The truck operator was traveling from the 870 level of the quarry to the waste dump with a load of waste material. The truck left the road, overtraveled a berm and continued down the highwall, rolling over before coming to rest int he quarry. It is believed that the truck operator suffered a medical event and as a result was unable to control the truck as it left the road.

EE was using grinder with a grinding disc. The grinder kicked back cutting EE's knuckle on EE's right hand.

Employee was using a plasma cutter on a fab table. The sparks ignited a spray paint can which shot flames causing a minor burn to their right arm.
While performing the workplace inspection, a coworker walking overhead knocked clinker dust down onto employee.
A group of 4 maintenance technicians were doing a team lift to get a wheelbarrow welder up onto the bed of a truck. One of the technicians felt a very sharp pain in their right forearm.

Maintenance was working on the bypass fan to remove the cap on the bearing. A maintenance technician began to hit the top of the cap with a hammer to loosen it up. The cap popped off the bearing and flew up hitting another maintenance technician in the left wrist as EE used EE's left arm to block EE's face.
Employee was traveling through the walkway by the bypass waste fan when EE stepped off the clear sidewalk and reinjured left ankle. This was EE's first day back after injuring the same ankle on 7/30/23.
Employee was working on repairing the cutting edge of the dozer. EE struck the sledgehammer on the cutting edge and a piece of metal broke off and went through EE's glove into EE's finger.
Employee was getting ready to replace belt on reclaimer when the employee injured left ankle when employee lifted left leg to step over a 24""+ high concrete wall. Employee was unable to support own weight and had to be assisted with walking.
An electrician was working up in a manlift basket that would not return to the ground. The Field PCS and the electrician on the ground attempted to bring the lift down on battery power it would not work. After verifying it is safe to jump the lift battery, the electrician was safely brought to the ground under the lifts power. It took about one hour.
While assembling a mobile crane, employee stumbled and fell forward catching full body weight with left arm causing shoulder injury.
At 6:05am on 3/22/23, a call reporting the elevator was not working was made to the control room. Elevator company was called immediately. At 6:16am it was realized that there were 3 people stuck inside. They were released at 07:15am.
The individual has experienced a MSHA reportable hearing shift/loss in left ear. Test results received Feb. 14, 2023.

The Admiral Security Guard tripped in a dip in the road while moving a cone for a truck driver to exit the plant.
The electrician arrived early for EE's shift. EE arrived at 6:40am and EE's shift starts at 7am. At 6:45am, EE was opening a package of brand new tools EE purchased over the weekend. EE took out EE's utility knife to cut the plastic wire tie and the knife slipped and sliced open EE's left index finger.
While placing the jib into its travel position, the employee was folding up the jib ramp and EE's finger was caught in a pinch point causing a laceration.
EE was reaching out welding. A piece of brick fell off the wall and hit EE's right arm and broke it.
Employee was looking for a pry bar and a Lab tech running the 4x4 mule struck the employee.
Pancake style safety tee was opened to remove vacuum from a hose being used to dust but clean the upper building of a silo. Cover plate did not have a locking mechanism to hold open. Employee 1 was cutting hose while employee 2 was dragging hose back in. Hose hit safety tee closing plate 3/4 engaging vacuum. Employee 1 had arm caught in hose approximately 45 seconds to one minute.

While covering a pallet with a plastic wrap bag, the employee waved the bag in the air similar to the way one opens a trash bag. The bag caught the edge of the employee's headlamp that was mounted to EE's hardhat via elastic band. The waving process pulled the flashlight away and then snapped back in EE's face, resulting in EE's tooth cutting all the way through EE's lip.
While working to remove a piece of old vac hose, a production tech cut EE's thumb just to the left of the knuckle.
The employee was walking up a set of open ended steps when EE's left boot got caught on the step causing EE to fall forward. When EE fell forward, EE put all of EE's weight on the left shin. A large contusion formed on EE's left shin.

