Employee got EE's finger caught between a gate lever and handrail. Employee lacerated EE's finger and received 8 stitches. Employee was returned to regular duty.

While installing a chute to a steel frame, Employee improperly placed hand in a pinch point area causing a laceration to index and fiddle finger on left hand, requiring sutures. Employee returned to work with no restrictions, No Lost days.

Employee was cranking the handle on a winch when EE's arm came in contact with a cotter pin. The cotter pin cut the arm of the employee which required 3 stitches.

Employee was changing the cutting edge on a 988 loader. Employee was holding the bolt up into shoulder of the cutting edge to keep the bolt from turning. Bolt moved down and turned in employees hand causing a cut. Employee received stitched in hand.

Employee was carrying punch plate screen with another employee, it pulled away from other employee and caused injured employee to drop his side. The plate smashed his hand cutting open his middle finger.

Loader operator returned to his loader after lunch and turned on the air conditioner. When the air conditioner fan turned on, it blew out some dust and small debris into his face. Employee flushed out his eye and returned to work. The next day his eye was still irritated so he went to the doctor and was treated for a scratched cornea and prescribed antibiotic eyedrops.

Employee was changing screens on a screen deck and hit his head on an angle iron bracket causing a small laceration to his scalp. Laceration was sutured and employee return to full duty on 07/08/2011 after a safety shut down and repositioning of angle iron brackets.

Employee was removing the face plate of the gravel pump. The face plate was guided by studs. The face plate slipped past the studs and strcuk employee's left index finger. The employee recieved sutures in his letf index finger and returned to work.
