Over-exertion in lifting objects

Two employees were lifting a welding cart onto a ride when employee felt pain in back and left knee

Struck by falling object

Employee was making first cut inby the intersection of #1 entry, oo3 mining section. Because of wet conditions EE was sitting on some loose material at the corner of #1 when a piece of rock measuring aprox. 30 inches by 30 inches by 2 inches thick fell from between the bolt and the rib striking the employee on the head.

Accident type, without injuries

Miners heard a bump in roof and the section foreman pulled all persons on the section outby. At 10:15 pm a roof fall occurred above the bolt anchorage on the 003 section, 2 northwest spad 15+00 in the crosscut between #6 and #7 entries.

EMPLOYEE USING SLATE BAR TO PRY OPEN RUB RAIL ON SHUTTLE CAR...RAIL BROKE LOOSE SUDDENLY CAUSING BAR TO HIT EMPLOYEE IN MOUTH.

Employee was running the weed eater adjacent to the parking lot when EE's leg slipped causing EE's knee to buckle.

Fall onto or against objects

Employee was pushing against a pry bar to pry cat tracks off of planetary when the bar slipped causing the employee to fall forward striking the continuous mining machine with their shoulder. The equipment was locked and tagged out and blocked against motion.

While loading highline cable at the #5 head drive, a loop of cable flipped over and hit the employee on the right knee above the kneecap and hyperextended EE's knee.

While performing a belt move the employee was driving a pin through the belt rail with a hammer striking EE's left index finger/knuckle. The employee was wearing metacarpal gloves at the time of the incident.

EMPLOYEE WAS CHANGING TIRE ON TWO PERSON BUGGY; BENT OVER TO PICK UP TIRE AND FELT STABBING PAIN AND HEARD POP IN LOWER BACK.

EMPLOYEE WAS USING A HAMMER TO BACK A PIN OUT DURING PROCESS OF CHANGING JACK ON SHUTTLE CAR WHEN PAIN WAS FELT; BEGAN IN LOWER BACK AND RADIATED DOWN LEFT BUTTOCK AND LEFT THIGH. THEY CONTINUED TO WORK FOR WEEKS UNTIL THE PAIN BEGAN TO LIMIT THEIR MOBILITY.

Flash burns (electric)

Employee was helping the ELECTRICIAN splice #1 car cable, proceeded to put cathead in, flipped the breaker and states: saw a flash and does not remember anything after that.

Struck by powered moving object

Foreman was struck by shuttle car when shuttle car left loaded, hitting offside rib causing shuttle car to shift suddenly and striking foreman. Foreman sustained injury to left wrist resulting in fracture of left distal radius.

Employee was bolting in the #5 entry were they swing out to put up outside bolt when a piece of rock hit them in the back.

Struck against stationary object

Miner was crimping water line with banding tool. After completeing the job, EE then pulled the excess band out of the banding tool and the sharp edge cut the thumb and finger on left hand.

Struck by falling object

EE was swinging Drill Boom on Fletcher Bolter to install the inside bolt after installing outside bolt. Piece of draw rock struck EE on the back of the right foot while crawling with boom swing.

Employee was making repair to the miner when employee slipped and fell injuring left leg when hitting the stand off of the miner

Operator was moving from one heading to another and placed hand on safety post outside. The operator when turning the cable chain came in contact with safety post pinching left pinky finger.

Over-exertion in lifting objects

Employee and 3 other men were picking up and pulling the miner electrical cable. Employee states ""I bent over and it felt like a knife went into my back"". Employee went down to his hands and knees; and complained of severe lower back pain. He was taken to the hospital ER and was discharged with a BACK STRAIN.

Struck by falling object

Two men were putting the rub rail on the continuous miner. The rub rail was dropped and struck this man's right foot. The foot was fractured, cut, bruised, and swollen. The cut required 10 stitches.

4 men were working in pairs of 2, driving pins out of the ripper chain on the miner. This man was holding the bar while the other was hitting with the sledge hammer. This man got hit in the forehead. His forehead was bruised, swollen, and cut. The cut required 7 stitches.

Caught in, under or between a moving and a stationary object

The Section Foreman, and 3 men were cleaning the conveyor belt rollers and structure. The belt was started and ee right hand and lower arm was mashed between the belt and a metal top roller. ee suffered a crushing injury to his lower right arm; no broken bones; bruising and swelling. An MRI was done on 3-11-14 to determine any damage to tendons and ligaments.

Struck by flying object

EE and co-worker were working on feeder jack. Co-worker threw a crib block toward the feeder; the crib block struck EE on his upper left leg causing him to fall onto the jack. EE had pain and bruising to left leg and left side. Transported to hospital for exam, x-rays, etc. No fractures. Discharged to return to full duty based on mobility on left leg.

Miner operator trammed miner into entry for first cut. He was sitting beside miner when a rock fell from between roof bolts; and struck the top and left side of his hard hat; causing an injury to his neck. His hard hat caused small scrap and swelling to the left side of his face between eye and ear. Seen at hospital and discharged day of injury.

EE was roof bolting. The glove on his left hand got caught in the chuck/drill pot. This cut his little finger (7 stitches) and fractured three fingers (little, ring, middle) on his left hand.

Over-exertion in lifting objects

05/07/2012-2:00pm EE reported that he had hurt his back at 7:30am when he removed a rock from top of miner. He complained of pain in his lower back on right side; and he wanted to go to the doctor. Appointment was set for 05/09. After his appointment, he stated that he might have ""pulled a muscle"" and needed an MRI. Per Doctor, he returned on light duty on 05/11/2012.

Struck by flying object

EE(left handed)was using a 4lb hammer with both hands to drive a dump shaft onto mobile bridge(#3).The hammer glanced off shaft and hit sprocket; and a small piece of metal from sprocket flew into his right hand causing injury. The piece of metal entered between middle knuckles and embedded itself in top of his right hand. Puncture wound, laceration, & cut to vein; top of hand.

Struck by falling object

EE had a set of come a longs hooked to a roofbolt plate. The plate broke causing the come a longs to fall and strike him in the side of his head above his right ear. This caused an egg sized knot and cut that required stitches. No fractures or other injury.

Employee went to get tools out of a metal tool box near the section. He propped the lid open with a bar. He hit the bar and the lid fell onto his right hand. No broke bones; no cuts; Rt hand sore and swollen.

EE was putting last row of bolts (entry #5). He was working the levers with his right hand. He had his left hand on the drill pot. His middle finger on his left hand got pinched/cut. The tip of the finger - required 10 stitches and bone broken.
