EE STRAIGHTENING YIELD SIGN, HAD BOTH ARMS AROUND SIGN WHILE APPLYING PRESSURE. AS EE WAS APPLYING PRESSURE, SHOULDER POPPED. RELEASED PRESSURE FROM AROUND SIGN AND MOVED ARM AROUND TO SEE IF IT WAS HURT OR A "NORMAL POP", WAS JUST A LITTLE SORE. COMPLETED TASK & TOOK SOME IBUPROPHEN. REPORTED INCIDENT NEXT MORNING

Showing 50 of 164EE was in the process of replacing a heat shield #4 Kiln, #3 Pier South East trunnion. While in the process of this, the kiln caught the heat shield and trapped his foot crushing it as the kiln came around.
Employee lifted then tossed a piece of scrap metal that weighed approximately 50 pounds, on to a trash pile. This act resulted in the employee straining his left buttock muscle.
Employee was working in the cooler lifting buckets out during clean up duty. When he lifted a bucket he felt a pain in the abdomen. He was diagnosed by comapany doctor as having strained his abdominal muscle. He was released to return to work on light duty for 2 weeks.
EMPLOYEE WAS HANDLING A PIECE OF METAL WHEN IT SLIPPED & CUT HIS FINGER. THE CUT REQUIRED 6 STITCHES IN HIS RT. PINKY FINGER.
THE EMPLOYEE WAS WORKING ALONE IN THE QUARRY PICKING UP WATER LINES, HE SAID HIS LEFT CALF MUSCLE STARTED HURTING BUT HE DID NOT NOTIFY ANYONE ON IT THAT EVENING. THE NEXT MORNING, HE COULD HARDLY WALK SO HE WAS SENT TO COMPANY DOCTOR.
Excess bending and the lifting of heavy weight manually resulted in a strained lower back.
Employee was helping another change a flat tire on the maintainer. While rotating tire to get the valve stem in correct position, employee's fore finger on his right hand was smashed between the rim and a wheel stud.
Employee was removing and sorting screen cloths on a secondary tower structure for the purpose of housekeeping when he lost control of a screen and smashed the tip of his middle finger on his right hand between the screen and top rail of a nearby hand rail.
EE WAS CHANGING IDLERS ON A CONVEYOR AND WAS TRYING TO REPOSITION HANDS TO HOLD THE IDLER, WHEN IT SLIPPED CAUSING FINGER TO BE PINCHED BETWEEN THE CAMS ON THE IDLER.
Employee cut his finger on right hand while helping move clean up screw behind #2 RM.
Ee was wrapping up & putting away his tools & equipment. He stated that he was extremely sweaty to the point it was running into his eyes causing burning & poor visibility. He lifted his safety glasses & wiped his eyes with his shirt sleeve & got something in his eye. He didn't realize it till next morning.
Employee received second degree burns on 4 small places on his right shoulder after he leaned against a hot piece of metal.
Employee was moving an acetylene bottle and lost his grip letting it slip catching his right third digit between the bottle and a trailer.
He was closeing a Valve on a slurry line and tore his bicep in his right arm
EE was looking at a coupling using the boat. He went under the pipeline and while going underneath the pipeline, his left leg became wedged in between the boat and pipeline causing swelling around the knee.
Ee was removing a choker from the back of a loader that look to not have any pressure and it sprang out and hit him just above the right eye causing a small lac. to the forhead that took 11 stiches not he had on his safety glasses and hardhat at the time.
WE WERE INSTALLING A SUCTION SIDE LINER ON THE GRAVEL PUMP. WE UNBOLTED THE LINER FROM THE DOOR, LIFTED ON THE DOOR W/THE CRANE & THE LINER WAS STUCK. WE PRYED ON THE LINER & THE RUBBER GASKET CAME LOOSE. WHEN I REACHED UP TO GRAB IT, THE LINER FELL FROM A HEIGHT OF ABOUT 3' ON MY FOOT.
THE MINER WAS WORKING FROM A PADDLE BOAT INSTALLING A 8"JET SLEEVE ONTO AN 8"PIPE. HIS FOOTING WAS UNSTABLE & WHEN HE LIFTED ON THE RUBBER SLEEVE HE RELEASED A BIND THAT IT WAS IN & THE SLEEVE JERKED AWAY FROM HIM. HE SAID HE HEARD A LOUD POP IN HIS RT. ARM & NOTICED A BULGE ON THE LOWER END OF HIS BICEP.
Employee grabbed a shovel and got his thumb caught between the shovel's handle and a locker. When employee pulled his hand away, he cut his thumb and pulled his thumbnail off.
Rock from tower fell and struck employee's left knee. Employee called Southern Refractories, Inc. office on August 13, 2004 and informed me that he was having tingling and burning sensations in his knee that was traveling down his leg to his toes. He is at Care Now Medical Clinic today having his leg x-rayed.
