The employee injured EE's back while dismounting a 992G loader. The injury appeared to be a muscle strain on EE's left side. EE refused initial medical attention but later visited EE's primary care physician when the soreness didn't subside. EE was treated and given a full release.

The employee was injured while observing the roll out of track for a Caterpillar D10R dozer. As the track unfolded a thin 1"x1" piece of metal broke loose from the track rail and struck the employee in the groin area. The employee was taken to the hospital for an examination and was placed on light duty.

The employee was working on realigning the tail pulley on a conveyor belt and twisted knee during the work process. The employee stated ee wasn't for sure if the accident occurred when ee got out of the truck or during the realignment process.

The employee was removing an oil filter. As the employee applied pressure to the oil filter in a twisting motion the oil filter broke loose causing pain in the employee's right shoulder. On July 12th, 2018 the employee was diagnosed with a torn rotator cuff and placed on light duty.

There was no accident, the employee was suffering from a bulging disc in which employee had previously had surgery for years ago. The condition was worsening and while working on the job that day employee was operating a 777D rock truck. As the employee stood up to exit the truck employee's back was burning and cramping.

Employee removed a hydraulic hose and got hydraulic fluid on EE's safety glasses and in EE's eye. After EE's eyes were flushed and cleaned, EE had blurred vision and was taken to an optometrist to be checked out. Blurred vision lasted for 2 days.

Employee was attempting reposition a ceramic shoot for reassembly of the lid covers. The employee kicked the chute causing damage to the metal clamp that secured the shoot onto the piping that ran from over head. This allowed the chute to fall striking the employee on the right leg. Employee was examined at the local hospital and placed on light duty.

EE WAS LOADING THE EXPLOSIVES TRUCK. WHEN HE RAISED THE HANDLE OF THE POWDER BIN (WHICH ALLOWS AMONIUM NITRATE TO FLOW INTO THE TRUCK) HE FELT ASHARP PAIN IN HIS LOWER BACK. HE DROVE HIMSELF T O THE HAZARD ARH WHERE HE WAS TREATED FOR A PULLED MUSCLE IN HIS BACK AND RELEASED.
