Employee was splicing conveyor belt with three other ees at Bellino Tunnel Belt. While positioning belt, ee was standing, slightly bent at waist pulling slack from belt and felt sharp pain in lower back, diagnosed as a strain.

Injury first reported to Foreman on 3/29/17. Date of incident and time are approximate. Employee has burning sensation in left shoulder joint when pulling fuel hose from reel on lube truck to fuel 385 excavator. Employee is still experiencing pain. Possible overuse injury from pulling hose reels many times per day.

During shift miner unknowingly sustained a minor superficial abrasion on EE's right knee. Two hours after shift, EE noticed inflammation, and rapid swelling. At ER EE was diagnosed with prepatellar bursitis, given antibiotics, and released. After 48 hours swelling continued. EE was admitted to hospital for drainage of knee, IV antibiotics, and observation.

Employee was constructing new conveyor, frame section had no belt, unpowered and no stored energy. While moving frame into place employees hand passed through area between diagonal and horizontal troughing rollers. Space was large enough for employees right hand pinky finger to pass through, but small enough to cause laceration and damage to finger.

Employee was replacing a belt on the Power Screen and bumped his knee, causing a contusion that subsequently became infected. Employee declined medical treatment until 12/13/11. Claim was accepted on 12/13/11 as reportable.

Employee was replacing a 15 HP electric motor on the crusher feeder. As he tried to position the motor onto the mount, the motor shifted and caught his finger against the frame of the crusher feeder. Employee experienced a laceration on the small finger of his right hand.

EMPLOYEE GRASPED A HANDRAIL USED FOR ACCESS TO A PLATFORM UTILIZED TO CHANGE THE BLOW BARS ON A RECYCLE PLANT. EMPLOYEE FELT A SHARP PAIN IN HIS RIGHT SHOULDER. THE PAIN DID NOT SUBSIDE, SO TH E EMPLOYEE WAS DRIVEN TO CALIFORNIA OCCUPATION PHYSICIANS IN MODESTO, CA. FURTHER EVALUATION REVEALED AN ACUTE SPRAIN ON HIS RIGHT SHOULDER.
