The Employee while exiting the vehicle lost his footing thus holding on the support bar felt a sudden pain in his right arm.

Struck against stationary object

Employee was changing an idler roller of the belt conveyor E. Passing over the belt he put his right foot over loose clay on the floor causing a sprain in his right ankle.

EE was removing a hydraulic cylinder from a loader with his supervisor at the heavy equipment shop. The cylinder was held with a crane. When the EE moved the cylinder slipped from one end of the loader structure causing a quick movement of cylin der pinching his finger between cylinder & loader bucket arm at the other end.

Struck by falling object

EE was removing a steel plate from the plow feeder using a manual chain hoist. After a while he tried to move the steel plate with his hands. Suddenly the plate slipped fron the hoist's hook and fell on his left foot.

Employee was walking by the travel way next to the conveyor belt BC-3. After a while he tried to pull out a piece of broken skirt when his hand was trapped between the belt and the belt alinging rollers (Idler).

Employee was performing cleaning duties with a shovel at conveyor F. He was working with other employee. Accidentally his left arm was pinched between the belt and a steel structure. The conveyor was unguarded.

Employee was performing cleaning duties al Hammermill # 3 with other employees. He was carrying a wheelbarrow filled with stones. Suddenly he lost his balance and the wheelbarrow overturned pinching his left ring finger between the wheelbarrow and the floor

EE WAS PERFORMING CLEANING DUTIES WITH A SHOVEL AT THE PLOW FEEDER AREA NEAR CONVEYER BF3. FEW HOURS LATER HE BEGAN TO FEEL PAIN ON HIS RIGHT HIP. THE EE REPORTED THE ACCIDENT ON 9-3-03.

EE WAS CUTTING AN IRON PLATE WITH OXY-FUEL GAS CUTTING EQUIPMENT. WHEN HE WAS CLEANING THE TORCH SCORIA WITH HIS HAND ACCIDENTALLY THE EE ACTIVATED THE TORCH AND THE FLAME BURNED THE WELDING G LOVE SEAM. THE FLAME PASSED THROUGH THE SEAM OPENING CAUSING HIM BURNS IN HIS FINGER.

EE WAS REMOVING A PINION FROM A TRANSMISSION SHAFT USING A SLEDGE HAMMER. WHEN HE HIT THE PINION WITH THE SLEDGE HAMMER A PIECE OF METAL DISLODGED AND BECAME EMBEDDED IN HIS LEFT LEG.

EE WAS CLEANING AN ENGINE WITH A CLOTH WET WITH DIESEL. WHILE HE WAS PERFORMING THE TASK THE DIESEL DROPLETS FELL INTO HIS LEFT EYE.
