Struck by falling object

While changing a dozer blade on a D10 dozer, the tilt jack was suspended by a hoist. Employee laid his hand between a rock guard on the push arm and tilt jack. As the push arm moved forward the cable on the hoist slipped and the tilt jack fell catching his fingers between tilt jack and rock guard.

EE was cutting a tree. When the tree fell, it hit another fallen tree that was forked. The forked tee hit EE, causing him to fall down a steep hill. EE was examined by an EMT, then sent to hospital.

He was coming down a hill in a coal truck. He lost control of the truck and turned it over.

EE had a plastic cup holder lying on the dashboard of his drill. He was using a utility knife to cut it down. As he was putting pressure on the knife, the plastic broke causing the knife to continue downward and strike him in the leg.

Employee was prying on a chute with a pry bar. The pry bar slipped and employee smashed his right thumb against corner of chute.

While attempting to carry a crib block under a rock truck, employee fell on the crib block while still holding it causing a fracture to small bones in his hand.

Ee was on a haul road berm, picking up trash. As he descended down the berm, he was walking sideways to keep from sliding. When he made his last step off the base of the berm, his ankle turned causing a hair line fracture to the fibula bone just above the ankle.

While attempting to climb down the ladder of a drill, EE stepped off the bottom step onto uneven ground and turned his ankle causing a fracture. Restricted work duty no lta.

EMPLOYEES EXCAVATOR HAD BROKEN DOWN. WHILE ASSISTING A MECHANIC HE CLIMBED UP ON A DOZER BLADE TO RETRIEVE A TOOL. WHEN DESCENDING FROM THE BLADE HE TWISTED HIS KNEES. CONTINUED TO WORK THEN W ENT TO DR ON 12-4-01. MRI COMPLETED, TORN CARTILAGE. SCHEDULED FOR KNEE SURGERY ON 12-3-01.

EMPLOYEE HAD LIFTED A PUMP TO INSTALL ON A 992G LOADER ON 8-5-00. HE WAS HAVING CHEST PAINS ON 8-10-00 AND WENT TO HOSPITAL. HE WAS TOLD THAT HE HAD STRAINED LIFTING THE PUMP AND CAUSED A PI N HOLE IN OUTER WALL OF LUNG WHICH CAUSED THE CHEST PAINS.