Employee reports that they were trying to work a rock loose on a conveyor using a metal rod when the rock gave way and right hand came down onto the metal guard hitting right pinky finger. Employee reports that they noticed pain, bleeding and deformity of the finger.
Employee reported they were changing rollers on a conveyor belt & when it broke free the right middle finger was underneath smashing the finger tip, swelling & bruising all around finger. Employee evaluated by medical personnel and splint was provided to prevent further injury.
Abnormal accumulation of pulverized coal under the mill table for an unusual length of time led to coal self ignition and smoldering inside the mill. Incident happened during the cleanup process when fine coal got displaced and suspended by the rotation of the mill table and ignite when in contact with the smoldering coal. The combustion of the coal went from mill to bags
Dust was blowing out from an inspection lid on an air-slide. When employee attempted to reseal the lid dust blew out and went under employee's glasses into employee's eye.
An employee that had been exposed to UV light during the workday was taken for treatment after experiencing eye pain and blurred vision.
Employee was working on clearing the kiln feed end seal dust chute when material flushed out and got into EE's right eye.
An employee working in the warehouse was unloading the palletizer with a forklift. While waiting for the pallet to fill, the palletizer table stuck in the upwards position. Once stuck it started dumping bags, so the employee stopped the system to clear the bags. When the EE picked up the first bag off the rollers EE felt a sharp pain and tearing feeling in EE's lower middle back.
An employee was installing new cages into a compartment in the main baghouse. Employee lost footing and fell landing on wrist trying to catch themselves.
While working on seal sleaves in a kiln, the employee lifted the insulated cloth to position it. Dust fell from the cloth onto employee's eye. Employee was wearing safety glasses. Employee rubbed the eye with hand scratching the eye. It was flushed at the site but employee asked for Dr and the Dr rinsed it and no foreign object was found. Eye dropes were prescribed.

An employee injured the thumb on EE's left hand while operating the trap for the vacuum truck hose at the raw mill area. While the employee was trying to clear a clog in the hose, the trap lid fell down and hit EE's thumb causing a cut and swelling. It was elevated and determined to be broken in two places.
While working on replacing the lid for the bypass baghouse, the lid spun around, landing on the top of the employees foot. The employee sustained a large bruise on the top of EE's left foot.
Employee climbed a fixed ladder to access the top of FM2 DC when they felt a pull in right shoulder. The incident happened around 11am. The initial injury was ruled First Aid for muscle strain (non-MSHA reportable.) EE was placed on light duty to rest. Further evaluation over the past 2 months, lead to physical therapy making it a medical.
Pulling on wrench in upward motion on push bolt to take sleeve out of pump.
On 2/5/19 a few minutes to 8am, the employee walked towards the pack house. EE stopped to talk to a contractor. When the conversation was done, EE went to exit the area onto the sidewalk. EE did not lift EE's foot high enough to clear the rail causing EE to lose EE's balance and fall. The employee immediately reported the incident to EE's supervisor.

Worker has been working in Airslides and the cement dust irritated arms and created a rash. EE was prescribe a medication to help with irritation.
Operator was setting up to dump Vac Truck when the vehicle began rolling. The operator tried to get back into truck to stop it when the operator fell off and broke leg. Truck rolled into a berm and stopped.
Mobile Mechanic was working on an excavator removing a cover plate to access hoses when the plate stuck on one side making the Mechanic's hand slip and drop plate then pinching thumb on other side between cab and plate
Miner was entering an elevator when the door shut unexpectedly hitting their head and causing discomfort on neck.

Employee was clearing a blockage in the stage 5 duct and the woma gun slipped out of hand allowing it to move around due to the trigger potentially being stuck. Causing a puncture and laceration wound to right and left thigh.
Employee was moving a piece of angle iron in a walkway 6""x2""x4', while picking it up and setting it down the material got away from them and they got their left middle linger smashed when it was caught between the metal and the surface. Split finger nail, no other injuries except sore.
Employee was cleaning out clinker cooler by tossing large chunks of material onto a chute take the material outside. A gust of wind blew back through the door bring dust back inside into the face of the laborer. Laborer has dust mask on, safety glasses on and indicated had goggles on.
Employee was working on a spillage chain, had to get in the housing. Once inside had to bring a heavy piece of channel iron in the area to complete the repair planned. Employee had no injury on 4/8 and had no symptoms. The employee reported the alleged incident on 04/11/2017.
Pan conveyor guard was being secured by co-worker via wire strand. Wire strand broke and guard went down steps hitting employee in the face.
Employee was changing wire rope on overhead crane when fingers were pinched between the drum and the wire rope.
Electrician's were attempting to engage a 4160 breaker, throw would not move, risk analysis determined they would have to look at throw. Went upstream to LOTO at power source feed to breaker located in Main Sub, located the power source, Loto'ed the power source. Went back to the ER with the breaker. Opened the box and a employee came in contact with energy inside the box.

