Standing at ground level putting a bar back on grizzly screen. Was trying to guide the bar in to the bolt holes, lost footing and fell back to the ground and the bar fell on employee. Lower back/butt sore.

Employee was returning to the pit from primary. As EE neared the bottom of the long ramp, the auto retarder engaged which caused EE's shoulder belt to lock up and slip up EE's neck. Strain to unspecified parts of the neck.

Employee was cutting a piece of conveyor belt with a pocket style utility knife. The knife slipped out of the belt and struck the employee in EE's left knee lacerating it. The employee received 4 sutures to close the wound.

Employee was on the 8 wash screen changing poly decks. Employee was using a screwdriver to pry pin out of the poly screen. Screwdriver broke and the employee fell forward into the broken screwdriver puncturing their left wrist.

The injured worker was preparing new high voltage cables to install connecting lugs. While using a knife to trim the insulating jacket from the cable the injured worker cut EE's left hand when the knife slipped from the cable. Five sutures were used to repair the laceration.

Employee was working under wash screens to pull the drain plug out of the concrete screw. The drain plug was frozen and the employee walked back to the maintenance truck to get tools. EE slipped on the ice and struck their head on the ground below.

Cleaning and entered an area where sand / fines fall onto the floor. Employee stepped onto a plate that covers an opening. Plate flipped came back down and EE's foot slid and the plate turned in the opening causing EE's foot to be pinched between the plate and opening.

Employee Statement - Put tubes out. Had one remaining and took it to stemming pile where extra tubes are kept. Walking back to drill, I slipped on some stemming #6 stones that were on top of smooth bedrock. My right foot went up and my left foot ended up underneath me. I felt my left ankle pop.

Welding in S-3 discharge chute. Lowered piece of 16" x 2' patch down to the tacked into place. Was then welding in chute between EE's legs and sparks caught pant leg on fire.

Employee was cleaning around the plant in a skid steer working around the surge pile tunnel at the quarry, the weather was wet and the employee was dumping a bucket load of wet sand on to a spoil pile near the tunnel. As the employee dumped the load of material it splashed back and the material splashed on the employee's face running down behind their safety glasses.

The injured employee was stocking plate steel in the storage area in the fabrication building. The employee was using their left arm and hand to support 4 sheets of steel. The sheets of 3/8" steel that the employee was bracing with their left arm shifted, falling back towards employee's arm causing the employee's elbow to strike the steel plate on the other side of the rack.

There was a buildup of frozen sand on the rail car door that two employees had to remove in order to properly close the door. After the material was removed, an employee inserted a spud bar into place on the car door and began to pull the bar towards self with a staggered stance. While pulling the spud bar back, the employee felt sharp pain around ankle.

Employee was preparing to line up a bolt on the wash screen. The bonnet of the wash screen moved forward which resulted in the employee "jerking" their hand away, which resulted in the employee suffering a laceration to their right index finger. The employee received 7 to 8 sutures to close the wound.

An employee working for a contractor was using a 6" inch DeWalt angle grinder on the corner of a new cold fee bin to prepare to weld. When he began to grind the surface in a vertical fashion, the grinder "kicked" back at the employee striking him above his right eye and on the right side of his nose.

"Pulled up with Terex (26.0012) to fuel. Employee removed the fuel cap. Stuck the fuel nozzle in and went to turn pump on. Went back to location and squeezed the trigger and the nozzle popped out and sprayed ee on the top of the hard hat and it entered into his right eye".

Injury was lowering a 400 watt flood light fixture which was attached to the top of a 10' piece of 2" rigid conduit. After loosening the jamb bolts, and removing the pin that holds the pipe in place; the pipe slid through his hands and before he could stop the pipe and fixture from slamming down it smashed the tips of the two middle fingers on his left hand.

The water level in the settling pond increased higher than past years and washed out over the top of the berm releasing approximately three feet of water from the pond, leaving the ground level at 4" - 6" deep in the immediate area. No employees working in area. New overflow pipe installed and the dike has been repaired.

Employee & co-worker were patching a railcar from underneath the car, it was a tight spot, when 2 small pieces of rust got behind his safety glasses & became lodged in his eye. We were unsuccessful in removing the specks with water so he was taken to an eye dr. who removed it. He was prescribed eye drops as a precaution for any eye infection. He then returned to work that day.

EE was cleaning up after a shot and he didn't have his buckel all the way down on the ground, He ran over a rock. That made his upper body twist and his head hit the center post in the cab. He cut his head on the left side and had to get 2 staples in his head. I took him to hospital. He returned to work with out restrictions.

Employee was clearing a rock that had become wedged between the tailgate & the bed of an R-65 Euclid haul truck using a long pipe. When the rock came loose it fell onto the tire below & bounced towards EE hitting him in the face. He was taken to the ER where he received 10 stitches to his upper lip.

EE drove his truck down to the lower bench and stopped his truck 100' behind and to the left of thr 992G Cat loader, in a blind spot wher the loader operator (injured) could not see him. Not knowing truck was in area injured cont. to back drag floor for 2nd operation. Truck driver seen loader backing up darted truck forward in path of loader hitting tailgate of truck bed.

While working on front-end loader with a wrench in his left hand he reached down to loosen a hose nut, when it broke loose his hand moved across a section of the hose that had a double clamps with one end sticking up and one down. The top of his hand racked across a sharp end of a clamp end cutting it to the point to require stitches.

While backing an R65 Euclid truck up a ramp to dump fill material, the left rear tires went into a low spot causing the truck to dip. Employee said this caused him to be thrown against the door handle of the truck. We do not know if he was wearing a seat belt. This injury did not become reportable until he had surgery on Sept. 29, 2006.

THE DRIVER WAS ATTEMPTING TO CLOSE THE SLIDING WINDOW ON THE WATER TRUCK FROM THE SEAT & AFTER BEING JAMMED ON THE 1st ATTEMPT, PULLED HARDER ON THE 2nd ATTEMPT & THE WINDOW BECAME FREE & THE DRIVER LOST HIS BALANCE & FELL OUT OF THE CAB ONTO THE GROUND LANDING ON HIS FEET & THEN FELL FORWARD ONTO A PALLET OF SCREEN CLOTH CUTTING HIS LEFT KNEE.

While climbing up into the terex haul truck (45 ton #26.8939) His right foot slipped off the step, causing him to fall He fell about 4 ft. He said he had pain in right foot and right knee. About and hour after accident he went to St. Lukes Hospital. They took x rays and was discharge )sprain right knee and ankle). He had wet shoes when he went to get into the truck.

The employee was opening railcar doors to clean the cars. One of the door latches was stuck so he pried up on it with a spud bar and when he did he felt a sharp pain in his back left shoulder. He did not seek treatment for this injury until 8-1-05. He was given a prescription for physical therapy for 3 times per week for 3 weeks.

EE WAS WORKING HIS REGULAR JOB ON THE RAILCREW, WHEN SOMETIME DURING HIS SHIFT DUST BLEW INTO HIS EYES. THE WINDS THAT DAY WERE BLOWING EXTREMELY HEAVY AND ALTHOUGH HE WAS WEARING SAFETY GLASSES THE CONDITIONS CREATED A SITUATION MORE THAN THE GLASSES COULD OBSTRUCT. HE RECEIVED EYE DROPS AND AN ANTIBIOTIC FOR AN EYE INFECTION THAT LATER DEVELOPED.
