Employee was using a pry bar on the water valve of the water truck, when the bar slipped puncturing the left hand. Employee went to Emergency Room and received 4 stitches and a tetnus shot.

Fall to the walkway or working surface

Employee was attempting to help free vehicles stuck on road when he slipped on ice and landed on his right hip. Accident occured on the county road not on mine property.

Employee was checking oil levels on grader when employee was dismounting the machine, one foot hit ice and employee fell to the ground. Employee bruised his shoulder blades and left hip.

Employee was on top of water truck to check and see if tank was full, when he slipped and fell on top of the water truck.

Over-exertion in lifting objects

Mechanic was moving the jack pad on the drill when he started feeling pain in his lower back. Pulled muscle in lower back. Missed work from 9/6 thru 9/10/2005.

Fall from machine

Operator was dismounting truck, the bottom cable step slipped and caused his left foot to strike the ground very hard. It had been raining & there was a lot of mud. His left knee was dislocated and will require surgery.

EE and another mechanic were performing repair work on a drill. He used a hammer to loosen parts on the final drive. Impact from hammer caused a splinter of metal to chip off and imbed in his left wrist area. Doctors determined that it would be best to leave splinter where it is. Missed two days work

Employee says that he was ?jarred? in the rock truck while being loaded. Two days rest.

Victim was dismounting the loader when he stepped on the bottom step that had gotten broken during his shift and he fell to the ground. Complained of pain in lower back, diagnosed with swollen disc.

EE was dismounting the truck when he slipped on the bottom step and fell, bruising his left hip. Missed two days work.

EE was climbing onto the truck to start his shift. He had a lunch bucket and another item in his hands. He slipped and fell approximately ten feet to the ground. His left leg was broken just below the left knee. Required surgery to insert pins and plates to stabilize the bones.

EE was tugging on the tire on a mechanics truck trying to remove it from the truck. He felt a pain in his lower abdomen/groin area.He is still off work due to hernia.

EE was climbing out of the loader to clean the windows when he slipped and fell from the loader to the ground. He is still off work due to pain and stiffness in his neck.

Employee was operating a front end loader, loading gravel into to be spread on the haul road. He says that somehow he shifted or twisted in the seat and pulled his left shoulder out of socket.

EE states that twice on his shift a large rock shook the truck he was operating when the loader was putting shot overburden in the bed. His back began hurting, however he continued working until 1/18/2005 before missing the first day of work. He was off until 1/24 and returned to work. After five hours his back was hurting so bad he took off again. Still off from work.

Struck against stationary object

Employee was helping out on the powder crew when he stepped on rock and twisted his right leg. Swelling above the ankle in the calf area of right leg. Unknown extents of injury have not been able to contact him.

Victim states that he initially hurt his back on 9/16/04 but never reported it to anyone. On 9/21/04 he aggravated the injury while performing repairs on a truck. Reported to supervisors and went to doctor this date. Strained muscle in lower back.

Struck by rolling or sliding object

Employee cut a tree when a grapevine dislodged a rock approx 75 ft uphill from the tree. The rock rolled down the hill striking employee on the knee.

Operator parked his truck too close to the highwall. A rock fell striking the right rear of the truck. Operator sustained a strained muscle in his right arm.

Victim suffered a stroke on the job. The next day he had a fatal stroke while still in the hospital. THIS IS NOT A MINING ACCIDENT OR OCCUPATIONAL ILLNESS. THIS REPORT IS BEING FILED UNDER PROTEST AS REQUIRED BY MINE CITATION 7229029.

Operator twisted around in the seat to look behind him and felt a pain in his lower back radiating all the way down his left leg.

Absorption of radiations, caustics, toxic and noxious substances

Battery was making noise, so he raised the compartment cover and the battery exploded. Got battery acid on left hand. ER physician applied 6 stitches.

EE CAUGHT FINGER IN BELT ON AIR COMPRESSOR WHILE ATTEMPTING TO START AIR COMPRESSOR ON MECHANICS TRUCK.

A ROCK WEDGED IN THE FEED BELT OF CRUSHER. AFTER SHUTDOWN OF CRUSHER, EE MOVED ROCK BY HAND, RESULTING IN A BACK STRAIN AND PAIN IN THE GROIN AREA.

WHILE WORKING EXCAVATOR IN ROCK SHOT NEAR OUTER EDGE SHOT ROCK COLLAPSED FROM UNDER RIGHT TRACK CAUSING EXCAVATOR TO OVER TURN. THIS CAUSED DAMAGE TO CAB PINNING OPERATOR INSIDE BY BOTH LEGS.

EMPLOYEE REMOVING A LUMP OF COAL FROM STACKER BELT ON HYDROSCREEN AND A PIECE OF COAL FELL FROM BELT ONTO HIS HAND.

HOSE ON TRIPLE SEVEN CAUGHT ON FIRE BECAUSE OF PRESSURE. EE DID NOT FOLLOW SAFETY AND JUMPED OFF THE TRUCK.

EE USING BOOM TO LIFT WATER PIPE. THE PIPE BEGAN SWINGING. EE LIFTED ARM TO STOP PIPE FROM HITTING ANOTHER PIECE OF EQUIP.

DAY WEATHER CONDITIONS, EE ALLEGES WAS CLIMBING DOWN OFF EQUIPMENT SLIPPED OFF STEP AND LANDED ON NECK SHOULDER. THERE WAS MD WITNESSES. HE LEFT AT 10 30 PM TO GO TO HOSPITAL.

MECHANIC WAS WALKING AROUND CAB & STEPPED ON A PLASTIC PIECE & FELL STRAIGHT DOWN. TRYED TO CATCH SELF & SMACKED ARM AGAINST TRUCK TIRE.

WHILE DISMOUNTING FRONT ENDLOADER EE SLIPPED ON BOTTOM STEP AND FELL ON HIS BACK AND NECK. IT WAS RAINING MIXED WITH SNOW.
