Employee was helping to lift a pipe in valley fill, causing muscles in his back to be pulled and strained.
Bent over picking up crib block & injured lower back. Did not go to Dr. until 5/3/07. Then he was off work from 5/3/07 to 5/14/07.
Employee was repairing a drill, picked up a solenoid resulting in pulling his back.

Employee was changing out a belt roller, while pulling roller out of position the weight of handling belt dropped down putting the roller in a position which pinched the employee's left middle fingertip between the roller and structure, resulting in stitches.
Removing and installing segment on D11 Dozer. EE did not miss any work with this injury at first. His back was still bothering him so he went to Dr. on 4/20/06. & Dr. took him off work. EE is currently off work due to injury on 3/10/06.

Mechanic was moving the jack pad on the drill when he started feeling pain in his lower back. Pulled muscle in lower back. Missed work from 9/6 thru 9/10/2005.
Employee states that he thinks original injury occurred on 6-1-05. He didn't report it until 10-14-05 when he felt a sharp in lower left groin area. He went to doctor and was diagnosed with a hernia. Surgery to repair is scheduled for 11-4-05.
Victim picked up a drill bit for highwall drill and caused an existing hernia to be reinjured. Missed one days work.
EE was tugging on the tire on a mechanics truck trying to remove it from the truck. He felt a pain in his lower abdomen/groin area.He is still off work due to hernia.
EE was installing a blade on a Caterpillar 16-G Grader. The chain holding the blade upright slipped off and the blade fell forward. The top edge of the blade struck his left foot just behind the steel toe. He incurred a deep laceration to the top of his foot and fractured three metatarsus bones (big toe and the next two). He is still off from work.
EE says he rolled a small rock off a push arm on a dozer. He then proceeded to another piece of equipment and his back started hurting.
EE was lying on the ground on his back installing cutting edges on the blade of a 16-G grader when he says he had a sharp pain in his lower back going down his right leg.
Victim states that he initially hurt his back on 9/16/04 but never reported it to anyone. On 9/21/04 he aggravated the injury while performing repairs on a truck. Reported to supervisors and went to doctor this date. Strained muscle in lower back.
Victim was hooking up a power connection (cathead) on a highwall miner and lacerated the middle finger on his right hand. Six stitches to close the wound.
Victim struck his head on a cross member of the Anfo Bin while preparing to load a truck with Anfo. Six staples to close the wound. Employee was a summer employee and scheduled for 08/20/2004 to be his last day of work before returning to school. He just asked to be terminated one day prior to schedule. No lost time involved and voluntary termination.
Victim removed the radiator cap from a D-9 dozer while engine was still hot. The hot anti-freeze blew out and hit him on the forearms, chest and stomach resulting in 1st and 2nd degree burns.
Victim was working on a dozer when he slipped & hurt his back. He is being treated for a herniated disk in his back. He continued to work through June 21, 2004 and began missing work on June 22, 2004 and is still off work.

While dozer was being serviced by greasers, the mechanic began doing maintenance on the track rollers. The fuel cap rolled off the machine and hit him in the head. Required fifteen (15) clips/staples to close.
EMPLOYEE WAS PULLING ELECTRICAL WIRE WHEN HIS HAND SLIPPED AND LACERATED INDEX FINGER ON RIGHT HAND. FIVE STITCHES TO CLOSE.

EE WAS REMOVING PN FROM TOOTH ON BUCKET, WHEN A PIECE OF METAL CHIPPED OFF & STRUCK HIM IN THE CHEEK ON THE RIGHT SIDE OF HIS FACE. WAS REMOVED BY PHYSICIAN AT E.R.
EMPLOYEE STEPPED OFF THE BUMPER OF MECHANIC'S TRUCK AND TWISTED HIS ANKLE.
EMPLOYEE WAS UNLOADING WET HOLE OF EXPLOSIVES WHEN HE DEVELOPED A HERNIA, SCHEDULED FOR SURGERY.
