Employee was going for a pick on a water pump. The employee was holding the chain for the Telehandler to slide the fork between. As the fork was in position for the chain to slide over the fork employee's finger got caught between chain and fork pinching employee's finger.

Showing all 8Employee was placing outrigger pads into storage locations. When placing pad into storage box, employee felt a sharp pain in lower back.
Employee was replacing v belts on screen. When lifting the motor up with the lifting eyelet on the motor, employee was placing belt back into the correct sheave when the eyelet sheared off. Employee thumb got pinned between belt and sheave.
Employee was performing task inside screen. When he entered the screen he was wearing his safety glasses. Due to heat and humidity, he could not keep his glasses from fogging over and removed them to complete his task. While he had his glasses off, sand fell from the above screen and fell into his left eye. He tried washing the foreign matter out of his eye but was unsuccessful.
Unloading PVC Drill Casing, casing fell on finger
EE was removing a wear plate in crusher, lifting with a mobile device. The wear was bound on a chute corner. When EE attempted to free the wear plate, it slid out striking the EE on the right knee causing a laceration to the right knee.
Employee was assisting in v-belt replacement, hand (right) was pinched between pulley & v-belt. Equipment was not running & locked & tagged.
Employee was loading cutting blades onto a forklift to be moved; when he went to set the blade down he caught his finger with the other blade.
