Employee was removing a cement bag that was lodged on a conveyor belt and when the bag came loose it caused cement to blow onto employee's face falling in EE's eyes. Causing redness and inflammation to EE's right eye and eyelid.
Employee alleges that the employee was placing an iron tripod to place a cell phone to record and the legs of the tripod and the base were sapped, impacting the left hand, causing fractured nails and bleeding in the index and middle fingers.
Employee was handling a bag of ice and scraped their left pinky at the cuticle area with a wire used to close the 40lbs bag. They notified on 02/09/2024 that they felt pain and has redness and swelling at the area. Antibiotics and vaccine were prescribed.
Miner was allegedly filling a bucket and then emptying it on a locked out conveyor belt. After approx. 18 times their wrist bent backwards when raising the bucket. Miner had pain and limited range of motion. Miner was taken to ER where X rays were performed and found no fractures. Miner was discharged and given pain medication.
A cement silo overflowed and a cement dust cloud formed causing cement dust to fall in an employee's eyes causing irritation and redness.
Employee alleges EE was reclining pallets on a metal column, a burst of wind pushed one of the pallets and it hit the back of employee's left calf causing a laceration that required stitches.
Employee alleges EE was moving mill plates and EE's left index finger was pinched between 2 plates causing a bruise/hematoma. Ice pack was provided as first aid.
Employee alleges cement was blown into EE's left eye when a bag broke. EE was evaluated and prescribed antibiotic eye drops to prevent infection.
An employee was removing oversized material from Belt Conveyor V and while moving material EE hit hand with a metal beam causing a laceration on right 3rd finger.
The employee was installing a clamp on a scaffolding ladder of the first level, in the interior of the Cyclone 4th stage, in the pre-heater tower, when some brick fragments fell down and hit employee on the right elbow. The employee was taken to a medical facility & was authorized to return to restricted work.
The employee proceeded to remove a truck starter, which has an approximately weight of 60 pounds, alone, but it slipped and jabbed the left hand ring finger against the concrete; causing an open wound of approximately one inch in length. Stitches were took on the finger. The employee will return to restrictive work.
The employee was working in the third floor when he swing his body hit over the eyebrow with something fix and stationary.
The employee was inspecting the area of the air blasters which are located in the Cooling System (Cooler), employee placed his right hand against a fan/blower setting it on a plate to revise the blaster, when employee removed his hand from the fan the plate fell on his hand pushing his against one of the air blasters and then pinching his ring finger of his right hand.
The miner pinch his right hand with a metal piece during maintenance works in a mill machine.
The miner was hit in his forearm with a stone when cleaning the machine.

Injury said that a metal cover placed over an edge, fall over his foot hitting over where the metal of the safety shoe finish resulting in a trauma to his left foot.

The employee was verifiying the process equipment, suddenly a release of florida lime blew in his face and the left eye.
The employee was installing an iron plate inside a Cement Mill. Accidentally the plate slipped by few inches hitting his left ankle and foot. The iron plate was located at the Cement mill's floor
Three employees were dislodging the terciary air duct at the second floor of the Preheater Tower. When they opened the duct gate hot material flushed away. At the same time EE was passing underneath the duct when the hot material reached his feet.
EE was placing metal drums near the Packing House when his left middle finger was pinched between 2 drums.
Employee was changing a belt conveyor at the packing house area. When he placed the conveyor on the floor a belt fastener hit his wrist causing an open wound.
Employee was performing cleaning duties at the Packing House. Suddenly cement dust feel into his eyes.
EE was removing a hydraulic cylinder from a loader with his supervisor at the heavy equipment shop. The cylinder was held with a crane. When the EE moved the cylinder slipped from one end of the loader structure causing a quick movement of cylin der pinching his finger between cylinder & loader bucket arm at the other end.
EE was removing a steel plate from the plow feeder using a manual chain hoist. After a while he tried to move the steel plate with his hands. Suddenly the plate slipped fron the hoist's hook and fell on his left foot.
EE was performing mechanical duties a Kiln's # 6 cooler. After he finished the task his proceeded to keep two manual chain hoists to the tool room. while he was passing by underneath a conveyor he began to feel back pain. ***The employee reported the accident on 11/22/05
Employee was performing cleaning duties at Cement Mills area. Suddenly cement dust fell into his left eye causing him an irritation. He was wearing safety glasses.
EE was inspecting a mobile vehicule at Carpenter's shop. While he was passing around the vehicle he rubbed a piece of glass that was in the cargo area causing him an open wound.
EE was installing a conveyor belt with other empoyee on the Clinker Stacker. While performing this task EE began to feel back pain due to the uncoordinated movement performed.
Employee was working on palletizer # 2 at the Packing House area. He was removing a wood pallet that was stuck in the palletizer when suddenly few pallets fell over his left elbow causing him a contusion.
EE WAS REMOVING BAG FILTERS FROM A DUST COLLECTOR IN THE MILL AREA. THE REMOVAL REQUIED REPETITIVE MOTION WHILE REMOVING THE SCREWS THAT HELD THE FILTERS IN PLACE. AFTER A WHILE HIS RIGHT SHOU LDER BEGAN TO ACHE. AFTER WORK THE ACHING WORSENED TO A SHARP PAIN IN THE SHOULDER.
