IW struck left little finger with hammer.

Slip while dismounting caught self straining side

IW was hosing under equipment and when IW stepped back to turn, twisted left knee.

IW working on a tarp and little finger got caught between rope and shaft.

IW was operating a loader when IW struck a solid rock with the bucket, jolting IW forward.

IW was dismounting equipment when EE lost EE's footing on the last step banging EE's right knee.

IW lost footing while dismounting equipment, fell backward hurting back and right hip. Received restrictions 8/8, following initial treatment on 8/5.

Employee was putting roller in and was struggling to get the roller to stand up next to another roller and while doing so, the hand rolled into the roller and got pinched between the two rollers.

Lifting rock out of the cone crusher chute. It slid out and ripped skin and nail of right pinky finger.

While operating an excavator a small rock became dislodged and flew through the open window of the cab striking employee in the face above left eye.

Tightening up a hose that was leaking, standing between tire and step fender on loader #260068 foot slipped, fell backwards onto blacktop approx. 4 feet, injuring center left side of back.

Lifting 12"" x 6"" steel beam that was 8' long into loader bucket

Changing rollers at the Secondary crusher area on the 2-A (S-3) conveyor. While removing old roller frame, twisted left shoulder, treated at WorkNet for shoulder injury from time of accident. Went for arthroscopic surgery 7-30-12, had been working up until now.

Employee was moving water hose from drive side of crusher to non drive side of crusher. Employee fell off crusher.

Climbing ladder to haul truck #060090, slipped and fell to the ground.

It was cold, raining and damp, EE stepped on wet platform of his trailer. He slipped and fell, twisting ankle.

EE was cleaning out chute under D-1 scren behind primary crusher on top of D-2 conveyor.

Climbing onto truck #060076 to check the fluid levels before starting it for the day, while climbing up ladder he felt a twinge on his right shoulder upper arm.

EE was starting and checking fluid levels of the haul trucks for the start of the shift. While checking truck #06-0090, EE slipped of the ladder on the front of the truck and fell backwards.

Working on primary crusher he dropped his wire brush onto D4 conveyor. He climbed down to get the wire brush. As he climbed out and off the conveyor he went to jump over the railing and his right knee locked up & he could not bend it.

BLOCK SLID OFF PALLET DUE TO ICE ON BLOCK WHICH HIT EE IN THE LEFT SHIN. HE WAS MANUALLY HANDLING BLOCKS ON PALLET ON FORKLIFT.

EMPLOYEE OPENED DOOR OF FRONT END LOADER TO TALKTO TRUCK DRIVER, FOREIGN BODY BLEW INTO LEFT EYE.

REMOVING ELECTRICAL DRIVE MOTOR ON D3 SCREEN AT THE SECONDARY CRUSHER PLANT. BIG TOE ON RIGHT FOOT WAS HIT WHEN LIFTING EYE ON MOTOR BROKE DROPPING MOTOR.

EMPLOYEE WAS USING MIG WELDER TO BUILD UP BARS INSIDE PRIMARY CRUSHER. HE RAISED HIS WELDING SHIELD UP WITH SAFETY GLASSES ON UNDERNEATH. HE BENT OVER TO MEASURE THICKNESS OF THE BAR, SLAG IN THE WELD POPPED, WENT UNDER SAFETY GLASSES.

CRUSHER WELDER HIT EMPLOYEE'S RIGHT HAND WITH A SLEDGE HAMMER WHEN THEY WERE HITTING I BEAM TRYING TO FREE IT.

CLIMBING AND WALKING ON CONVEYORS, CRUSHER, AND SCREEN DECK.

EE WAS WALKING DOWN STEPS TWISTED LEFT ANKLE AT LANDING.

WHILE INSTALLING WEAR PLATE ON WET SIDE SCREEN DECK NUMBER W2, EMPLOYEE SLIPPED AND SLID DOWN CHUTE, HITTING LEFT SIDE AND RIB AREA AT THE BOTTOM OF CHUTE; AREA WAS WET AND SLIPPERY.

INJURED WAS REMOVING A SPROCKET FROM THE SHAFT OF AN APRON FEEDER TO FACILITATE THE CHANGING OF THE SHAFTS BEARING, INJURED HIS LOWER BACK. HE WAS IN A BENT OVER POSITION AND WHILE REMOVING A HEAVY SPROCKET FROM THE SHAFT, TWISTED TO THE SIDE. HE REPORTEDLY FELT A SHARP PAIN IN HIS LOWER BACK WHILE PERFORMING THIS ACTIVITY.
