Standing at ground level putting a bar back on grizzly screen. Was trying to guide the bar in to the bolt holes, lost footing and fell back to the ground and the bar fell on employee. Lower back/butt sore.

The injured employee was stocking plate steel in the storage area in the fabrication building. The employee was using their left arm and hand to support 4 sheets of steel. The sheets of 3/8" steel that the employee was bracing with their left arm shifted, falling back towards employee's arm causing the employee's elbow to strike the steel plate on the other side of the rack.

Employee was preparing to line up a bolt on the wash screen. The bonnet of the wash screen moved forward which resulted in the employee "jerking" their hand away, which resulted in the employee suffering a laceration to their right index finger. The employee received 7 to 8 sutures to close the wound.

"Pulled up with Terex (26.0012) to fuel. Employee removed the fuel cap. Stuck the fuel nozzle in and went to turn pump on. Went back to location and squeezed the trigger and the nozzle popped out and sprayed ee on the top of the hard hat and it entered into his right eye".

Injury was lowering a 400 watt flood light fixture which was attached to the top of a 10' piece of 2" rigid conduit. After loosening the jamb bolts, and removing the pin that holds the pipe in place; the pipe slid through his hands and before he could stop the pipe and fixture from slamming down it smashed the tips of the two middle fingers on his left hand.

Employee & co-worker were patching a railcar from underneath the car, it was a tight spot, when 2 small pieces of rust got behind his safety glasses & became lodged in his eye. We were unsuccessful in removing the specks with water so he was taken to an eye dr. who removed it. He was prescribed eye drops as a precaution for any eye infection. He then returned to work that day.

Employee was clearing a rock that had become wedged between the tailgate & the bed of an R-65 Euclid haul truck using a long pipe. When the rock came loose it fell onto the tire below & bounced towards EE hitting him in the face. He was taken to the ER where he received 10 stitches to his upper lip.

EMPLOYEE WAS BALANCING AN 18" I BEAM WITH ANOTHER PERSON ON THE OTHER END. THE BEAM WAS ON THE GROUND. WHILE A LOADER WAS TRY TO CURL THE I BEAM INTO ITS BUCKET THE I BEAM TIPPED OVER. IT PINC HED ONE EMPLOYEE'S LITTLE FINGER. THE LITTLE LEFT FINGER TIP WAS PARTIALLY AMPUTED. THE FINGER REQUIRED STITCHES AND THE EMPLOYEE REPORTED BACK TO WORK THAT DAY WITH LIMITATIONS.
