Struck against a moving object

Employee was returning to the pit from primary. As EE neared the bottom of the long ramp, the auto retarder engaged which caused EE's shoulder belt to lock up and slip up EE's neck. Strain to unspecified parts of the neck.

Struck by falling object

Fell, hard hat came off and rock fell from the overhead chute and hit me in the head.

Employee stepped on a patch or ice/snow causing employee to slip and fall on left side. After they fell, they stated their knee was sore.

Employee Statement - Put tubes out. Had one remaining and took it to stemming pile where extra tubes are kept. Walking back to drill, I slipped on some stemming #6 stones that were on top of smooth bedrock. My right foot went up and my left foot ended up underneath me. I felt my left ankle pop.

Employee was working on replacing a twenty amp circuit breaker. Employee de-energized panel by turning 100 amp main circuit breaker off. Leads on primary side of main circuit breaker shorted and caused flash.

EE was cleaning up after a shot and he didn't have his buckel all the way down on the ground, He ran over a rock. That made his upper body twist and his head hit the center post in the cab. He cut his head on the left side and had to get 2 staples in his head. I took him to hospital. He returned to work with out restrictions.

Fall from machine

While climbing up into the terex haul truck (45 ton #26.8939) His right foot slipped off the step, causing him to fall He fell about 4 ft. He said he had pain in right foot and right knee. About and hour after accident he went to St. Lukes Hospital. They took x rays and was discharge )sprain right knee and ankle). He had wet shoes when he went to get into the truck.

EE ROLLED A ROLL OF WIRE ONTO ITS SIDE IN ORDER TO ROLL IT TO JOB TRAILER. EE FELT PAIN IN CHEST AND EXPERIENCED SHORTNESS OF BREATH. EMERGENCY PERSONNEL WERE CALLED AND TRANSPORTED TO HOSPITAL. IT WAS SUSPECTED THAT THE EE WAS SUFFERING A HEART ATTACK

While walking under conveyor 110 towards the service truck employee stepped over a small pile of rocks, and when his weight was on his left foot the small rock he was standing on rolled and he lost his balance and twisted his left ankle.

EE WAS FUELING UP LOADER AT THE END OF HIS SHIFT, HE HAD ONE HAND ON THE LOADER AND ONE HAND ON THE FUEL HOSE. EE WAS GETTING OFF THE LOADER WHEN HE SLIPPED OFF THE LADDER OF LOADER & FELL BAC KWARDS HITTING HIS BACK ON THE GUARD RAIL.
