Employee was operating a Komatsu 605 haul truck being loaded by a 395 Cat Excavator. When the boulder was dropped into the bed of the truck, the impact caused the haul truck operator to have severe pain in EE's lower back and legs.

Employee was pulling on a pry bar trying to move a wear plate into place when the pry bar slipped and the end of it scraped across the employees face cutting their cheek.
Employee was operating a Cat 374 Excavator when a rock fell from the highwall striking the bucket and ricocheting into the cab of the excavator. The rock struck the employee in the leg breaking EE's left tibula.

Employee was going for a pick on a water pump. The employee was holding the chain for the Telehandler to slide the fork between. As the fork was in position for the chain to slide over the fork employee's finger got caught between chain and fork pinching employee's finger.
While changing cutting edges, employee was holding down the head side of bolt (which is flat) and using an impact gun to loosen nut. Bolt came loose and came up causing it to spin and cut through leather glove and into the side of the palm of EE's left hand.

The employee got dizzy & disoriented. The crew helped the employee into the truck & got employee some water & got ice packs on emp. to cool emp. off. It was then decided that emp. needed to go to the hospital so the ambulance was called. Emp. was picked up around 3:30pm & transported to the hospital where they administered fluids through IV's. The Temperature was 112.

Helper parked truck by drill on other side of shot berm. Then crawled over berm to get to the drill. As EE was going over the berm, fell and broke arm. Helper should have walked around to opening on berm instead of going over it.

Employee was removing bearing from housing with a hammer and punch. The bearing shelled when struck and metal fragments were propelled through the air. Employee was struck in the stomach area with a small piece of metal.

Employee was operating a John Deere 300D end-dump haul truck while performing dirt stripping activities. Employee was pulling away from dump-site when a hydraulic line supplying pressure to the bed ruptured causing the bed to slam on the frame of the tractor. Employee didn't wish to seek medical treatment and completed tasks for the entire shift.
Employee was performing task inside screen. When he entered the screen he was wearing his safety glasses. Due to heat and humidity, he could not keep his glasses from fogging over and removed them to complete his task. While he had his glasses off, sand fell from the above screen and fell into his left eye. He tried washing the foreign matter out of his eye but was unsuccessful.

Employee was in process of unbolting a hydraulic motor on the Plant to replace it. With 2 bolts remaining the motor unexpectedly began to fall. The employee caught the 80 pound motor with the top of the impact wrench causing it to jolt his right shoulder.

On 8/30/10 employee, while dismounting off of the LULL forklift stepped on a rock approximately 3"" and rolled his left foot causing him to fall and land on his left arm. The impact hurt his foot and arm requiring him to get it checked out by a doctor.
On 8/30/10 employee was greasing the bottom right pin of a 988G when the operator tilted the bucket back rolling it onto the right part of the right foot smashing his foot. This crushed the two right bones going to the two right toes just behind his toes.
On 4/29/10 EE reported that back on 3/3/10 he was sweeping rocks off of the pit floor with the 988G loader he was operating when he caught the bucket on something solid causing a sudden stop of the loader. When this happened he said it felt like it ripped his insides. The accident was not reported to the supervisor until 4/29/10.

EE was removing a wear plate in crusher, lifting with a mobile device. The wear was bound on a chute corner. When EE attempted to free the wear plate, it slid out striking the EE on the right knee causing a laceration to the right knee.