Employee came in early to help with a mechanical problem this was fixed, moving bags of ice melt off pickup bed, turned took a step and felt a sharp pain in the back.
Employee was cutting legs on a cross-over while kneeling. The 6"" grinder wheel kicked back and cut right knee. Employee received medical attention to close laceration with stitches.
Employee was inspecting conveyors belts, found a froze idler, found a bar and tapped the frame of the idler several times and the bar slipped after several attempt, catching the tip of the bar on the next roller downstream, approx. 18"" away, started to pull the bar in and the bar was forced upward into the frame which smashed the employees thumb between the bar and the frame.
Employee was working in the finish mill building cleaning out a blockage in FM1 step chute for approximately 10 hours using a jackhammer. Jackhammer caused dust to stir from the material they were working on. The employee had cleaned self off several times and changed clothes twice throughout the day. Employee noticed the burn the next am before work.
Employee was sampling cement from the air slide on FM 2 with safety glasses, hardhat and dust respirator on when there was a pulse of material through the air slide. Material flew out of the port and into the face of the employee, getting into right eye. EE immediately washed eye out and went to the ER for treatment.
Employee was helping pull a breaker out of a panel when the bucket release became stuck and employee jerked upward and strained employee's back. employee was checked out at clinic and returned to full duty.
Employee was removing a lower air slide from under hopper when it slipped and fell onto EE's hand smashing it between the air slide and pipe below.

Employee was walking up a set of stairs and in his words, ""tripped over his own feet"", falling forward getting a finger caught in stair grating
Unloading truck for work. Chain from chain hoist, caught on vehicle bumper while wrapped around hand and pulled on wrist.
When employee went to retrieve the piece of metal in FM 1 rotary feeder the ram slipped off the feeder vane, rolled back, and pinned said employees left thumb between the feeder and the feeder housing. The ram had to be replaced to back the feeder off in order for said employee to remove his thumb.
Worker was assisting in the assembly of an air slide. Sections were being lifted into place by a chain fall from above. When lifted the air slide shifted and pinched his hands.
Employee was air lancing next to a bulk truck loading snorkle and material blew back up into face and got under safety glasses. Employee was not wearing the proper PPE for the job task. Goggle or face shield are required.

Spontaneous combustion of rough raw coal. No damage or loss to any equipment, no injuries, no illness as a result of this incident. No interruption in plant operation. Employee was cleaning up material in the by pass compressor room, went to pick up a 1 gallon of metal parts and twisted his back, then causing a small pain. Employee told his immediate sup. Employee continued to work several days, on 11-10-14 employee requested to seek medical attention. Sprain in lower back.
Employee was driving a bearing into an end cap housing. Employee injured was using a steel plate to drive bearing and swung a 3-4 lb dead blow hammer, forgot where his right thumb was and the hammer he swung strongly and hit his right thumb. Employee was taken to ER. Employee was released to light/ restricted duty.
Worker was attempting to close an inspection door on the pre-heater tower using a sledge hammer. He was hitting one of the hinges with the sledge and missed the hinge causing his pinky finger to be caught between the door and the handle.
While troubleshoot a cement flow problem, the employee proceeded to close a manual gate from the silo that fed a pneumatic gate. The employee unbolted an inspection cover on the flange that connected the manual gate to the pneumatic gate. Upon unbolted the cover plate, cement shot into his face due to incomplete closure of the manual gate.
A co-work found a worker in an apparent state of heat exhaustion. Worker had been performing his assigned clean-up tasks and apparently became ill due to dehydration.
Employee, while wearing proper PPE was in the process of removing a sensing meter from a lime slurry system when he accidentally got lime slurry in his eye requiring flushing. After employee got lime slurry in his eye he became startled, lost his balance, and fell over to the ground straining his back.
Employee was going to clean out the cement air slide under cement silo and accidentally twisted his left ankle in a small trench like trough that runs under the air slide.

Began working in Cumberland Coal storage area (CCSA) Tue Apr 2. Smell indicated something going on but no fire determined. Previous week smoldering coal removed from ARJ storage side. Smoldering & red coals found in CCSA after access was made Apr 5. Crane used to move coal to center to use with kiln start Apr 6. Kiln didn't start. Open flame detected Apr 8 w/MSHA Inspector.
Employee was using a hammer drill to drill refractory. Drill caught and spun out of operators hand striking him in the cheek causing a small laceration. Employee received stitches. Laceration occurred at the end of shift, employee returned next day to work.

