Worker thought digging bar was clear of two pipes, and began to lift and walk away with it. The bar got jarred and as EE pulled, the motion over extended EE's shoulder. Worker denied care at time of incident. EE worked at full capacity until EE requested and went for an initial evaluated on 11/23/22.

Employee said that they were working down in the hole, there was a roll of plastic liner on a rack being. As they were rolling out the plastic liner, the plastic liner slid down the rack and crushed employee's left hand between the liner and the rack.

Changing rollers at the Secondary crusher area on the 2-A (S-3) conveyor. While removing old roller frame, twisted left shoulder, treated at WorkNet for shoulder injury from time of accident. Went for arthroscopic surgery 7-30-12, had been working up until now.

Working on primary crusher he dropped his wire brush onto D4 conveyor. He climbed down to get the wire brush. As he climbed out and off the conveyor he went to jump over the railing and his right knee locked up & he could not bend it.

On 11-23-05 employee reached for truck door and banged finger on door. On 12-13 employee went to personal physician & was given a prescription. On 12-27-05 he went back to physician and was put out of work for six weeks to prevent further dust exposure and cure finger.

INJURED WAS REMOVING A SPROCKET FROM THE SHAFT OF AN APRON FEEDER TO FACILITATE THE CHANGING OF THE SHAFTS BEARING, INJURED HIS LOWER BACK. HE WAS IN A BENT OVER POSITION AND WHILE REMOVING A HEAVY SPROCKET FROM THE SHAFT, TWISTED TO THE SIDE. HE REPORTEDLY FELT A SHARP PAIN IN HIS LOWER BACK WHILE PERFORMING THIS ACTIVITY.
