The employee was flipping the lid of a grid box when they felt a sharp pain in upper left arm. They requested help from company EMTs and was then transported to hospital. The hospital released them that night and advised them to follow up with Primary Physician and placed them off work until 4/4/2026. The employees Primary put them off work until they are cleared from Orthopedic.

Showing all 50Employee was working on a rock truck when a cordless drill fell from the top of the engine and landed on EE's right hand causing a fracture to the end of EE's finger. EE went to the ER and had a splint placed on EE's injured finger.
Employee picked up a piece of metal and stated EE felt something pop in their left shoulder. The Employee continued to work, and said it was ok. On 1/24/2025 EE informed the foreman that EE had scheduled a surgery for their shoulder for 1/28/2025.
Employee was rolling up the grease line when EE lost control of the hose, the handle caught EE's hand and caused a strained tendon.

Employee was replacing a starter on a Rock Truck. The employee stated it was a hard area to access. When EE lifted the starter, EE felt something in their back. The employee did not report until 11/28/2022 and did not go to the Doctor until 12/5/2022. EE has a pinched nerve in their back.
TRESSPASSER ON PROPERTY. GUARD WAS STANDING AT BACK OF THE VEHICLE WAITING FOR TRESSPASSER TO STOP. THE TRESSPASSER HIT THE GUARD PINNING EE BETWEEN VEHICLES CAUSING GUARD TO HAVE HIP PAIN.
Employee was lifting a boring bar and trying to slide it in the bearing. As they were lifting and pulling they felt a sharp pain in the groin area. EE was transported to the hospital where they were diagnosed with a hernia. The boring bar weighs approximately 75lbs. EE returned to work the next day and hasn't missed any work yet. Restricted to no lifting over 15 lbs.
Oiler was changing dust filters on highwall drill HD29. Due to the filters being wet they were heavier than normal. While changing the third filter EE's left hand was under the filter and when the filter dropped it was too heavy to hold and caught EE's hand between the dust box and filter. EE suffered a broken left pinkie finger with lacerations.
Employee was cleaning out the floor of a 993K Caterpillar Front End Loader. EE hit something sharp in the floor behind a steering control hose and cut EE's middle finger. EE was taken to the ER and glue was applied to the cut. EE returned to work the same evening.
Changing conveyer belt on rock crusher, one mechanic manually rolls the counterbalance wheel, as the belt was being rolled the injured technician failed to remove a hand and sustained a laceration to the 3rd finger on right hand.
Employee was attempting to connect a man-basket to a crane. When employee turned a screw type lever that adjusts the basket the arm fell on employee's left hand breaking the ring finger.
Mechanic used a chain as a puller to replace center bushings and chain broke and hit mechanic in the mouth. The lip was split open and employee was taken to hospital where it was sewn back up. Mechanic tried to come back to work the next scheduled work day but was advised to take a few days off with pay to let it start healing.
Employee was picking up a track pad and felt a pull in EE's left groin area. EE didn't go to the Dr until 3/4/2020 and was diagnosed with a hernia. EE didn't receive any treatment and has not missed any days of work.

While attempting to loosen an oil filter under a 993 loader, employee strained his neck. Employee went to the doctor on 2/20/2015 and the doctor put him on restricted duty. Note: EE went to Dr on 3/13/15 and the Dr took him off work beginning 3/13/15.
While installing a jack on a loader injured was attempting to align jack eye with ears when the strap holding the jack slipped causing the eye to fall catching his finger between eye and ears of jack
While carrying an axle to a D11 dozer, employee laid it on the dozer track and caught his finger between the axle and track
EE STATES WHILE PUTTING A LOCKRING ON THE INSIDE WHEEL OF A ROCK TRUCK MY RIGHT SHOULDER POPPED. I HAD PREVIOUSLY HURT MY RIGHT SHOULDER WHILE PUTTING THE LAST BEAD ON A SKIDDER TIRE ON 6/7/13 BUT INJURY WAS NOT REPORTED TO MANAGEMENT.
While attempting to drive a wedge between loader bucket and wire strip the wedge sprung back striking employee on the cheek resulting in a laceration requiring stitches.
On 4/20/11 employee was working on dozer. He placed his right elbow (which had a calcium deposit on it) on the grouser on the track and his arm rolled off and the calcium deposit moved. He told his supervisor what had happened and that this was not an accident. He refused to fill out accident report and he said no because it wasn't an accident.
While installing a pin in PC5500 bucket the bullet slipped while being removed and struck employee's finger resulting in a laceration.