Accident occurred while demolishing a retired/abandoned building structure in an area isolated from the rest of the plant at shut-down surface area. Excavator slid laterally on concrete floor surface sliding onto lower level area. Excavator landed on its side. Operator only involved in accident and sustained fractured right arm and broken ribs.
Was putting a pump into the day tank in #2 grinding aid room and strained my back.
Injured employee was standing next to a coworker that was striking a chisel with a hammer and was struck in the back of the right hand with what appears to be a small sliver of metal. This resulted in an embedded object on the back on of right hand.
EE was leaving the warehouse 1 location going to lunch. She was walking pass the big hanger bay door and tripped over the forks on the Grade-All which were pointed towards the hangar bay door. EE could not get up immediately after she fell so she crawled to the side walk and set there for approximately a minute and then got up and proceeded to walk back to her office.
Employee was loading steel into a sea container while load was being slid into position inside the container it pinched employee's left lower leg against the side of the container.
Employee was in the process of cleaning out the step chute on FM1 from the 014 East/West belt. The employee looked through the inspection door when the blaster that was installed to keep the chute clear discharged throwing material in his face. Even though he was wearing his safety glasses some material entered his left eye.
A fire was discovered by Production Control Supervisor in the 001 Pan Conveyor Gallery at 22:00. The PCS called the local fire department at 22:10 due to amount of fire and smoke in the gallery.
Employee was working on Preheater Tower spout for dust unloading, spout would not move so employee tried to move in manual. Dust fell from spout area and covered face/eyes of the employee. His left eye was covered in dust. Employee flushed out left eye, could not get all dust particles out of his eye. Employee was taken to MedExpress and was given antibiotic drops for his eye.

Prior to erecting scaffold a ""headache"" deck was being installed to ensure no debris could fall on the crew. As the work was being finished a chuck of debris fell from above striking the employee on his lower right trunk.
Employee was working at old truck plant on the airslide to C-bin. All the air supply to the lines was turned off. Checked pressure by feeling line, removed air hose, material build-up in end of hose could not be detected. Material pushed from hose by air blowing material into the face and eyes of employee. Employee was wearing the safety approved glasses when working.
EE was working on cleaning the area of the clinker silo pan conveyor. The pan conveyor was down for most of the day & had restarted near the end of the shift. EE was at west side of pan conveyor gathering up tools at the end of the shift when he placed his hand on the pan conveyor rail & wheel of conveyor rolled over his fingers. All 4 fingers were injured.

Employee was using woma gun on the feed shelf to clear buildup in the tower. Coating (buildup) fell, hitting the gun barrel lifting the employee off of his feet causing him to fall into the scaffold when landing. He noticed a popping feeling in his right hip making it difficult to walk.
EE was usging a dewalt 3/8 drill to drill hole in steel. While using 5/8 bit the drill got stuck and twisted, catching employees right hand and breaking a small bone in hand.
After removing tools from back of truck EE was closing tailgate of the truck with his right hand on top of tailgate when the top door of cap fell, pinching his finger between top door and the tailgate.

EE was walking on unpaved snow covered road to clinker silo for sample when he stepped into a hole and twisted his left knee. EE was able to perform regular job duties unitl surgery which is scheduled on 02/09/2010.
EE was troubleshooting an electrical issue with compressor, removed access panel to access relay inside cabinet. Upon job completion, EE reinstalled access panel and felt a tear in his right shoulder. On 12/21/09 EE was breaking loose a large icicle from air cannon, thus aggravating shoulder. EE was able to perform regular job duties and surgery is scheduled for 1/15/10.
EE had slipped and fell 1-2 feet trying to decend to the next level of planking. Instant swelling had occcured at the time of the accident. Proper equipment was present for a safe decend.
Employee was walking from point A to point B on uneven ground and lost his footing. He never fell but tweaked his lower back causing a muscle strain. Employee went to panel physician and was put on restrictions.
While cleaning sprayer to apply stain the hose lifted out of the bucket and sprayed acetone on the left side of EE's face. EE immediately flushed his face with bottled water and reported to the job trailer for eye wash solution. EE contined to experience burning in his eye and was taken to the emergency room for treatment.
Employee on ladder attempted to repair wire insulation while energized. This type of work is contrary to site and contractor safety procedures. The employee caused an arc flash to occur which caused the employee to fall off the ladder and injury their right arm.
Choker on crane hit the crab handle knocking it off of the crab, the crab handle fell approx. 10 to 12 feet, striking employee on the back of his hard hat.
Employee was flipping a brace, using forklift. While flipping, the peice slipped off forklift and pinched employees right index finger between peice brace and gurter.
Injured employee was grinding steel pipe, and got metal dust inside of right eye. Employee was using PPE at that time.

