EE had their pinky pinched in between a piece of a wear steel plate and the side wall of a chute. Injury did not become reportable until treatment by a physician on 10/7/2025 where restrictions were issued.

Showing all 44The worker was reinstalling a spring onto a tension pulley system. During this process the worker was holding the pulley up with EE's right hand and installing a plate with EE's left hand, when the pulley slipped from EE's right hand and made contact with EE's left thumb.
Approximately 6:05 AM the employee was unlocking the gate to the entrance of the pit. The employee inadvertently stepped on one end of the chain and as they pushed the gate forward it caused the other end of the chain to uncoil and whip around the gate striking the employee in the nose/eye area.
Employee attempted to manually roll and lift rock out of haul road. Employee reported to Employer injury after end of shift, employee sent for medical evaluation following morning.

The Miner was assisting another miner carry a 80 pound screen bearing from a storage conex to a cart for transport to the work location. The bearing started to slip off the cart, as it was moving the miner stepped back to avoid being struck by the bearing. As they stepped back steel protruding from the conex struck their left leg just above the top of their boot.
EE was helping move, roll-out, and cut old conveyor belting for disposal. While EE was manually moving some belting EE felt a pop in EE's lower back resulting in a lower back sprain.
Hernia when lifting a bead flange for a Cat 980
Employee was handling a piece of wear steel and cut though company issued leather gloves cutting right index and middle fingers requiring stitches.
EE stood up from a knelt position while repairing a conveyor belt and felt a small amount of pain in right knee. EE then climbed into the ford F-350 service truck and once ee place heel on the floor board ee felt a pop in right knee.
Employee was replacing a screen panel when it slipped out of left hand and landed on right pinky finger, causing a puncture to right pinky finger.
The driver was trimming EE's load on an end dump trailer. EE closed the tailgate using the in cab controls and then went to the back tailgate to clear the seal so the tailgate could shut. EE placed EE's hand on the dog ear on the tailgate when the tailgate closed on EE's left hand crushing EE's hand and causing a laceration to EE's hand.
Employee was replacing a guard on a tail pulley and strained shoulder and lower back.
Changing screens hit elbow on steel cross member of screen
Employee was changing screens and got the tip of his right ring finger pinched underneath the screen railing, causing the finger to bruise and swell. Injury was reported and the employee was taken to Concentra to get finger looked at, x rays were taken and the tip of the right ring finger was broken.
Employee picked up a 140 pound log washer part and threw it causing a lower back strain.
Employee was picking mud and sprained / strained right wrist
Employee, blaster, was dewatering blast holes, when he went to lower the dewatering pump into a blast hole a worn spot on the dewatering hose showing metal cords caught his middle right hand finger. He tore the skin in a v shape 1/2" by 1/2". Employee was sent to the clinic and had 5 stitches to close up the wound.
Employee was holding a piece of steel (crusher mainframe liner) underneath crusher #2. The piece of steel was held on to the mainframe by some welds. As the other non injured employee was cutting it off, the steel recoiled and pushed injured employee's arm back into his shoulder.
Employee came into my office on the 2nd of Aug. complaining of irritation in his eye that happened on the 1st of Aug.
Replacing idler on CB2 tail section. As EE was tryng to line up the idler frame with the conveyor frame, the idler slipped off the frame hitting EE's finger.
Employee was lifting a 5 Gal container, shoulder height. As he lifted the container he felt a small twinge in his left forearm.
EE lifted large rock. Felt sharp pain in shoulder (left) approx. 1 hr. later.
EE was resting a 20 am motor on a handrail while still confected to the pully system, as employee tryed to move motor to the ground. The motor slipped off the rail pinching his hand in between the hand rail and the motor shaft.
Employee was working on his drill hammer. He bent down to stand it up on end when he had a sharp pain to his lower back.