Mechanic was lifting a part from the mechanic truck to make repairs on equipment - EE did not stop work nor go to the doctor at that time - on April 27, 2005, called in and said was going to the doctor as his back was hurting
The employee call the supervisor on this job on January 9, 2005, to report this accident, he did not mention anything to him on the date he said it happened and continued to work that shift as well as every day that week. The employee said that his knees were sore, he was pulling the hose for grout down the hill and hurt his knee.
EMPLOYEE WAS INSTRUCTING A CONTRACT EMPLOYEE AS TO THE PROPER OPERATION OF A BACKHOE. WHILE BEGINNING TO SWITCH POSITIONS IN THE MACHINE, EE WAS ATTEMPTING TO ADJUST THE SEAT BY USING THE DOOR FRAME TO PULL SEAT INTO POSITION. DURING THIS THE CONTRACT EE CLOSED THE DOOR, INJURING THE EMPLOYEE'S FINGERS.

EE WAS REMOVING THE REAR END FROM A FUEL TRUCK. WHEN REMOVING THE LAST BOLT THE REAR END BEGAN TO DROP. HIS HAND BECAME CAUGHT BETWEEN THE REAR END AND HIS 1" DRIVE IMPACT WRENCH.
REMOVING A STUCK SPOOL IN A DUAL TILT CONTROL VALVE AND SPOOL RELEASED MASHING FINGER BETWEEN SPOOL AND METAL ON HYDRAULIC HOSE.
EMPLOYEE WAS HANDING UP A CAB FILTER TO ANOTHER MECHANIC ON MACHINE. FILTER SLIPPED FROM HAND. HE TRIED CATCHING FILTER AND LEFT MIDDLE AND 4TH FINGERS CAME OUT OF SOCKET, SWOLLEN & PAINFUL.
EE WAS ASSISTING A WALKER MACHINERY EE IN LOADING A DOZER IDLER INTO A PARTS TRUCK. WHILE ATTEMPTING TO HOOK A CHAIN TO THE IDLER FOR LIFTING WITH BACKHOE, THE IDLER TILTED TO THE GROUND STRIK ING EE IN LEFT LEG.
EE WAS CHANGING A TIRE AND WHEN HE WAS LOWER THE LOCK RING TO THE GROUND IT PINCHED HIS LEFT INDEX IFNGER IN LOCK RING GAP HAD TO HAVE SUTURES.
WAS REPAIRING COAL TRUCK TIRE, IMPROPERLY INSTALLED INNER LOCK RING, STARTED TO AIR UP TIRE, CAUSING THE LOCK RING TO SLIP AND HIT THE AND OF AIR HOSE. THUS SENT IT AIR BORNE TO WHERE HE WAS S TANDING.
EE WAS ATTEMPTING TO OPEN THE DOOR ON POWDER TRKHE REACHED DOWN WITH HIS LEFT HAND FOR THE DOORHANDLE BUT DIDNT PULL HARD ENOUGH TO OPEN THE DOOR. HE THEN REACHED WITH HIS RIGHT HAND FOR THE T OTTOM CORNER OF DOOR & PULLED AS HE HELD THE HANDLE WITH HIS LEFT HAND & WHEN HE PULLED HIS RIGHT HAND WAS VERY NEAR EXHAUST GUARD HIS HAND STRUCK THE GUARD CAUSING A LACERATION
EMPLOYEE WAS UNLOADING A ROLL OF HOSE WHEN THE PLASTIC STRAP BROKE. EMPLOYEE FELL BACK AND HIT HIS LEFT ELBOW/ARM ON THE SIDE BOARD OF TRUCK.
EMPLOYEE HAD LIFTED A PUMP TO INSTALL ON A 992G LOADER ON 8-5-00. HE WAS HAVING CHEST PAINS ON 8-10-00 AND WENT TO HOSPITAL. HE WAS TOLD THAT HE HAD STRAINED LIFTING THE PUMP AND CAUSED A PI N HOLE IN OUTER WALL OF LUNG WHICH CAUSED THE CHEST PAINS.
