Showing 50 of 69EE was attempting to rotate a rotor shaft manually to grease the bearing housing. EE placed EE's hands on the belt to move the shaft and when EE pulled EE's pinky finger was pulled into the motor sheave causing a laceration and a fracture to EE's pinky finger.
Employee sprained EE's back while cleaning spillage near primary crusher
EE claims injury (lower back sprain) occurred on 7/4/24. EE reported injury to company on 7/9/24. EE requested to seek treatment on 7/11. On 7/11 EE was taken to Concentra and received work restrictions of, "No shoveling, may lift up to 50lbs constantly, and May push/pull up to 100lbs constantly". On 7/11 the injury became a MSHA reportable.
Injury was reported to employer on June 20, 2023 at 12:30 PM. Employee states, in mid-May IE was helping another EE change the top screen cloths on an 8x20' middle deck. The other EE cut the screen in 1/2 while IE was pulling up on the half screen. While pulling up IE states IE's neck and spine began to hurt.

Employee was using a hose to washout an area and strained their back.
Employee tried to push blow bar out of way as it was moving toward the employee, straining left shoulder.
Loader operator got out of equipment and tried to pick up a 300 lb rock by hand.

Pipe fitting on water pipe was leaking, employee tried to tighten valve and pipe swung against the employee's arm, pinching between pipe and handrail. Employee has bruised and torn right bicep
Finger caught between drill bit & drivers sub while changing bit. Required stitches & may have slight fracture. Treated at local hospital ER & released. Middle index finger required 7 stitches.
Employee lifted bag off conveyor belt and back of thumb grazed sprocket on row pusher of conveyor. Conveyor was not working at time.
Hood of haul truck fell on to employee thumb because it was not latched correctly.

Employee claims to have injured left knee while climbing down from loader. Claim is very suspect, no witnesses and employee has been observed playing football. Investigation is ongoing.
Working in Pumping Station. Started to install 18" pipe with 90 on end, 7' long. Put pipe into position, started to leave it down, when hook on lug was noticed; the chain was under the hook. Chain came around hook, pipe jolted down, sprung back up and then down again, busting the rigging. Employee grabbed the pipe and smashed his thumb.

Employee was shoveling stone onto conveyor belt and felt discomfort in his back. He did not wish to seek treatment at that time.
Employee pinched pinky finger while trying to adjust wooden platform. Laceration to right pinky finger, four sutures.

Employee had hood of truck shut on thumb
Employee was helping to change head light on skid steer. Operator of skid steer moved forward to position light and pushed throttle forward with his body and ran over employees foot.
Employee worked 6/14 and 6/15 with no report of any injury. Was a no call/no show for work on Monday 6/17/13. Reported to work on 6/18/13 and said he might have hurt his back on Friday while lifting drill bits into pick up truck with another employee.
Employee opened hauler door and tore left arm tendon
Employee was changing screens on wash plant and when he tried to put screen over the rail part of the screen cut his wrist.

Employee pinched 2nd finger on left hand between roller foot and upper rail of man basket. Required 3 stitches.
Employee was trying to align a pin wheel and his right thumb became pinched, causing laceration.
Employee pinched finger while closing Mill door. Received 2 stitches in 3rd finger of left hand.
Employee was placing PTO onto ground when left little finger got caught between PTO and nut that was on ground. Finger was pinched. Employee required 3 stitches. No loss time or work restrictions imposed.
Employee was lifting and moving a sump pump from the waste pit to the service water pump house. When lifting the pump he felt a pull in his back muscle.
EE was unloading a boxed fan with metal strapping. He reached behind the box where his R index finger caught an unseen piece of strapping sticking out.
EE was cleaning feeder pans and belt on feeder #3. He attempted to pull himself up and over metal tubing to reach debris and felt a sharp pain in his L shoulder.
Employee was stretch wrapping pallets of bagged material and felt a sharp pain in his lower back. Pain subsided but the next morning he had trouble bending over.
Employee claims he heard a "pop" in his shoulder area while climbing ladder to tank 3. He reported a personal injury to his neck/L shoulder on 11/2/10 and then on 11/4/10 reported the injury as having occurred at work on 10/29/10. Employee was placed on restricted duty by physician from 11/2/10 until 11/8/10.
Employee was lifting steel C Channel (@75 lbs) up to a saw to cut down for a sign. He strained his back but waited until 8/3/10 to seek medical attention. Possible disc herniation.
EE was welding a plate on the crusher and a piece of slag fell into his left boot and burned the side of his left foot. He did not report the injury until his leg began to swell several days later due to infection.
Employee was asked to use a shovel to clear dirt covering a pipe. When he lifted the shovel at one point he felt a tightening and pain in his back.