EE GOT AIRLINE DEICER IN HIS EYES HAD IRRITATION AND WAS GIVEN RX AT CARE PROVIDER TO ELIMINATE IRRITATION AND THEN RET'D TO WORK. EE WAS WORKING WITH ALL OF HIS PPE ON AT THE TIME OF ACCIDENT
EE WAS DRIVING A COMPANY PICK UP WHEN IT OVERHEATED. THE EE STOPPED THE VECHICLE AND STARTED TO REMOVE THE RADIATOR CAP. THE EE THEN CLOSED THE CAP AND THEN REOPENED IT AND RECIEVED 2ND DEGREE BURNS ON HIS ARM BETWEEN HIS WRIST AND ELBOW ON HIS RIGHT ARM.
CHAIN SPRINGS ON SPRING PORTA POWER SITTING ON WOOD BLOCK, WOOD BLOCK FAILED LETTING PORTA POWER SLIP, SCREEN DROPPED STRIKING HAND.
HE WAS POSITIONING 10" PLASTIC PIPE TO BE FUSED TOGETHER AND SPRAINED LEFT WRIST - PULLED TENDON.
EE WAS PICKING MUD OFF CONVEYOR BELT AND HIS RIGHT HAND BEGAN BOTHERING HIM FROM PICKING MUD. EE WAS DIAGNOSED WITH A SPRAINED RIGHT WRIST AND SLIGHT CONTUSION.
EE WAS REMOVING RETURN ROLLER FROM UNDER SCREEN RETURNS. HE LIFTED ROLLER. HIS FINGER WAS SMASHED BETWEEN THE ROLLER AND SCREEN AXLE. EQUIPMENT WAS NOT OPERATING AT THE TIME.
EMPLOYEE WAS MOVING SMALL TIN BOX HAT CONTAINED WELDING ROD HOLDING BOX BY HOLE CUT IN TOP. BOX SLIPPED FROM HIS HAND CUTTING LEFT INDEX FINGER.HE RECEIVED THREE STITCHES.
LIFTING A HEAVY OBJECT IN AN AKWARD POSITION.
IT WAS DARK, EE WAS CLIMBING OFF OF A 773B CAT HAUL TO TAKE A BREAK. BENDING OVER TO GO DOWN THE LADDER ANTENNA POKED HIM IN THE RIGHT EYE. SAFETY GLASSES WERE NOT USED.
WHEN A WRENCH WAS HANDED TO THE EMPLOYEE IT ACCIDENTALY STRUCK HIM IN THE MOUTH.
EE WAS WORKING ON JAW CRUSHER CONVEYOR BELT KNEELING BY THE TAIL PULLEY ON RIGHT KNEE. THE NEXT MORNING VERY SORE & BADLY SWOLLEN
EMPLOYEE WAS REMOVING ROCKS FROM A PLUGGED CHUTE ABOVE THE ELJAY CRUSHER, HE LIFTED A ROCK IMPROPERLY AND STRAINED HIS BACK, CAUSING LIMITED MOBILITY FOR A SHORT TIME.
EMPLOYEE PATCHING WIRE CLOTH SCREEN, HAD DIRT FALL ON FACE AND HARD HAT. EMPLOYEE HAD PIECE OF EITHER IRON OR IRON ORE LODGE IN CORNEA.

EE WAS REMOVING A SLING FROM A CRANE HOOK. WHEN THE SLING CAME OFF THE HOOK IT HAD ENOUGH STORED ENERGY TO WHIP AROUND AND CUT HIS LIP.
CHANGING SCREEN ON CEDAR RAPIDS 6 X 20 SCREEN, EMPLOYEE INSTALLING SIDE RAIL BOLT HAD FINGER ON BOLT HEAD WHILE BOLT WAS BEING TIGHTENED, AND BOLT HEAD SPUN, CUTTING FINGER.
EE WAS REMOVING A SMALL SCREEN FROM AN AGGREGATE SHAKER WHEN HE CAUGHT HIS THIRD FINGER LEFT HAND ON SHARP EDGE.
EE AND OTHER WERE CLEANING UP PLANT AREA PUTTING. USED SCREEN ON BACK OF FLAT BED TRUCK. EE SMASHED HIS FINGER BETWEEN SCREEN AND ROLLER ON TRUCK.