EE was lifting floor tile on raised floor, tile slipped out of his hand and smashed middle finger of left hand, received 6 stitches in finger, returned to work that day. Root cause was EE did not clean off tile and apply suction cups to lift tile.
Injured employee was welding and grinding and debri got inside of right eye, employee was using safety glases and face shield.
Employee got a piece of welding slag in his eye from his glove when he wiped sweat from his forehead while still wearing his glove.
Employee was unpacking new hydraulic lines from a crate, when the material in crate shifted and the employes finger was cut by sharp edge on metal bracket requiring 5 stiches.
Employee entered office trailer and upon stepping off of carpeted runner, slipped on floor and sprained ankle.
Employee was rotating coupling when finger slipped and entered hole, causing finger to be caught between coupling halves, causing injury to finger tip.
Injured employee was using a crab drill to bring up a beam, then the drill jammed and hit employee's right hand.
Team member was pulling demo temp support angles which were 3"" x 3"" x 1/4"" angle 6' long from a pile of support braces. One support brace had a 2' section welded at the end to make a 90 degree angle. He pulled on brace which was caught on another piece. He continued to pull when he felt something wrong in left shoulder. Team member reported to his supervisor.
Employee was wearing a harness. As he stepped over a toolbag laying in his path, the lanyard swung under his foot and he tripped and landed on his left knee. He requested to get checked due to pain. He was sent to provider, x-ray was negative, he was treated and released. However over the course of the following 8 days it became a lost time.
The injured employee was moving a large step ladder and experienced pain in his groin area.
The injures employee was adjusting a conveyor belt when he accidently placed his left hand on a moving roller. His had was pulled underneath the roller crushing his hand.
Injured employee attempted to assist two other employees in removing a form from a silo opening and as he was backing up he stepped into a hole 14"" diameter X 6"" deep, spraining his ankle. Form boards fell striking employee's ankle.

Employee was meeting with his crew near the material storage container. When he finished he turned and walked into the door on accident. When his face hit the door, it pushed his safety goggles up and he suffered a laceration about his eye.
Injured employees lanyard was attached to a retractable(yoyo), retractable stuck on a Rat Line, employee pulled hard to release it and then ducked. Employee struck his head on a piece of rebar, causing a half inch laceration to his forehead.
Employee was reaching for a cable while working off a wall and strained his lower back.
The injured was using a grinder to grind off a pin on a cyclinder. Approximately two days later, 1-8-09, the injured experienced discomfort in his left eye. The injured sought medical attention and had a piece of metal removed from his eye.

on 12/21/2008 Injury sliped on an ice patch while inspecting the crane he was going to be operating. He reported the incident to his superviser as a report only incident. On 1/16/2009 He had an MRI because he started to feel more pain. The MRI revealed damage to his left shoulder.
Two team members working together to remove the angle iron bracing from inside of the ductwork. With angles about 7'-0"" in the air. One team member was supporting the angles as they were being burned out. He lost control & it fell hitting his shin.
Employee was attempting to flip an ""H"" beam over, when he felt a pull in his back.
Welding inside kiln, weld splatter got behind kneepad and employee had a minor burn in a small area just below kneecap. The burn later became infected and he went to ER to get cleaned.
Employee was maneuvering a pump hose from a concrete pump truck and as he went to push the hose in one direction the operator moved it the opposite direction and the employee strained his lower back. EE was given medication and put on 20 lb restrictions.
Employee was working with a crew building a transfer tower for a new crushing plant. There was a floor opening on the first work platform. Employee was taking measurements of the opening. He stood up and fell through the opening to the ground fifteen feet below.
Worker was trying to seperate stacked conveyor cover supports. The supports were still stacked from delivery. While attempting to seperate the first two supports, the supports snapped back to the origional position causing a fracture to his left middle finger. His finger nail was torn off as he quickly pulled his hand away.
EE was getting tools from side box of truck. He placed tools on ground, shut box and thought he had the box latched. He picked the tools up, stood up and the tool box door fell open catching him across the forehead just below the brim of his hard hat, cutting his forehead about 2 inches long and requiring 5 stitches.
Employee attempted to set steel support piece for boom assembly on a crane, when 4th finger from left hand got caught between the steel support piece and boom assembly.
Miner was removing wooden 2x4 plates with a pry bar. While striking the pry bar, a metal shard chipped off of the striking surface of his hammer shot in his hand through his leather work glove. The shard was removed at the hospital and the miner returned to work.
Employee was spreading steel beams for the upper level to be hoisted by the crane, beams shifted and caught employee's 3rd finger right hand.
Team states that while taking a break sitting on the lid of a gang box (greenlee), EE thought his hand was in the rolled edge area (used to open lid), but in fact his hand was between lid and box. Pinch point lid struck open and closed shut when co-worker got up.
Sitting in a crouched position, the team member was adjusting flow meter for argon gas mix at manifold header. The TM stood straight up and made contact with the crossmember brace in the rubb building, resulting in a cervial stain.
Employee was lifting an 8x2 form panel (80lbs) and strained a muscle in his lower back.
The employee was prying open a crate, it was raining at the time. His hand slipped off the pry bar and hit the crate, wedging his hand between the bar and the crate.His hand was sore but there was no visable damage, swelling or bruising.
Team member had to air arc a misalignment on a calciner piece, after air arcing, he took a short handled 3# hammer and started banging on the piece to see if the weld was cut all the way through. on the 5th swing, he pinched his lettle finger between the steel and the hammer handle.
Organzing wall forms neatly to relocate with the lull. EE was manually shifting the wall panels that weighted about 250lbs.

