Over the road driver was removing a tow rope from the mounted bracket on a loader. Employee began lifting the tow rope overhead when they felt a pop in their left shoulder, followed by immediate discomfort in that shoulder. Employee was evaluated and diagnosed with a strain to their left shoulder.

The employee's leg was struck by a shifting slurry hose while disconnecting a coupling. They went to a doctor and was diagnosed with a tear in their ACL. The injured employee had surgery on 9/4/25 which made this injury reportable.

Haul truck driver reached between the seat and counsel to get a part, when EE pulled EE's hand out it was cut between the middle finger and ring finger between the knuckles, causing EE to get stitches. Operator returned to work the next morning.

Employee was conducting maintenance on a disconnected rotary seal valve. Employee spun the internal rotor manually with EE's left hand to check tolerances, when the middle finger of EE's left hand was caught between the vein of the rotor and the housing of the rotary seal valve. This resulted in a fingertip amputation just below the nail.

EE was assisting with carrying an electrical cabinet. While re-positioning after walking through a doorway, EE twisted right arm and felt a pop in right elbow. The injury was initially treated as first aid with no restrictions. On 8/16/2023, EE visited a medical clinic and work restrictions were given.

The employee was pulling a power cable to a pump barge from shore. The employee did not know they were injured until the next day when they woke up and had lower back pain. They then informed their supervisor and the injured employee was brought to the Doctor. A MRI was scheduled for 12/5/22. After the results were read EE was given Physical therapy on 12/8/22 2 weeks.

Miner was repositioning an overhead loadout spout to load a railcar when the spout was caught in a bind on the railcar top chute door. The miner attempted to pull the spout over the chute door lip when the miner's left hand was caught in between the spout and the railcar door chute.

Employee was adjusting the armrest inside the L60H front end loader, they grabbed the lever with left hand and pulled up quickly as the lever was stuck and knuckle contacted the track for the arm rest and lacerated the knuckle. Employee received 4 stitches.

Technician was tapping on dust collector piping when debris fell between EE's eyes and EE's safety glasses. EE immediately rubbed the right eye and started to feel some discomfort. EE finished the shift and went home. EE woke up that afternoon and EE's eye was irritated and red so EE flushed it out numerous times.

The employee was dumping a dust collector hopper. The hopper would not return to the locked position, so the employee tried locking it manually. In doing so, EE's finger was caught between forklift mast and hopper, transported to the hospital: tuft fracture and stitches to EE's thumb.

Due to employees' failure to follow company policy for fall protection, EE fell off a truck. The employee was transported to the medical facility, where they received stitches to right knee. EE also has lacerations on chin, shin, a bone chip in wrist, and a chipped tooth.

The employee was removing excavator teeth from a metal bin. As EE was removing a tooth, EE's left hand struck a piece of sheet metal. The sheet metal cut EE's left index finger. As a result, stitches were required.

The injured employee was treated with first aid measures and EE's pain did not improve. EE was seen by a doctor on 8/12/2020 and issued work restrictions.

In the process of loading a railcar, the affected employee received a laceration on right hand due to inadvertent contact with the loadout spout. The laceration required 5 stitches to close which prevented the employee from using that hand for the employee's job.

A polyvinyl alcohol (PVA) holding tank and pipelines were being flushed. Investigation shows that the employee was standing on a flange 2 ft. off ground to adjust an overhead valve. EE's foot slipped and turned a ground level valve that sprayed a mixture of 99% water and 1% PVA at a temperature of 210 F onto EE's left inner calf resulting in second-degree burns.

EE was removing a 1.5" aluminum fitting from a steel pipe collar. The fitting broke and separated from the threads inside the collar. The momentum cause the EE to strike an elbow on a 4" gate valve located behind EE. The injury was treated as a First Aid. EE was evaluated and had X-Rays on 12/17/19. The X-Rays revealed a chipped bone.

employee was hooking up a new 2 1/2 inch hose to a booster pump located at the plant. The hose was brand new and was raveled and kinked up prior to putting water pressure to it. The employee had a grip on the hose as the pressure increased it began to unwind, the hose caught the employee's right thumb and twisted it.

Two EEs were changing out the screens at Rotex 5186. In the process of removing a screen the injured EE received burns on forearms. 1st AID measures were taken by plant personnel, recommended by Axiom Medical Service. Subsequently, EE was seen by MD for follow up evaluation on 9/18/19. The MD would not allow the EE return to work on EE's next assigned work day of 9/19/19

Employee was working in the bagging/packaging area. The table for bags dropped and left a 100 pound bag of material suspended. Employee opened the door to catch the 100 pound bag before it fell on the floor. As employee attempted to grab the bag, the 100 pound bag pulled EE's left arm backwards. Employee started physical therapy on 10/15/2019.

EE was walking up stairs, inside the Mill Building when EE felt/heard EE's Knee "pop". The injury was treated as a first aid. Several months the EE stated that EE's knee was giving EE problems again. EE was examined by a Dr. in October 2019 and sent for an MRI. Surgery was performed on the Knee 1/24/2020 to repair soft tissue damage.

SIC EMPLOYEE was unloading the boxes, they reached over the side of the truck and into the bed to lift the box out. When they brought the box up and over the bed of the truck they felt a slight catch in their back. On 2/11/19 employee went to Occupational Health (A Lexington Medical Center Practice where they were given a prescription for pain.

Employee finished installing the last fastener during installation of a wire mesh conveyor guard. EE took off gloves to install the fastener. Employee stood up from kneeling beside the guard panel, EE's bare hand brushed across a spliced area of the expanded wire mesh on the guard panel. EE's right finger received two extensive lacerations which required stitches.

While pulling a water hose across the floor it became tangled causing it to jerk. The jerking motion resulted in pain to the employees right shoulder. The employee went to the doctor on 10/08/2018 at which time employee was given restrictions and referred for physical therapy.

Employee, along with 3 others were repairing splice on Thayer belt feeder. While removing tail pulley shaft, tail pulley shifted trapping left ring finger between tail pulley drum and bearing assembly. Employee's left ring finger fractured. Restricted duty - no lost time.

Employees were removing a hopper with a chainfall attached. With the hopper secured, last bolt was removed, the employees did not realize hopper was top-heavy, hopper flipped back and crushed employee hand between hopper and HDPE pipe that was below.
