Operator was pushing trees with 700H Dozer and a tree got stuck on a stump and swung around and shattered glass on the window. The glass shattered and a small piece got caught in EE's contact. The operator was given a tetanus shot and a prescription antibiotic. Returned to work immediately following medical treatment.

The injured miner was replacing dead batteries on the repeater in the quarry. EE jarred EE's right shoulder and felt pain. EE went to physical therapy and was eventually recommended surgery to repair a torn rotator cuff. The surgery occurred on 10/16/25 making this an MSHA reportable injury.
At 05:14 the Graymont kiln crew noticed a Quarry Services truck was on fire. 911 call was made at 05:16. The MSHA hotline was called at 05:24. At 06:05 after the fire diminished a loader dumped limestone fines on the truck which extinguished most of it. The Fire Dept. responded at 06:10 and put out what fire remained. The fire resulted in a total loss of the shuttle truck.

The injured miner was swinging a 10 pound sledge hammer to attempt to unplug a lime kiln dust (LKD) bin and chute. While swinging the hammer they felt a pop and pain in right shoulder/arm. They visited an orthopedist who released them to restricted duty. An MRI is scheduled for 12/15/2023 to determine the extent of the injury.
Employee was cleaning out the cab of the haul truck and slammed a thumb in the door while closing the cab door.

Employee was positioning a 40' extension ladder when the ladder began to tip. The employee caught the ladder instead of letting it fall. EE felt a tweak in their shoulder. No treatment occurred at the time. The injury was not improving, so EE sought medical evaluation in February 2022. Surgery was recommended for torn rotator cuff ligaments. Employee underwent surgery on 5/24/2022.

Operator attended safety meeting (6am) then performed inspection on loader. EE entered the cab and soon passed out in the cab (prior to operating). A haul truck operator secured them as EE was breathing but not responding. Ambulance came and EE was responding at that time. Earlier in the week the operator stayed home sick 2 days(dehydrated and they required IV when at hospital).
The employee was parking the grader in the shop to begin routine maintenance. When EE was climbing down the grader ladder (using three points of contact) EE lost EE's footing and grip with EE's left hand, EE caught self with EE's right arm/hand. EE originally thought EE just pulled a muscle but the range of motion and pain level continued to decline.

Driver was unloading coal at Indian Creek. Coal was frozen to the sides of the trailers and the driver was scraping and trying to beat the trailers (with mallet) to get some of the coal to break loose. Extended exposure to the freezing temperatures resulted in frostbite on both hands.
Employee was cleaning frozen chunks of coal and snow from the solid fuel grizzly when EE slipped and EE's foot/leg dropped into the grizzly. As the employee fell EE twisted EE's back.
Mechanic starts equipment in the morning. The mechanic was coming down after starting a haul truck and slipped on the ladder system. Sprained/strained right knee. The instance was noted at the time but EE did not seek medical care. Evaluation was sought in April and surgery was completed on 5/22/19 on EE's knee.

EE was removing bolts from a final drive on haul truck. Later that afternoon, back started bothering EE (strain).
EE used a come along to reposition a take-up pulley, this process was inadequate. So they got a crane. The come along was hooked onto an I-beam which worked with the previous direction of pull. As the crane passed the come along, direction of pull changed. Adjusting, the employee pulled the come along to the side, allowing it to fall off the I-beam causing an ulnar fracture.
Employee was using a air chisel to dislodge mud and debris from screen hopper. EE was thrusting at an upward angle causing a strain to right shoulder.
Climbing down from the 970 loader, operator missed the last rung and landed wrong (sprained knee). There was originally no ""injury"" as the medical evaluation found no injury. However, employee recently had surgery on knee and will miss a week of work recovering.
Contractor on site caused an unplanned fire in the Ametek building of Kiln #1.
Employee was using hammer to work on trailer gates (sprained shoulder).

Note: employee did not seek medical for significant time following actual date of injury. Med status for days away 1/9/2018. Installing cylinder on loader, the cylinder slipped and forced weight to the shoulder causing their leg to slip out from under them causing strain to hip/shoulder.
While opening the air filter access door on the 450 loader the door appeared to be fixed into the up and open position when in fact it was not secure. The air filter door fell and pinched the pad of the right index finger between a washer and door opening. The washer had been previously welded into place to use a lifting device for the motor hood on the loader.
Employee was climbing onto the track mobile using ladder. As they applied their body weight to the first rung they felt their left knee pop causing pain. They notified supervisor and was immediately taken to the BCHS for medical evaluation. BCHC recommended they see orthopedic Dr. employee commented their knee was sore prior to incident, scheduled to see a doctor on 07/14/2017.