EE rcited rack truck and walked toward connex box. He fely a stingling pain in his left hand as an excavator moved past him. He noticed a small cut and blood on his left hand. A small silver of steel 1/8"" came off of the excavator track rollers and embedded in his hand. He was approx. 10 feet away as it passed.
An air slide fell on my hand/finger. I was lowering an air slide down to replace it and it pinched my finger. I thought I was fine but after work my fingertip began to swell so I sought medical attention.
EE was unstrapping a section of crane boom from a trailer. He rolled the strap on the trailer winch and then hooked it to the side rail. He then put the bar in the winch to tighten the strap. He states something slipped, unsure if his hand slipped from the bar handle or his feet slipped in the gravel. He fell forward striking his face and nose, needing stitches.

EE was desending flight of steps, holding onto the handrail, when his foot slipped and he slid down 3 steps and injured his shoulder. Restricted duty.
Employee stepped on a pipe in his work area and twisted his ankle. (Restricted duty.)
Employee cleaning out CKD tank with vac truck. Employees were entering tank using 26' extension ladder while tied off to hoist. While exiting the tank EE had one foot out of the tank and one foot on ladder when hoist cable broke & shackle fell on his knee.
Employee was in the 2nd compartment of 9 cooler for approximately 10 minutes on his hands and knees inspecting the cooler. After exiting the cooler, rising to his feet he felt a sharp pain in back - restricted duty starting 10-27-2006.
Employee was walking on top of truck plant when he tripped over a cable that had come out of channel lying on floor to protect cable. Employee injured his shoulder & had shoulder operated on 12/26/06.
Employee shoveled around and under feed belt for a couple of hours in a stooped position. He later complained of sore back.
EMPLOYEE WAS WELDING AND CHIPPING WHEN SLAG ENTERED RIGHT EYE.
Employee was working with a hydraulic rotary drill and place hand around the hammer on the drill. His small finger was between the hammer and the drill bit and the operator released the hammer causing it to come down to the drill. The left little finger was crushed. (The entire finger was later amputated).
The employee was using a small jack hammer on lumps in a bin. Dust got on his arm and in his eyes causing a chemical burn. (Restricted duty the next day).
Employee carried a full tank of propane across firing floor (approximately 60 ft) and installed it on a forklift, straining his back. Initially went to doctor, was treated and released. Did not miss work until 8/20/2006.
Employee was working in cooler with vac hose. Area was warm and Employee suffered from heat exhaustion.
Complacency walking down set of narrow steps that the EE had traveled numerous times before. This time his foot slipped off the front of the step, causing the injury.
Employee was working on belt installing rollers. While working on a platform above the belt, he stepped into a hole in the platform causing a small abrasion. 5 days later he had an infection in his leg & went to the hospital. He began missing time on 5/9/06.
Employee was jack hammering at #2 drag while 2 other employees were vacuuming with vac hose. The vac hose became plugged. When vac operator unplugged vac hose, the hose jumped, latched on employee's leg. He pulled the hose off his leg and it grabbed his arm.
After Replacing belt on metal roller conveyor belt the employee and another repairman were tracking the belt. The other employee asked EE to hand him a wrench across the belt. As he handed the wrench across the belt with his right hand his gloved left hand was pulled into the roller.
EMPLOYEE WAS USING HOSE FROM VAC TRUCK IN CLOSE QUARTERS BETWEEN 2 & 4 DRAG. HE STATED THAT HE STEPPED ON UNEVEN SURFACE AND TWISTED HIS ANKLE.