Ywo miners were cutting loose a chute from the log washer when it came off it struck one miner on his hand breaking his little finger, the other miner was struck across his shoulder, but was not injured.
Ee was shoveling on Monday morning, June 7th, 2004 around 9:00 A.M. at the pugmill area of our finished end area of the Plant. Ee was shoveling around concrete area when the shovel got caught under a holding clip at base of concrete wall. When he lifted up the shovel, the shovel did not release and ee got a sharp pain in his lower back.
Employee was working with his CB radio in distribution dept. when he hit his right elbow on his desk causing swelling. He went to the dr. and was given meds for inflamation & has taken it all with no improvements. He went on 07/02 for checkup and had fluid drawn from elbow and given antibiotics.
EMPLOYEE ALLEGES THAT WHEN HE AND TWO CO-WORKERS WERE LIFTING A SNUB ROLLER, HIS FINGER GOT STUCK IN BETWEEN THE SNUB ROLLER & THE CATWALK PULLING OFF THE TIP OF HIS LITTLE FINGER.
Employee was helping change blow bars in the 3 bar secondary crusher. They had removed one bar. They were rolling the rotor to position the second bar in the upright position. Because they could not get the rotor pinned the hold all of the weight the rotor, it rolled back pinning his right foot between the housing and the rotor. This action caused 5 broken bones
EE & CO-WORKER WERE CARRYING AN ALUMINUM LADDER TO THE SHAKER. WHILE TURNING TO GO UP THE STAIRS EE FELT A PULL IN HIS UPPER LOWER BACK. THE ACCIDENT WAS CAUSED BY THE LENGTH OF THE LADDER & THE SHARP TURN ATOP THE STAIRS.
EE WAS ASSISTING ANOTHER EE REMOVE PULLING PUMP PAKCING WHEN HE PULLED A MUSCLE IN HIS UPPER LEFT ARM.
EMPLOYEE WAS CHANGING IMPLEMENTS ON A BOBCAT. WHEN HE PUSHED DOWN ON LOCKING LEVER IT CAUGHT HIS FINGER BETWEEN LEVER AND BUCKET FRAME. HE WAS WEARING GLOVES. THE PINCH RESULTED IN A LACERATIO N TO THE FINGER REQUIRING THREE STITCHES.
EE WAS REMOVING THE FRONT LINER FROM PUMP. THE BOTTOM OF THE LINER SLID FORWARD CAUSING THE TOP OF THE LINER TO SLIDE BACK. HIS LEFT MIDDLE FINGER GOT CAUGHT BETWEEN THE LINER AND THE PUMP SHE LL. HIS FINGER TIP IS CRUSHED AND BROKEN AND HIS FINGERNAIL HAD TO BE SEWN BACK ON REQUIRING 8 STITCHES.
EMPLOYEE WAS KNEELING ON THE FLOOR PREPARING TO SLIDE A SHEET OF METAL. WHEN HE LEANED FORWARD HE FELT A CATCH IN HIS BACK AND HAD DIFFICUTLY STANDING UP.
ON MONDAY DEC. 22, EE WAS PERFORMING MAINTENANCEAT DREDGE. HE WAS HELPING CO-WORKER DO OIL CHANGE ON 3406 AND CLEANING UP CAB OF DREDGE, AND TEARING OUT OLD FLOOR SO NEW ONE COULD BE INSTALLE D. EE HELPED CARRY SOME CONCRETE BOARD USED TO REPAIR FLOOR. THE BOARD WAS CARRIED APPROX. 40 YDS DOWN A RAMP. EE CONTINUED TO WORK ON FLOOR PROJECT UNTIL COMPLETED... (1/13/04-HAD SURGERY)
EMPLOYEE WAS SETTING 48" TROUGHING IDLER UPON CATWALK TO BE PUT ON AFTER SETTING IDLERS DOWN. EMPLOYEE UNHOOKED CHAIN FROM IDLERS ONE OF THE IDLERS STARTED TO FALL OFF THE CATWALK. EMPLOYEE RE ACHED FOR IDLER TO PREVENT IT FROM FALLING. HIS MIDDLE FINGER WAS CAUGHT BETWEEN IDLER AND CATWALK HANDRAIL. EMPLOYEE WAS TAKEN TO DOCTOR. EMPLOYEE RECEIVED STITCHES IN MIDDLE FINGER.
EE WAS PUSHING A ROLLBACK CHUTE WHEN HE FELT A STRAIN IN HIS RIGHT SHOULDER AND NECK.
EE WAS SPLICING CONVEYOR BELT WHEN WORK WAS COMPLETED EE WAS GATHERING TOOLS WHEN THE BELT CLAMPS FELL OVER ONTO HIS RIGHT FOOT. EE WAS TAKEN TODR FOR XRAYS & TREATMENT ON 11/10/03. EE WAS PLA CED ON RESTRICTED DUTY TILL RELEASED BY DR.
