face, resulting in EE's tooth cutting all the way through EE's lip.

Mechanic had to enter a conveyor chute to cut a section of chain from a sprocket. While exiting the chute, mechanic dislocated right shoulder and had to be transported to hospital to relocate.

An employee working in the warehouse was unloading the palletizer with a forklift. While waiting for the pallet to fill, the palletizer table stuck in the upwards position. Once stuck it started dumping bags, so the employee stopped the system to clear the bags. When the EE picked up the first bag off the rollers EE felt a sharp pain and tearing feeling in EE's lower middle back.

While working on seal sleaves in a kiln, the employee lifted the insulated cloth to position it. Dust fell from the cloth onto employee's eye. Employee was wearing safety glasses. Employee rubbed the eye with hand scratching the eye. It was flushed at the site but employee asked for Dr and the Dr rinsed it and no foreign object was found. Eye dropes were prescribed.

Employee was attempting to unplug a chute. EE opened a port on the chute, and shortly after the chute became positive and blew dust into EE's face and eyes. Flushed eyes immediately at the facility and then transported employee to medical facility for further evaluation. Facility continued to flush eyes and had an ophthalmologist complete an evaluation.

An employee injured the thumb on EE's left hand while operating the trap for the vacuum truck hose at the raw mill area. While the employee was trying to clear a clog in the hose, the trap lid fell down and hit EE's thumb causing a cut and swelling. It was elevated and determined to be broken in two places.

Mechanic removing pan from D8 dozer. When Mech removed last bolt holding pan, the pan slipped out of EE's hand pinching left hand between pan and ground, causing cut to hand. 1st Aid provided and sent to clinic for exam. No broken bones or sutures required, but placed on restricted duty for 3 days (no use of left hand).

Employee climbed a fixed ladder to access the top of FM2 DC when they felt a pull in right shoulder. The incident happened around 11am. The initial injury was ruled First Aid for muscle strain (non-MSHA reportable.) EE was placed on light duty to rest. Further evaluation over the past 2 months, lead to physical therapy making it a medical.

Mechanic working in vertical raw mill and had to climb up and down to access separator. Mechanic believes EE strained right arm during climbing. Afterwards EE thought it was just sore and did not report until 3/27/19. Mechanic referred to clinic for eval and was placed on work restrictions, no lifting >5 lbs.

2 employees carrying metal box down stairs. When 1st employee was setting EE's end of box down to ground it dropped approx. 3 inches from the ground while the 2nd employee was still holding. The employee still holding the box was jarred due to impact on ground and injured EE's elbow. On 3/11/19 employee was placed on work restrictions per medical assessment.

PA cut the air off of one of the airlines used to aerate the Fringe silo. Cement had backed up in the air hose and blew out of the hose going under their glasses. PA advised that they had been rinsing their eyes and had the cement out of the right eye but still had cement in the left eye. Sent to ER, given prescription and follow up with Ophthalmologist.

Mechanic was assisting with the installation of kiln main burner pipe. While loosening bolts on a rod, bolt broke loose and clinker dust dispersed into the air. The dust got under safety glasses and into left eye. Eye was flushed and mechanic went to doctor to have foreign debris removed. Had to return on 9/16 due to eye still bothering and was given prescriptions.