Employee was climbing down from east crane trolley when he felt a sharp pain in back of left knee. Experienced discomfort in knee but had no medical attention until 2-25-2006 when pain got worse. (Began missing work 2-27-2006).
Calibrating #10 clinker fow. Test chain (74 lbs) was being placed on storage hook at #7-8 fow area. The test chain got stuck on glove. He let go of chain thinking it was hooked. Chain fell from hook twisting & pulling left arm down & back. Surgery May 22, 2006.
Working on slurry pump. opened door and found runner broken removed runner for further inspection and sat it on the floor a few moents later the ee put his hand on the runner to move it out of the way at that time the filing plate fell off dial pins cutting the ee s finger.
A lump was stuck in a feed chute. The employee was using a bar to strike the lump trying to dislodge the lump. The employee struck a flange with his hand on the bar fracturing his right index finger.
EE was inspecting area around 19 slurry tank for leaks. He stepped in wet material, slipped and injured his knee.
Employee was installing a door on a mill. He had a chain jack hooked on the mill door and to an overhead crane. Too much pressure was applied by the crane, bending the hook on the chain jack, causing it to slip off the door striking the employee on the cheek.
THE #18 SLURRY TANK RUPTURED RELEASING ABOUT 862,000 GALLONS OF SLURRY THROUGH OUT VARIOUS LOCATIONS OF THE PLANT
EMPLOYEE WAS OPERATING OVERHEAD CRANE ON A HOT EVENING, THE EMPLOYEE BECAME OVER HEATED.
AFTER CLEANING GRIZZLY AND REPLACING DOOR, THE EMPLOYEE TURNED TO WALK AWAY. HE STUMBLED ON LUMP OF MATERIAL AND TWISTED HIS BACK.
While walking to job, stepped on mill ball and injured knee.
EE TRIPPED ENTERING DOOR IN COOLER. HE FELL ON RIGHT SIDE, INJURING SHOULDER. THIS INCIDENT OCCURRED ON 5/7/05 - NO LOST TIME. ON 5/15/05 EE AGGRAVATED THE INJURY WHILE ATTEMPTING TO OPEN GATE ON SKID STEER LOADER. BEGAN MISSING TIME ON 5/16/05.
Reading sensor from pipe by sliding it down to lower flange, flange had 4"" to go when sensor moved down. Employee went to support it as it continued down thus catching his fingers between it and the flange. The injury required 9 stitches.
(2 EE's were injured.) Racking a discharged circuit breaker back into electrical panel. Just before insertion a click was heard & then an explosion took place that blew a hole in the front of the panel. EE received burns to face and hands (1st degree; treated & released at ER). Elec. panel is inoperable.

EE#2 REMOVED THE TIES SECURING THE FEED LINES AND ALLOWED THE ENERGIZED LINES TO FALL ON A METAL BRACKET CAUSING AN ELECTRICAL FAULT AND BURNING THE CONTRACTOR.

EE WAS REMOVING BOX FROM UPPER SHELF THAT WEIGHED APPROXIMATELY 70 LBS. WHEN HE PULLED BOX FROM SHELF HE FELT A TEAR IN HIS LEFT SHOULDER. DID NOT BEGIN MISSING WORK TILL 2-3-2005.
Employee has a history of back problems. During the week of 11/8/04 he worked many hours welding in tight quarters. By 11/13/04 the employee's back was hurting.
CARPAL TUNNEL, RIGHT HAND. REPETITIVE USE OF HANDS.
Employee was washing oil and dirt off concrete floor with water hose. after spraying an area he stepped on the area he had sprayed. there was oil residue where he stepped. ee slipped and sprained his ankle. returned to work 7-22-04 did not report to work 7-27-04.
WHILE SITTING IN THE OPERATORS CHAIR THE EE TURNED TO ENERGIZE THE CRANE BY THROWING A 600 AMP BREAKER. WHILE TURNING & PULLING ON THE BREAKER HE STRAINED HIS BACK. THE BREAKER IS LOCATED BEHIND & TO THE RIGHT & APPROX. 12"" LOWER THAN THE OPERATORS SEAT.
Overhaul on primary crusher, employee was covering crusher with tarp when he slipped and fell into hole where crusher is to go.
Employee rubbed his hand over a used blade for a pump Piece of metal/ fiber punctured small finger.

