The employee was using a sledgehammer on a wedge under the liner at the Raw Mill. An estimated 1.5cm piece of the inner shattered and went into the throat.

The electrician arrived early for EE's shift. EE arrived at 6:40am and EE's shift starts at 7am. At 6:45am, EE was opening a package of brand new tools EE purchased over the weekend. EE took out EE's utility knife to cut the plastic wire tie and the knife slipped and sliced open EE's left index finger.

While removing a socket liner from a raw mill using a hammer and pin to knock a bolt out, the EE that was helping was hit with a metal fragment and it entered EE's eye. The employee was wearing safety glasses at the time.

While aligning liner up against frame, the choker slipped down a little causing the liner to slip down smashing the tip of EE's left ring finger.

Mechanic was cutting a conveyor belt using a razor knife when the blade slipped and cut left wrist. Bleeding was severe and mechanic was transported to emergency department where they received three sutures and a tetanus shot. Employee placed on light duty.

Mechanic was working in Raw Mill tapping out bolt holes. While doing this EE felt a pain in the upper groin area. EE did not go to the doctor at first--thought it might go away. On 2/1/21 EE saw a doctor that said EE has a strain in EE's upper groin area. Dr. prescribed antibiotics and told EE to return to work with weight restrictions.

Mechanic replacing chain on primary crusher and while swinging sledge to join connecting link plate hammer hit pad eye causing hammer handle to impact hand caused bruising. Mechanic did not report injury until 01/19/21. After self-care did not help, placed on restrictions 01/25/21 resulting in medical incident.

Mechanic using pipe wrench to loosen blowpipe on dust collector and overextended EE's elbow. Incident was not reported until 01/08/2021. Employee instructed to have medical evaluation on 01/08/2021 and medical provider placed employee of work restrictions.

Employee reports that they were trying to work a rock loose on a conveyor using a metal rod when the rock gave way and right hand came down onto the metal guard hitting right pinky finger. Employee reports that they noticed pain, bleeding and deformity of the finger.

Maintenance mechanic was working on line 1 coal mill and was utilizing a come-a-long and other mechanical tools. Mechanic reported soreness in right elbow. Elbow pain continued to worsen until medical treatment was advised on 08/28/2020, at which time medical professional diagnosed lateral epicondylitis (tennis elbow. Employee was given tennis elbow brace by the doctor.

El empleado hacia labores de limpieza a traves de un escotilla de la tolva con espacio limitado el material bajo de forma inadvertida y golpeó la herramienta que a su vez golpió al empleado. EE was cleaning a hopper through a hatch with limited space. Material shifted and hit the tool which in turn hit the EE.

Mechanic trying to remove coupling hub from head shaft of limestone feeder. Using a strong back with a port-a-power ram and 1/2" all-thread rods to rig coupling for removal. While applying pressure to the ram, a rod broke & strong back swung around & hit mechanic on left hand. Treated as first aid but did not improve, instructed to go to doctor & x-ray found broken bone.

On 1/26/18, employee informs of a severe chest pain. We proceed to transport the employee for medical treatment. On 1/29/18, the employee proceeds to visit EE's personal MD, who recommended work rest. On 2/1/18, employee go to the Emerg room do to continue chest pain, where EE was evaluated and diagnostic muscular sprain work related and was referred to workers compensation.

The mechanic was removing a pipe nipple with a 36" pipe wrench from the down comer duct and reported severe pain in abdomen. EE was sent to the doctors and put on light duty until further evaluation could be completed.

A Production Attendant was cleaning off build up on the tip of the burner pipe when a chunk of material fell on the lance causing it to slip from attendant's hands. In trying to maintain grip of the lance, they felt a pain in the lower back.

A sledge hammer was used to remove a sprocket segment from a bucket elevator. When the sledge hammer hit the sprocket, metal came off and hit an employee about 15-20 feet away in the abdomen. The employee was taken to the ER to receive an X-Ray to verify no metal remained in the laceration, a tetanus shot, and two stitches before being returned to work.

While replacing inboard bearing assembly on VRM KF pump, mechanic used 1/2 in pull handle to try to remove 3 bolts. Mechanic felt discomfort in rt shoulder, but finished EE's shift without reporting any concerns. Overnight EE felt increased pain in shoulder and reported incident to EE's supervisor the next day. On 11/22 EE was placed on restricted duty after a follow-up exam

A production employee strained his back while swinging a sledge hammer in effort to unplug a chute. This incident was initially treated as a first aid. The employee later visited a doctor which prescribed medicine for pain and modified duty making the incident reportable.

Employee was removing 1st row of mill shell liners from compartment 1 on finish mill 3. Bull-pin & hammer was used to remove wedged mill balls found between liners & head liners. When striking bull-pin with the hammer the lodged mill ball came loose causing the bull-pin to ricochet hitting him in the left knee. Completed shift & once home had pain in knee. Went to doctor 11/25.

A maintenance mechanic was welding and stopped to chip at a piece of shim plate and when a piece of the metal flew back at him striking him in the arm. The incident was initially treated with first aid measures. The employee recently sought medical treatment due to possible infection.

EE WAS WORKING ON THE FINISH MILL FEED CONVEYOR REMOVING CLINKER BUILD UP FROM UNDER THE BELT. HE WAS PRYING A PIECE OF CLINKER WITH A SHOVEL AND ANOTHER EMPLOYEE WAS ON THE OTHER SIDE DOING THE SAME THING WHEN HIS SHOVEL SLIPPED HITTING EE IN THE LEFT KNEE.
