Employee has been diagnosed with recordable hearing loss in right ear. Loss has been progressive since 2004.

EE was using a sledge hammer to tighten down nut on crusher mantle liner. He struck his right thumb nailbed with the hammer.

Employee was tightening up a come-a-long and the chain broke. Chain hit employees rt hand, and broke rt middle finger.

Employee was asked to use a shovel to clear dirt covering a pipe. When he lifted the shovel at one point he felt a tightening and pain in his back.
