Miner stated that a piece of angle iron fell off the steel rack and smashed fingertip between the rack support.

Miner has had an audiometric standard threshold shift in R-ear, pending retest, results won't be available within the 10-day reporting criteria. Miner has been exposed to work noise for 12-years and wears ear plugs for protection. Miner experiences ringing in their ear.

Struck by flying object

Miner was breaking oversized rock with a sure-strike attachment. Though the excavator was boomed out fully, fly rock hit & broke both the windshield & plexiglass protective shield causing miner to get glass in eye.

Miner stepped on a rock and rolled ankle when walking down stairway to primary.

Tightening a nut with a pipe wrench when the wrench broke causing employee to fall back onto catwalk injuring arm and hand.

States pain occurred in left chest and ribs while swinging a sledge hammer.

Was working on a return-roller hanger bracket, when last bolt was removed the weight of belt caused the bracket to drop rapidly and broke EE's middle finger on right hand.

Miner claims to have slipped stepping down an A-frame ladder & injured shoulder at the primary, but wasn't witnessed by those working in the area. Didn't mention it to foreman until 1/5/2021, foreman did not take action as no medical issue was stated. We were notified by the staffing agency of the injury on 1/19/2021.

Miner was engaging the manually operated locking pins to secure the bucket on a skid steer. When EE initially pushed on it the handle got stuck. EE applied more pressure when it broke free the handle pinching EE's finger, fracturing it.

Removing a gear component when a metal fragment struck miners left arm, causing a laceration and embedding in arm.

Miner was reinstalling a troughing roller when it fell striking left hand. Fractured left middle finger, laceration left middle finger.

While clearing jaw crusher a large rock shifted into miners leg and foot, causing contusions to leg and foot

Miner was changing parts on a crusher when a part started to fall. The miner tried to hold onto the part. The miner strained left shoulder.

Employee was prying worn rubber that had been siliconed to a chute with a crowbar. When the rubber came loose the crowbar slipped from the employee's grasp causing the bar to drop to the ground which caused the employee to drop to his knees striking his left knee on the center hold down bolts of a screen.

EE WAS USING A BOTTLE JACK AND A PIECE OF PIPE TO RAISE THE ENGINE ON A LOADER. HE RAISED THE FRONT OF THE ENGINE AND INSTALLED A 1"" PLATE TO MAKE ROOM TO REMOVE THE OIL PAN. THE EMPLOYEE LOWE RED THE JACK AND REMOVED THE JACK AND PIPE. THE PIPE SLIPPED OUT OF HIS HAND AND FELL ON HIS ARM CAUSING A LACERATION TO THE LEFT ARM.
