Employee and 3 other men were picking up and pulling the miner electrical cable. Employee states "I bent over and it felt like a knife went into my back". Employee went down to his hands and knees; and complained of severe lower back pain. He was taken to the hospital ER and was discharged with a BACK STRAIN.

Two men were putting the rub rail on the continuous miner. The rub rail was dropped and struck this man's right foot. The foot was fractured, cut, bruised, and swollen. The cut required 10 stitches.

The Section Foreman, and 3 men were cleaning the conveyor belt rollers and structure. The belt was started and ee right hand and lower arm was mashed between the belt and a metal top roller. ee suffered a crushing injury to his lower right arm; no broken bones; bruising and swelling. An MRI was done on 3-11-14 to determine any damage to tendons and ligaments.

EE and co-worker were working on feeder jack. Co-worker threw a crib block toward the feeder; the crib block struck EE on his upper left leg causing him to fall onto the jack. EE had pain and bruising to left leg and left side. Transported to hospital for exam, x-rays, etc. No fractures. Discharged to return to full duty based on mobility on left leg.

05/07/2012-2:00pm EE reported that he had hurt his back at 7:30am when he removed a rock from top of miner. He complained of pain in his lower back on right side; and he wanted to go to the doctor. Appointment was set for 05/09. After his appointment, he stated that he might have "pulled a muscle" and needed an MRI. Per Doctor, he returned on light duty on 05/11/2012.

Employee went to get tools out of a metal tool box near the section. He propped the lid open with a bar. He hit the bar and the lid fell onto his right hand. No broke bones; no cuts; Rt hand sore and swollen.

EE and two others were moving motors in the hallway of the shop/office building. As he was helping place one motor beside another, the tip of his index finger, left hand, got mashed and cut. Stitches were required.

EE and other ee were replacing elevator buckets on the prep plant. As he let off the comealongs, a metal elevator bucket slipped-knocking him against a metal angle brace. The metal angle brace struck his right shoulder and right side.

Employee and two others were on surface area of UG mine, loading cribs, boards and wedges into a scoop bucket. The scoop bucket was on a crib instead of the ground. When almost fully loaded the scoop bucket fell off the crib and the operator side edge of the bucket struck the top right foot of employee.

He was walking with a 35lb can of oil in each hand toward the face equipment. He slipped, causing his back to twist and start hurting. He completed this shift and worked a full shift the next night. He went to the Dr. on Monday, 5/8/2006, because of continued pain in his lower back.

THREE EMPLOYEES WERE INSTALLING A 25 HP ELECTRIC MOTOR ON THE PREP PLANT. THE MOTOR WAS BEING POSITIONED SO THAT IT COULD BE BOLTED DOWN. THIS EMPLOYEE HAD HIS RIGHT ARM BETWEEN THE MOTOR AND THE SHAFT. THE MOTOR STRUCK HIS RIGHT ARM BELOW THE ELBOW. CONTUSION, BRUISING AND SOME SWELLING RESULTED.
