The employee suffered a deep laceration on the left side of the top of EE's head resulting in 7 stitches.

Slipped on wet steps resulting in a compression fracture to the back

Employee suffered from a 3/4 of an inch laceration on EE's left hand.

Employee was attempting reposition a ceramic shoot for reassembly of the lid covers. The employee kicked the chute causing damage to the metal clamp that secured the shoot onto the piping that ran from over head. This allowed the chute to fall striking the employee on the right leg. Employee was examined at the local hospital and placed on light duty.

Employee has filed a cumulative trauma claim with the Kentucky Labor Cabinet. ***THERE ARE NO ACCIDENTS ASSOCIATED WITH THIS CLAIM***

Coal dust / Black lung.

Employee was operating loader on haulroad to spread gravel when he drove over a rough patch in the road making the loader bounce. When this happened, the employee felt a sharp pain in his upper back area.

Employee was prying a slab of slate to get it to fall thru breaker when it moved, kicking the pry bar causing it to pinch his finger on his left hand against the edge of the cover resulting in a cut requiring stitches.

Employee was replacing metal siding. Metal fell and cut index and middle finger on left hand.

Employee was using a hammer to remove tile, EE failed to observe a piece of metal sticking out & caught the small finger on his left hand between metal & the handle of the hammer.

EE was adding oil to 740 CAT truck, looked to see sight glass, foot slipped, causing EE to fall.

Employee was removing a metal shipping band from a pallet of screens, the band snapped and struck employee on right fore finger resulting in a laceration.

Employee picked up drive shaft, the shaft slipped catching left ring finger under drive shaft.

EMPLOYEE WAS ON FOURTH FLOOR OF THE PREP PLANT AND COLLAPSED.

Employee was lifting a piece of metal and felt a pain in his lower back. Started missing work 3-8-2005.

Employee was lifting a wearplate. The wearplate slipped catching his middle finger of right hand.

Employee was working on discharge line in plant on cyclone pump. The next section of pipe was in place when employee hit the pipe with hammer and the come-a-long rolled around catching his left little finger.

Employee was changing top roller on feed belt and the roller turn over causing a jagged edge to cut forearm. (6 stitches)

LOADER WAS HOLDING TRUCK BED WHEN IT SLIPPED CATCHING THUMB BETWEEN TRUCK BED AND WINDOW BLOCK.

STRAINED BACK WHILE REMOVING BOLTS FROM LOADER ARM.

EE WAS HELPING PUT IN BOTTOM IDLER, PICKED UP BELT AND WAS HOLDING IDLER UP WITH PIPE WHEN HE FELT A BURNING IN STOMACH, THEN A KNOTT POPPED OUT AROUND HIS NAVEL.

EMP WAS CUTTING OLD HINGES OFF ACCESS DOOR. HEAD CHUTE, FIRE AND SLAG SPLATTERED BACK GOING UNDER EMPLOYEE'S GOGGLES.

EE WAS MOVING A PC. OF PLYWOOD WHEN THE WIND CAUGHT IT, TWISTING THE EE'S RT. SHOULDER.

EMPLOYEE WAS FUELING GRADER WHEN HE SLIPPED AND FELL FROM GRADER AND BROKE HIS LEFT ARM.

EMPLOYEE WAS MIXING FLOCK WHEN WIND BLEW THE FLOCK IN HIS EYE.