EE was pulling on drag chain that was broken. Was away from work 1-25-04 (report filed). Started missing work on 3-30-04.
EE WAS HOLDING A DRIFT PIN FOR A CO-WORKER, WHEN THE CO-WORKER MISSED THE DRIFT PIN. HE HIT EE'S FINGER (THUMB) AND BROKE HIS THUMB.

EMPLOYEE WAS SWINGING A HAMMER, STRIKING A WEDGE IN A CRANE BUCKET TO KNOCK IT OUT. EMPLOYEE STATED THAT HE FELT HIS RIGHT ELBOW POP.
EE WAS OPERATING VAC TRUCK. WITH THE VAC TRUCK RUNNING, THE EE TRIED TO DISCONNECT TWO SECTIONS OF FLEX HOSE. WHEN THE HOSES WERE DISCONNECTED THE EE'S LEFT ARM WAS SUCKED INTO THE HOSE UP TO HIS SHOULDER.
THE EMPLOYEE WAS WORKING ON A TRUCK ON A LEFT. EMPLOYEE USED A WAREHOUSE SAFETY LADDER TO ACCESS THE TRUCK. AS HE EXITED THE TRUCK HE STRUCK HIS KNEE ON THE SHOP LIFT INJURING HIS KNEE.

WHILE ATTEMPTING TO CLIMB INTO 50 TON TRUCK, FEET SLIPPED OUT FROM UNDER EE. HE FELL BACKWARD TO GROUND.
EMPLOYEE WAS MOPING THE FLOOR & STRAINED HIS LOWER BACK.

EE WAS PERFORMING AN AIR TEST. AS HE WAS POURING SAND IN THE MIXER HIS HAND SLIPPED AND HIS INDEX FINGER ON LEFT HAND WAS CAUGHT IN THE MIXER.
WHILE LIFTING A 25' SECTION OF 6"" ALUMINUM PIPE EE FELT A PULL AND A POP IN HIS UPPER BODY. THE CONDITION HAS CONTINUED AND ON MARCH 25, 2004 EMPLOYEE HAD SURGERY.
INJURED WAS PRESSURE WASHING THE ROCK BREAKER. AFTER CLIMBING DOWN FROM THE ROCK BREAKER TO REFILL THE PRESSURE WASHER WITH WATER, INJURED FELT A SUDDEN SPASM/PAIN IN HIS LEFT ABDOMINAL AREA. INJURED EXPERIENCED PAIN, BUT CONTINUED TO WORK. THE PAIN INTENSIFIED AS HE GOT HOME THAT EVENING.
INJURED WAS SITTING ON A WOOD BENCH IN THE LOCKER ROOM DRESSING HIMSELF FOR WORK, HE GOT A SPLINTER IN HIS RIGHT RING FINGER FROM THE WOODEN BENCH. A DR. REMOVED THE EMBEDDED SPLINTER AND PUT STITCHES IN HIS FINGER.
INJURED TWISTED LEFT KNEE WHILE CLIMBING STEPS/ LADDER TO CHECK A MOTION SENSOR ON THE EAST CLINKER ELEVATOR. INJURED HAS BEEN PUT ON WORK RESTRICTED DUTY UNTIL SWOLLEN KNEE HEALS AND THE DOCT OR EVALUATES THE CONDITION OVER THE NEXT COUPLE OF WEEKS.
IE WAS USING A SLEDGE HAMMER TO DRIVE LOOSE A LINER IN BALL MILL. THE LINER CAME LOOSE, BOUNCED OFF SIDE WALL OF MILL STRIKING EE IN LEFT LOWER LEG, CAUSING A COMPOUND FRACTURE OF LEFT FIBULA.
INJURED WAS WALKING DOWN A ROADWAY WHEN HE STEPPED ON SOME CLINKER BALLS IN THE ROADWAY. HE FELL HITTING HIS BACK ON THE GROUND.
INJURED WAS WALKING DOWN SNOW COVERED HILL WHEN HIS FEET SLIPPED OUT FROM UNDER HIM. INJURED WAS DIAGNOSED HAVING A CONTUSED KNEE WHICH REQUIRED SURGERY. INJURED HAD KNEE SURGERY ON 5/22/03.
DATE OF ACCIDENT ABOVE IS THE DATE THE EE STOPPED WORK FOR CARPAL TUNNEL SURGERY. REPORT NOT FILLED OUT THEN BECAUSE WE DID NOT KNOW IT WAS CLAIMED AS WORK RELATED.
