On 4/18/06 employee was loading holes with the prill truck when the boom swung out and employee twisted using his arm to block the boom. He twisted his lower back and felt pain in groin area. Employee RTW 4/19/06. Employee sought medical care on 5/8/06 and now off work due to scheduled hernia surgery on 5/22/06.

Employee states that he thinks original injury occurred on 6-1-05. He didn't report it until 10-14-05 when he felt a sharp in lower left groin area. He went to doctor and was diagnosed with a hernia. Surgery to repair is scheduled for 11-4-05.

EE was installing a blade on a Caterpillar 16-G Grader. The chain holding the blade upright slipped off and the blade fell forward. The top edge of the blade struck his left foot just behind the steel toe. He incurred a deep laceration to the top of his foot and fractured three metatarsus bones (big toe and the next two). He is still off from work.

Victim states that he initially hurt his back on 9/16/04 but never reported it to anyone. On 9/21/04 he aggravated the injury while performing repairs on a truck. Reported to supervisors and went to doctor this date. Strained muscle in lower back.

Victim struck his head on a cross member of the Anfo Bin while preparing to load a truck with Anfo. Six staples to close the wound. Employee was a summer employee and scheduled for 08/20/2004 to be his last day of work before returning to school. He just asked to be terminated one day prior to schedule. No lost time involved and voluntary termination.

Victim removed the radiator cap from a D-9 dozer while engine was still hot. The hot anti-freeze blew out and hit him on the forearms, chest and stomach resulting in 1st and 2nd degree burns.

Employee was installing new corner bits bolts in dozer blade. He had all old bolts removed and put a bar in bolt hole to hold bit in place. The bar slipped and the corner bit fell striking his left foot at the base of his toes. Three toes were surgically removed, the big toe and the next two.

While dozer was being serviced by greasers, the mechanic began doing maintenance on the track rollers. The fuel cap rolled off the machine and hit him in the head. Required fifteen (15) clips/staples to close.
