Employee was handling a bag of ice and scraped their left pinky at the cuticle area with a wire used to close the 40lbs bag. They notified on 02/09/2024 that they felt pain and has redness and swelling at the area. Antibiotics and vaccine were prescribed.

EE was removing a steel plate from the plow feeder using a manual chain hoist. After a while he tried to move the steel plate with his hands. Suddenly the plate slipped fron the hoist's hook and fell on his left foot.

EE WAS REMOVING CONCRETE FROM A DUST COLLECTOR. DURING THIS TASK CEMENT DUST FELL ON THE INSIDE OF HIS ELBOW, RIGHT ARM. THE CONTINUOUS FLEXING OF HIS ARM CAUSED AN ABRASION.
EE WAS TRYING TO CLEAN AN OBSTRUCTED AIR HOSE THAT WAS CONNECTED TO A DUST SILO WHEN THE HOSE DISCONNECTED AND BLEW DUST INTO HIS EYE.
EE WAS INSTALLING A VIBRATOR AT THE RAW MILL CHUTE. WHILE WALKING HE LOST HIS BALANCE & HIT HIS RT. FOREARM AGAINST A HOT DUCT CAUSING A BURN.

EE WAS REPAIRING A WOOD PALLET AT THE PACKING HOUSE. HE WAS USING A PIECE OF WOOD AS A LEVEL TO SEPARATE A DAMAGED PIECE OF WOOD FROM THE PALLET. SUDDENLY THE DAMAGED PIECE OF WOOD BROKE AND H IT HIS NOSE.

EMPLOYEE WAS ENTERING THE PURCHASING OFFICE IN THE ADMINISTRATIVE BUILDING, WHEN HE OPENED THE OFFICE DOOR HE TRIED TO STOP IT FROM CLOSING WITH HIS ELBOW. THE DOOR HIT HIS ELBOW CAUSING A CON TUSION.
WHILE INSTALLING A 6" STEEL PIPE, THE PIPE SLIPPED & PINCHED HIS INDEX FINGER OF THE RIGHT HAND. HE WAS GIVEN SIX STITCHES ON HIS FINGER.
EE HAD FINISHED WORK ON BELT CONVEYOR Q AND WAS LOWERING TOOLS WITH COWORKER. HIS COWORKER WAS LOWERING TOOLS FROM A 50 FT HEIGHT WHILE HE WAS UNTYING AND PLACING THE TOOLS IN A CART. WHILE LO WERING A BELT CUTTER ITS METAL PLATE SLID OFF THE CUTTER AND ROPE HITTING EE'S LEFT FOOT.
EE WAS PERFORMING CLEANING DUTIES IN THE CEMENT MILLS AREA. AFTER A FEW HOURS OF WORK HE BEGAN TO FEEL ITCHING ON HIS BACK AND NECK.
EE WAS FILLING CEMENT BIG BAGS WITH A SHOVEL. AFTER A WHILE HE BEGAN TO FEEL BACK PAIN. THE EE REPORTED THE ACCIDENT ON 9-13-02.
EMPLOYEE WAS CLEANING THE FLOOR AT CEMENT MILLS 11 AND 12 AREA. BY HIS OWN INITIATIVE HE BEGAN TO CLEAN A BELT CONVEYER ROLLER WITH A CHISEL. ACCIDENTLY HIS RIGHT ELBOW WAS PINCHED BETWEEN THE BELT AND THE ROLLER CAUSING HIM A TRAUMA.
