Slip while dismounting caught self straining side
IW was hosing under equipment and when IW stepped back to turn, twisted left knee.

IW lost footing while dismounting equipment, fell backward hurting back and right hip. Received restrictions 8/8, following initial treatment on 8/5.
Employee was putting roller in and was struggling to get the roller to stand up next to another roller and while doing so, the hand rolled into the roller and got pinched between the two rollers.
Lifting rock out of the cone crusher chute. It slid out and ripped skin and nail of right pinky finger.

Changing rollers at the Secondary crusher area on the 2-A (S-3) conveyor. While removing old roller frame, twisted left shoulder, treated at WorkNet for shoulder injury from time of accident. Went for arthroscopic surgery 7-30-12, had been working up until now.

Climbing onto truck #060076 to check the fluid levels before starting it for the day, while climbing up ladder he felt a twinge on his right shoulder upper arm.
EE was starting and checking fluid levels of the haul trucks for the start of the shift. While checking truck #06-0090, EE slipped of the ladder on the front of the truck and fell backwards.

Working on primary crusher he dropped his wire brush onto D4 conveyor. He climbed down to get the wire brush. As he climbed out and off the conveyor he went to jump over the railing and his right knee locked up & he could not bend it.

WHILE INSTALLING WEAR PLATE ON WET SIDE SCREEN DECK NUMBER W2, EMPLOYEE SLIPPED AND SLID DOWN CHUTE, HITTING LEFT SIDE AND RIB AREA AT THE BOTTOM OF CHUTE; AREA WAS WET AND SLIPPERY.
INJURED WAS REMOVING A SPROCKET FROM THE SHAFT OF AN APRON FEEDER TO FACILITATE THE CHANGING OF THE SHAFTS BEARING, INJURED HIS LOWER BACK. HE WAS IN A BENT OVER POSITION AND WHILE REMOVING A HEAVY SPROCKET FROM THE SHAFT, TWISTED TO THE SIDE. HE REPORTEDLY FELT A SHARP PAIN IN HIS LOWER BACK WHILE PERFORMING THIS ACTIVITY.
