Miner was moving a haul truck wheel chock and felt pain in EE's left shoulder. EE missed no days of work but has now been put on restrictions from EE's Dr. that won't allow EE to perform EE's work duties and will be off work until further notice as of 1/20/2026.

Was working inside of crusher chute when a lodged piece of metal fell striking EE below EE's hardhat causing a cut.

Was working on a return-roller hanger bracket, when last bolt was removed the weight of belt caused the bracket to drop rapidly and broke EE's middle finger on right hand.

Was replacing a return roller on a conveyor, used a wooden board for leverage to lift belt and hurt EE's shoulder. Continued working regular duty but found out EE will need surgery.

Employee stated through the course of duties changing screens on a shaker deck on 5/22/2020 that employee's knee tightened up. Employee thought it would be fine, but 5-hours into the shift on 5/23/2020 employee reported the injury.

Manual Labor, shoveling, clean up. Miner has a left inguinal hernia.

While clearing jaw crusher a large rock shifted into miners leg and foot, causing contusions to leg and foot

Miner was changing parts on a crusher when a part started to fall. The miner tried to hold onto the part. The miner strained left shoulder.

The primary jaw had bridged over causing rocks to overflow the jaw. The injured miner was cleaning rocks off the deck and misjudged the distance between the rock in his hand and another rock. The result was a broken left index finger.

Employee fabricated a lifting device out of steel to lift a sheave for a v-belt drive assembly, upon lifting the sheave one of his weld broke and the sheave dropped a couple feet hitting the ground and rolling over his right foot, thus he incurred a bruised right foot needing a few days to recover.

EE was removing a return roller and when he rolled it out of the bracket he got his middle finger on his left hand slightly smashed between the roller and the roller bracket. We weren't even aware he went to the clinic until he turned paperwork in 3 weeks later.

Employee was the checking the gates on a bottom dump Haul Truck, when he instructed another employee to close the gates his thumb got caught between the cylinder and the ram.

WHILE OPERATING DOZER, IT OVERHEATED. EE FOR SOME REASON DECIDED TO LOOSEN AND TAKE OFF THE RADIATOR CAP WHILE THE ENGINE WAS STILL HOT.

WHILE EE AND ANOTHER EE WERE BUILDING A HOPPER THE 2 SIDE PANELS WERE BEING HELD BY TACK WELDS WHICH BROKE WHEN THE PANELS WERE BEING ALIGNED CAUSING ONE PANEL TO FALL ONTO EE.KICKERS WERE ALS O IN PLACE.