Loading rock with loader...hit a hard spot and jolted the operator. The sudden stop caused shoulder soreness.
An employee at the Indian Creek plant was injured on Thursday October 13, 2016 when ee lifted a five gallon bucket containing approximately 15 lbs. of window washing solution. Employee received treatment from a chiropractor on Friday October 14, 2016.
Stepped off an excavator onto a rock and twisted knee. Employee did not get evaluated until a couple of months after the incident date. Then had surgery in late November, resulting in this report.

Installing 4x4x3/8 support angles in #1 preheater. Lost footing on rack. Angle smashed right hand against inner cone. Cut pinkie and ring finger.
Driver entered trailer (post-unload) to clean out residual coal from trailer walls. Lockout was not applied to gates. Electrical failure (assumed at this point) caused the gates to close. Driver wedged the shovel between gates but gates on his legs caused a broken right leg and soft tissue damage in the left. Note: the reporting period for the 7000-1 was inadvertently overlooked.

See Attached. Driller had removed drill steel from the coupler on the ECM-370 Drill and had placed the bottom on the pocket on the mast but failed to secure the top behind the hook. The driller then walked away when the unsecured steel fell striking the driller on the back of the head. Driller was taken to Health Center where he rec. stitches and then returned to work.
Injured rt elbow raking grates. Felt like he may have hyper-extended it. Later, when reaching 4 something, elbow locked up & was very painful when he tried to straighten it further. At time of injury, it was not considered a reportable, however, on 5/14/12, he had surgery on this rt elbow due to the initial accident, which now makes it an MSHA reportable incident
We believe that EE may have had a pre-existing condition as he had already had carpal tunnel surgery on his opposite hand prior to commencing work with Graymont. The work conditions at Graymont may have exasperated the existing condition, but to my knowledge we have never had a carpal tunnel claim at the Indian Creek Plant prior to this illness.
Employee was loading a truck with lime. The truck became overfilled and the spout plugged. The employee, in haste, pulled on the closing mechanism and tore the rotator cuff on his left shoulder.
While using a 51/2"" circular saw to cut wooden wedges 6"" in length from an 8' 2"" X 4"" board. The worker was injured when the saw blade hit a knot in the board. The saw kicked back & the blade cut his left hand breaking the 4th finger and lacerating the 3rd and 4th fingers.

At approx 20:10 a fire was discovered in bottom of coal silo (T90), while EE was performing his rounds. He contacted fire dept & on-call supervisor who contacted ass't plant mgr. Not knowing how long it was going to take to put fire out, ass't mgr told supervisor to have EE contact MSHA. Fire dept left plant site at 21:31. No one injured and no equipment damaged.

HE DIDN'T GET BETTER SO HE WENT TO DOCTOR ON 3-12-01. HE WAS TREATED BY A CHIROPRACTOR AND ALSO GOT A PRESCRIPTION FOR THE PAIN.
EMPLOYEE TRIPPED ON STEP COMING OUT OF COOLER PIT, TRIED TO GRAB HANDRAIL AND TWISTED LEFT SHOULDER. INJURED ROTATOR CUP. SHOULDER DID NOT GET ANY BETTER SO HE WENT TO SEE A DOCTOR ON 2-6-01. EMPLOYEE WAS GIVEN A PRESCRIPTION AND WILL BE TAKING PHYSICAL THERAPY.

THE EE TOOK A LADDER OUT OF THE HYDRATOR PIT AND CARRIED IT UP THE STAIRS. AT THE TOP OF THE STAIRS HE TURNED AND THINKS HE PULLED MUSCLES IN HIS LOWER BACK AND THE LEFT HAMSTRING GROIN AREA O F HIS LEG.
COWORKER WAS 3' UP ON A LADDER, DRILLING A HOLE. EE WAS WORKING BELOW HIM ON HIS HANDS AND KNEES, ON THE LEFT HAND SIDE OF THE LADDER. THE DR5ILL BIT THE COWORKER WAS USING BROKE, CAUSING HIM TO LOSE HIS BALANCE AND FALL ONTO EE'S BACK.
