Over-exertion in lifting objects

EE was helping move, roll-out, and cut old conveyor belting for disposal. While EE was manually moving some belting EE felt a pop in EE's lower back resulting in a lower back sprain.

Struck by flying object

The employee was welding on a water truck frame when a spark went behind EE's welding helmet and glasses and struck EE's eye. The employee had 2 pieces of embedded metal removed from EE's right eye by the doctor. Employee reported injury 5/3/18. No time missed, employee was wearing welding jacket, helmet, safety glasses, steel toed boots, and leather gloves.

WHILE OPERATING A SKID STEER EMPLOYEE HIT A POT HOLE IN THE ROAD CAUSING IT TO BOUNCE AND JOLTING HIS BACK RESULTING IN PAIN BY THE END OF THE DAY

WHEN BRINGING A SCREEN CHUTE INTO THE SHOP FOR REPAIRS THE CHUTE SHIFTED ON THE FORKLIFT AND STRUCK EE UPON THE WELDING HELMET AND SHOULDER KNOCKING HIM TO THE GROUND AND WAS UNRESPONSIVE FOR APROX 10 SECONDS. HE SAID HE WAS OK BUT 911 WAS CALLED ANYWAY AND WAS TRANSPORTED TO BE CHECKED OUT. HE SUFFERED Cervical, hip AND RT SHOULDER STRAIN

When reattaching a spring on a conveyor the pliers slipped and the resulting release caused his hand to strike a bracket corner causing a laceration on his left index finger, requiring 9 sutures

WHILE MOVING A BARREL OF GREASE IT TIPPED TOWARDS EMPLOYEE AND HE PUSHED IT UP BEFORE IT COULD TIP OVER ON HIM AND HE FELT A TEAR OR STRAIN IN HIS STOMACH AREA AND WHEN HE LATER DECIDED TO GO TO THE CLINIC THERE WAS A DIAGNOSIS OF AN ABDOMINAL HERNIA

While descending a three step ladder, subject slipped on the second rung and missed the first rung and upon contacting the ground suffered a knee strain.

When dismounting a conveyor, subject tripped and caught the handrail with his hand, causing an injury to his elbow. He did not think that it was very serious but by the time he got home that night his elbow was swollen and sore. He went to an urgent care the next day (Saturday) and received treatment for an elbow strain.

While cleaning a grate, employee received smalll cut on the hand between the thumb and fore finger. Employee recieved 4 stiches. He did not miss work.

EE WAS REPAIRING A HYDRAULIC LINE ON A LOADER. AS HE CLIMBED DOWN OFF THE LOADER HE LOST HIS FOOTING AND FELL 2 TO 3 FEET. HE PUT HIS HAND OUT TO CATCH HIS FALL AND HIS RIGHT WRIST BROKE UPON IMPACT.

EE WAS ON THE GROUND INSPECTING A CONVEYOR BELT WHEN HE SLIPPED ON THE ICE. WHEN HE HIT THE GROUND HE DISLOCATED HIS RIGHT MIDDLE FINGER. THE FINGER WAS SET AND SPLINTED - RESTRICTED DUTY.

EE WAS ROLLING A BARREL AND STRAINED HIS SHOULDER. RECEIVED 10 DAYS OF PRESCRIPTION PAIN.

EE WAS CLEANING ALONG THE FENCE LINE AND STEPPED ON A BOARD WITH A NAIL IN IT. THE NAIL PENETRATED HIS FOOT. MEDICAL TREATMENT CONSISTED OF A TETNUS SHOT AND A SHOT OF ANTIBIOTICS.
