An employee was hooking up a drum basket to a forklift, EE bent down to lift and slide the right-side fork to align with the basket, when EE lifted the fork, EE felt like an impact to EE's lower back. EE was taken to the Tooele Utah hospital where EE was diagnosed with lower back muscle sprain. EE received 2 injections for pain and inflammation.

The injured miner was replacing dead batteries on the repeater in the quarry. EE jarred EE's right shoulder and felt pain. EE went to physical therapy and was eventually recommended surgery to repair a torn rotator cuff. The surgery occurred on 10/16/25 making this an MSHA reportable injury.
Employee was pulling out a pump, when pulling EE caught EE's right-hand pinky finger between the pump and the pipe flange.
Hydraulic hose broke and sprayed oil in the face of the employee causing a burn or contusion on the cheek.
A rack of scaffold equipment was being hoisted with a forklift. The rack bumped into the scaffold structure and started to fall off the forks. The worker attempted to push the rack back onto the forks when the rack tipped forward. The worker pinched the tip of middle finger between the rack and a hand rail. Worker received one suture an antibiotic and pain meds.
Employee was cleaning out the cab of the haul truck and slammed a thumb in the door while closing the cab door.
Employee was unplugging core product from the core chute, when product dislodged lime went into EE's left eye.

Emp was trying to open a dump chute using the slide hammer. As EE was sliding the hammer, the handle broke and EE's thumb slipped between 2 pieces of steel, pinching it. EE was taken for treatment and received sutures. EE was released full duty and returned to work.
Employee was lifting a coal mill liner, it slipped and as they tried to catch it, felt a pain in R shoulder. Went to clinic and was given restrictions on lifting and use of right arm.
While opening the lids on trailer, grabbed latch and pulled up. It was stiff and didn't open so EE pulled harder. It snapped open and twisted EE's wrist. EE didn't think much about it until EE was home and it started hurting pretty badly.
Employee was positioning a 40' extension ladder when the ladder began to tip. The employee caught the ladder instead of letting it fall. EE felt a tweak in their shoulder. No treatment occurred at the time. The injury was not improving, so EE sought medical evaluation in February 2022. Surgery was recommended for torn rotator cuff ligaments. Employee underwent surgery on 5/24/2022.
Employee was working to open a stuck door on a hydrator. When door came open it broke out of the hinges and fell to the floor, striking employee in the left leg/ankle. Employee was taken for treatment and later diagnosed with a contusion and was given restricted duty.
Contractor's employee smashed fingers under a brick while unkeying brick ring for removal from kiln.
Emp was reinstalling a guard. Emp caught R index finger between the guard and the frame, causing a laceration. Emp was taken in for treatment and received 4 sutures
Employee was helping with cleanup when a small bit of dirt flew up and went under the safety glasses, hitting the eye. Employee sustained a small abrasion and was given prescription eye drops and returned to work.
Emp was helping to remove a drill head. The drill head swung free and contacted the frame, pinching EE's left ring finger. EE received sutures and returned to work.
While opening the air filter access door on the 450 loader the door appeared to be fixed into the up and open position when in fact it was not secure. The air filter door fell and pinched the pad of the right index finger between a washer and door opening. The washer had been previously welded into place to use a lifting device for the motor hood on the loader.
WORKING ON CABLES FOR A TRANSFORMER, UNDER A CRAWL SPACE. EE EXPERIENCED BACK PAIN AND MUSCLE SPASM.
Emp. was changing a belt and caught a finger between the sheave and belt. Emp was treated onsite and returned to work that shift. On 5-1-17 Emp complained of pain and was taken to clinic where a small fracture of the tip of the finger was found. Incident became reportable at that time.
Employee was lifting wear liners for the coal mill repetitively. At the time ee did not feel this was anything more than a sore back from a long day of work. Two days after the heavy lifting ee notified supervisor that back was hurting and not getting better.
An employee at the Indian Creek plant was injured on Thursday October 13, 2016 when ee lifted a five gallon bucket containing approximately 15 lbs. of window washing solution. Employee received treatment from a chiropractor on Friday October 14, 2016.
Driver closed lid on top of trailer, lining up cam-lock on lid. Didn't realize finger was in latch. Closed cam-lock pinching left index finger, splitting end and taking off back portion of fingernail.
The employee was exiting the 660 Drill to go to lunch. He had spent the morning drilling on the Big Sage Road. As he went to close the door to the drill he lost his footing, he had his hand on the bottom of the door, which caused him to pinch his right index finger between the door and the frame. It was noted that he fractured the top of his finger required several stiches.
During bulk bag packaging forklift driver was mounting forklift to operate, to remove a palletized bag of lime from the loading machine. As he was engaged in reaching for his third point of contact his right hand impacted the handle mounted on the side of the chair injuring his finger. Employee taken to clinic for further assessment. X-rays showed broken pinky finger.
Installing 4x4x3/8 support angles in #1 preheater. Lost footing on rack. Angle smashed right hand against inner cone. Cut pinkie and ring finger.
Driver stepped into hydrate, which got into the left boot and caused a minor burn.

The miner was dumping lime dust when the latch on the rear gate of the dump truck wouldn't shut. The miner exited the vehicle to unstick the latch traveling thru the dust. The lime dust entered the miner's boots and was not removed before causing burns to the miner's feet. The burns were caused by the reaction of the dust and perspiration.
Contractor was removing refractory when brick slipped and landed on right thumb. Causing a fracture.
Employee pinched r little finger between rock and wall of crusher.
Employee was removing material from the crusher, when a large rock broke free and crushed the right ring finger.

Employee moving an electrical motor from #4 screen house to door. He felt a slight burning pain in left groin area.
Hyper-extended knee while walking down stairs.
Employee was emptying the baghouse hopper. The dust from it went up pant leg.
See Attached. Driller had removed drill steel from the coupler on the ECM-370 Drill and had placed the bottom on the pocket on the mast but failed to secure the top behind the hook. The driller then walked away when the unsecured steel fell striking the driller on the back of the head. Driller was taken to Health Center where he rec. stitches and then returned to work.
Injured rt elbow raking grates. Felt like he may have hyper-extended it. Later, when reaching 4 something, elbow locked up & was very painful when he tried to straighten it further. At time of injury, it was not considered a reportable, however, on 5/14/12, he had surgery on this rt elbow due to the initial accident, which now makes it an MSHA reportable incident
Employee was loading a truck with lime. The truck became overfilled and the spout plugged. The employee, in haste, pulled on the closing mechanism and tore the rotator cuff on his left shoulder.
EE was carrying scaffolding planks in his arms when he dropped the plank on the ground a wood splinter went into his right arm. He removed most of the sliver himself but a piece remained.
Employee shoveling coal when he turned & felt sharp pain in lower back.
Working with bags from bag house inhaled lime dust.
Employee was rodding plugged coal hopper with rod plug gave way and rod was pushed to metal side cutting finger.
Under investigation --- PLEASE NOTE: Injured was a contractor --- injured contractor would not provide last 4 digits of SS# or DOB. Per call for ampulation tip middle finger(75), ring of 1st joint(60) all together two are 135(left hand).