EE WAS CHANGING TROUGHING IDLERS OUT ON A CONVEYOR WHILE THE EE WAS PULLING THE OLD IDLER OUT HEATTEMPTED TO DROP THE OLD IDLER TO THE GROUND ITHIT EE'S RIGHT FOOT. EE WAS TAKEN TO DR TO HAVE IT XRAYED. EE WAS PUT ON RESTRICTED DUTY UNTIL FOLLOW UP BY DR
EMPLOYEE ALLEGES THAT WHILE HE WAS ROLLING UP AN AIR HOSE, THE HOSE CAUGHT ON A PIECE OF STEEL CAUSING THE STEEL TO FALL ON HIS FOOT.
EE WAS PULLING A PULLEY OFF A SHAFT (TAIL PULLEY). THE PULLEY ENCOUNTERED RESISTANCE ON THE SHAFT AND BECAME HARD TO MOVE. EE PULLED HARDER AND PULLEY SUDDENLY CAME ALL THE WAY OFF THE SHAFT, HITTING THE EE ON THE KNEE.
EE WAS LOWER LOUVER BLADES THE NIGHT OF THE INJURY HE S NOT QUIT SURE WHEN BACK SPRAIN HAPPEN. HE WORKE UP ON THE 28TH WITH A PAIN IN HIS LOWER BACK.
EE WAS THROWING A FLIGHT OUT OF A SAND SCREW AND MASHED HIS RIGHT INDEX FINGER AGAINST THE SIDE WALL OF THE SCREW.
EE WAS IN A CLASSIFIER TUB DOING MAINTENANCE, PULLING ON A VALVE ROD WHICH WAS STUCK IN THE SAND TO TRY TO DISLODGE IT. HE FELT A PAIN IN THE LEFT SIDE OF HIS BACK. IT HURT FOR A LITTLE WHILE THEN LATER THAT DAY HIS BACK POPPED. IT DID NOT HURT MUCH AFTER THAT BUT PAIN INCREASED AS THE WEEK PROGRESSED. HE BEGAN WEARING & BACK BRACE & RELATED THE EVENTS OF THE INJURY TO TEAM COOR.
EMPLOYEE WAS HELPING TO LIFT SUGAR SCOOP AT THE KILN INLET. IT FELL AND HIT EE'S HAND CAUSING A FRACTURED RING FINGER AND AN OPEN FRACTURE TO HIS PINKY FINGER ON HIS RIGHT HAND.
EMPLOYEE WAS TRYING TO BEND LANCE AND IT HIT HIM IN THE HEAD CAUSING CUT TO THE FOREHEAD.
EE ALLEGES THAT WHILE HE WAS LINING A 5-15/16" SHAFT IN A PULLEY, HE FELT A SHARP PAIN IN HIS BACK.
EE WAS HELPING INSTALL A 54: RETURN IDLER ON A FIELD CONVEYOR. THE EE HAD ONE END PUT IN AND WAS ATTEMPTING TO PUT THE OTHER END IN WHEN THE CHAIN THEY WERE USING TO HELP LIFT THE IDLER IN PLA CE SLIPPED CAUSE THE IDLER TO FALL STRIKING THE EE IN THE FOREHEAD. THE EE RECEIVED A CUT ON HIS FOREHEAD, EE WAS TAKEN TO THE DR. EE RECEIVED FOUR STITCHES TO CLOSE THE CUT. EE WAS RELEASED
MEMBER BENT OVER TO OPEN A VALVE AND FELT A POPIN HIS BACK.
EE WAS TYING A FILTER SCREEN TO THE END OF A PUMP SUCTION HOSE TO PREVENT IT FROM SUCKING ROCKS INTO THE PUMP. WHEN HE PULLED ON THE ROPE TO TIGHTEN IT, HIS SHOULDER POPPED.
EE CHECKING BRAKE FLUID LEVELS, FINGER SLIPPED, HIT EDGE OF CASING R THUMB LACERATION
WHILE PERFORMING ROUTINE JOB DUTIES EMPLOYEE LIFTED A 5 GALLON BUCKER OF MATERIAL (APPROX 30 # SAMPLE) OVER THE SIDE OF THE BED OF A PICKUP TRUCK, PULLING SOMETHING IN HIS BACK.
CREW WAS IN THE PROCESS OF CHANGING HAMMERS. HELPER HAD HOLDING WRENCH BOLTED UP AGAINST THE MAST. WHILE CONNECTING, DRILL STEEL CROSS THREADED ONTO HAMMER. DRILLER REVERSED ROTATION TO UNTHRE AD. WHEN HE DID THIS IT CAUSED THE HOLDING WRENCH TO MOVE. HELPER RE-GUIDED DRILL STEEL ONTO HAMMER, THE DRILLER HIT THE ROTATION TO SCREW ONTO THE HAMMER, CAUSING THE WRENCH TO HIT HIS ANKLE