EE WAS REMOVING CONCRETE FROM A DUST COLLECTOR. DURING THIS TASK CEMENT DUST FELL ON THE INSIDE OF HIS ELBOW, RIGHT ARM. THE CONTINUOUS FLEXING OF HIS ARM CAUSED AN ABRASION.
EE WAS TRYING TO CLEAN AN OBSTRUCTED AIR HOSE THAT WAS CONNECTED TO A DUST SILO WHEN THE HOSE DISCONNECTED AND BLEW DUST INTO HIS EYE.
EE WAS INSTALLING A VIBRATOR AT THE RAW MILL CHUTE. WHILE WALKING HE LOST HIS BALANCE & HIT HIS RT. FOREARM AGAINST A HOT DUCT CAUSING A BURN.
EE WAS LIFTING A PORTABLE PLASTIC WATER COOLER FILLED WITH ICE AND WATER WHEN HE FELT PAIN IN HIS BACK. A FEW MINUTES LATER WHILE WALKING TO HIS JOB SITE HE BEGAN TO FEEL PAIN AGAIN.
EE WAS STACKING FILLED LIME BAGS AT THE PACKING HOUSE AREA. SUDDENLY A BAG BROKE AND LIME DUST FELL INTO HIS RIGHT EYE CAUSING AN IRRITATION. THE EMPLOYEE WAS WEARING SAFETY GLASSES.
EE WAS PERFORMING CLEANING DUTIES WITH A SHOVEL AT THE PLOW FEEDER AREA NEAR CONVEYER BF3. FEW HOURS LATER HE BEGAN TO FEEL PAIN ON HIS RIGHT HIP. THE EE REPORTED THE ACCIDENT ON 9-3-03.
EE WAS REPAIRING A WOOD PALLET AT THE PACKING HOUSE. HE WAS USING A PIECE OF WOOD AS A LEVEL TO SEPARATE A DAMAGED PIECE OF WOOD FROM THE PALLET. SUDDENLY THE DAMAGED PIECE OF WOOD BROKE AND H IT HIS NOSE.
EE WAS INSTALLING A STEEL COVER ON THE COOLER'S DRAG CHAIN CONVEYOR. WHEN HE PUSHED THE COVER INTO PLACE HE PINCHED HIS LEFT HAND RING FINGER AGAINST ANOTHER COVER CAUSING AN OPEN WOUND.
HAD A CUT ON THE TIP OF LITTLE FINGER 6/10/03, CUT GOT INFECTED 6-13-03 TOOK HIM TO DOCTOR, TREATED WITH ANTIBIOTICS.
PUSHING AND FLIPPING SUPPORT BEAM WHEN RIGHT HAND GOT PINNED BETWEEN BEAMS, FINGER REQUIRED STITCHES (TWO MIDDLE FINGERS)
EE WAS MOVING A WOODEN BOX THAT HAD AN ELEVATOR BRAKE INSIDE. ACCIDENTALLY THE BOX SLIPPED SIDEWAYS AND PINCHED HIS LEFT RING FINGER AGAINST A WALL CAUSING HIM AN OPEN WOUND.
EE WAS PLACING A WHEELBARROW IN THE PRE-HEATER ELEVATOR. SUDDENLY HE HIT HIS RIGHT HAND WITH THE DOOR AND THE WHEELBARROW HANDLE. THE EE REPORTED THE ACCIDENT ON 5-6-03.
EMPLOYEE WAS ENTERING THE PURCHASING OFFICE IN THE ADMINISTRATIVE BUILDING, WHEN HE OPENED THE OFFICE DOOR HE TRIED TO STOP IT FROM CLOSING WITH HIS ELBOW. THE DOOR HIT HIS ELBOW CAUSING A CON TUSION.
WHILE INSTALLING A 6" STEEL PIPE, THE PIPE SLIPPED & PINCHED HIS INDEX FINGER OF THE RIGHT HAND. HE WAS GIVEN SIX STITCHES ON HIS FINGER.
EE HAD FINISHED WORK ON BELT CONVEYOR Q AND WAS LOWERING TOOLS WITH COWORKER. HIS COWORKER WAS LOWERING TOOLS FROM A 50 FT HEIGHT WHILE HE WAS UNTYING AND PLACING THE TOOLS IN A CART. WHILE LO WERING A BELT CUTTER ITS METAL PLATE SLID OFF THE CUTTER AND ROPE HITTING EE'S LEFT FOOT.
EE WAS PERFORMING CLEANING DUTIES IN THE CEMENT MILLS AREA. AFTER A FEW HOURS OF WORK HE BEGAN TO FEEL ITCHING ON HIS BACK AND NECK.
EE WAS FILLING CEMENT BIG BAGS WITH A SHOVEL. AFTER A WHILE HE BEGAN TO FEEL BACK PAIN. THE EE REPORTED THE ACCIDENT ON 9-13-02.
EMPLOYEE WAS CLEANING THE FLOOR AT CEMENT MILLS 11 AND 12 AREA. BY HIS OWN INITIATIVE HE BEGAN TO CLEAN A BELT CONVEYER ROLLER WITH A CHISEL. ACCIDENTLY HIS RIGHT ELBOW WAS PINCHED BETWEEN THE BELT AND THE ROLLER CAUSING HIM A TRAUMA.
