The mine employee was checking the buckets on a bucket elevator and got a particle of something in EE's right eye.
The employee was fabricating a base for a motor and gearbox using a steel plate 24"x24"x3/4"thick. It was on a steel work table in the shop. While repositioning the plate the employee smashed the end of left index finger between the steel plate and the table top. The finger was split open and required six stitches to close the wound.
Employee was retrieving model number on a gear box. In order to do this they used a manlift to elevate 24'. EE removed the guarding, retrieved information, and proceeded to remove the motor assembly. The motor was not properly secured, it fell striking the employees leg. EMS was called to the site employee was taken to DVMC for treatment. Employee was released on 6/13/2019
While replacing the shaft guard on a D10 ripper laying on a work bench the injured felt a back strain.
Setting screen box, crane raised. While raised employee reached up to check the load to twist it, and pinched his right thumb.
Crossing over drill, slipped, caught self with right arm (pulling shoulder)
