IW pulled a tool tray out and strained EE's back.

IW was performing maintenance; carrying a guard and EE pulled a groin muscle.

IW was cutting out a bearing and hot slag went under their safety glasses and into their eye.

Worker has no specific incident or occurrence, but EE believes the sore back is from doing daily job - driving a haul truck.

Worker thought digging bar was clear of two pipes, and began to lift and walk away with it. The bar got jarred and as EE pulled, the motion over extended EE's shoulder. Worker denied care at time of incident. EE worked at full capacity until EE requested and went for an initial evaluated on 11/23/22.

Injured worker was struck in the head when a drum dolly became dislodged.

Putting a nut on a bolt while on a small ladder, slipped off ladder, fell approximately 2 feet and landed wrong on EE's foot, injuring ankle.

Digging out chute. Employee has been working regular job since June, was treating with therapy. Employee has not improved and is going for surgery Fri. 9-15-17. Employee will be off work following surgery.

Pain in right arm from elbow to wrist. Employee was treating with WorkNet doctors and Phila Hand Ctr. until surgery 1/20/14 and still working. Started missing work 1/20/14.

Electrician EE was trouble shooting motor starter he placed the meter leads on the load side of the breaker and an arc flash occurred.

Stepping off step ladder, missed bottom step.

Slipped off of a ladder fell a few feet ( 2 - 3)

On 11-23-05 employee reached for truck door and banged finger on door. On 12-13 employee went to personal physician & was given a prescription. On 12-27-05 he went back to physician and was put out of work for six weeks to prevent further dust exposure and cure finger.

DRIVING FUEL TRUCK DOWN INTO THE QUARRY TO FUEL PUMPS, EMPLOYEE SAYS VEHICLE LOST BRAKES, RAN INTO BERM AND TURNED OVER ON DRIVER'S SIDE. INVESTIGATION FOLLOWING ACCIDENT REPORTED BRAKES O.K.

GRINDING A SPLICE IN CONVEYER BELT, FOREIGN MATTER ENTERED WORKER'S EYE.
