The Admiral Security Guard tripped in a dip in the road while moving a cone for a truck driver to exit the plant.

Employee was using a 3 lb hammer to form metal after welding it. Employee missed target with hammer and felt strain on elbow. Placed on work restriction, no lifting >5 # for next 7 days.

Employee state that they were busting concrete bags and some of the bags had rocks in them and a finger got caught in between two rocks and it caused a laceration.

The employee was operating the locomotive when the locomotive derailed through a switch. Employee jumped off the locomotive and injured right wrist upon landing on the ground.

At approximately 10:30 a vendor installing a new HVAC unit started experiencing chest pains and felt bad. Asked co-worker to call 911 as a precaution.

The employee was using a sledgehammer on a wedge under the liner at the Raw Mill. An estimated 1.5cm piece of the inner shattered and went into the throat.

Employee went to investigate a dusting situation at the silo discharge. EE climbed the ladder to the upper platform and found spillage on the platform. Upon trying to descend the ladder EE lost EE's footing and fell to the lower level.

The employee strained back while moving a rotary air lock into position for installation.

Truck driver was picking up a load of bagged cement at the shipping department. EE was told several times to remain in EE's truck. EE exited the truck and walked into the side of a fork truck carrying a pallet of cement. EE struck the fork truck behind the load at the front right tire with EE's foot.

Maintenance mechanic was working at VRM roll 1 when a disruption in air flow caused the VRM to become positive, and dust escaped from an inspection door. The mechanic was exposed to hot dust, and was removed from the area. Upon evaluation, left wrist received a minor burn and mechanic was placed on restricted work duty.

While clearing a plug on the Stage 1-West vessel material flushed out of the vessel opening onto employees foot, causing a burn to EE's right foot.

Walking in rough terrain to access a settling pond

Employees were removing trees with an excavator and a sling. The sling was wrapped around the trees to pull them out with the excavator. When one of the trees was being pulled out, the top portion snapped off. An employee on the ground ran to avoid the falling debris. However, the falling debris brushed across their left shoulder and back, injuring the left shoulder.

Based on initial investigation, appears unknown hot material or object was inside alt fuel storage trailer. As material unloaded from trailer material/object exposed to oxygen causing spontaneous combustion. Dry chem initially extinguished the fire, but quickly reignited and spread. Local FD came on site to fully extinguish.

Electrician working on trouble shooting the limit switch on a turning pod sending cement to the Finish Mill 3 cement cooler or FK pump. Electrician's arm was caught by the mechanism that moves the turning pod and fractured the radius bone.

The electrician arrived early for EE's shift. EE arrived at 6:40am and EE's shift starts at 7am. At 6:45am, EE was opening a package of brand new tools EE purchased over the weekend. EE took out EE's utility knife to cut the plastic wire tie and the knife slipped and sliced open EE's left index finger.

While lowering a steel cover from a roller, mechanic pinch EE's right middle finger between steel plates.

Employee welding on a silo. Leaned down and bumped a pipe with their face. Lip Contusion.

While removing a socket liner from a raw mill using a hammer and pin to knock a bolt out, the EE that was helping was hit with a metal fragment and it entered EE's eye. The employee was wearing safety glasses at the time.

Mechanic was assisting welding when a piece of slag got into their eye.

While aligning liner up against frame, the choker slipped down a little causing the liner to slip down smashing the tip of EE's left ring finger.

Employee fell off of a skid steer at bucket height on ground while getting the hours of operation from the meter.

An employee was assisting with the installation of a guard. The employee pushed against the guard to help get it into place. When EE did this, EE heard something pop in EE's right shoulder.

EMPLOYEE WAS PREPARING A MIX MATERIAL TO POUR ON THE LILN NOSE RING, THE MIXER LOCKED UP AND HAD TO DUMP MATERIAL OUT, THEY PLACED MATERIAL ON PALLET AND WAS USING A FORKLIFT TO MOVE THE MATERIAL THE PALLET WAS NOT LEVEL, EMPLOYEE PUT PRESSURE ON ONE SIDE OF PALLET AND LEFT FOOT WAS CAUGHT BETWEEN PALLET AND FORKLIFT RACK

While placing the jib into its travel position, the employee was folding up the jib ramp and EE's finger was caught in a pinch point causing a laceration.

Two employees were installing a metal cover on top of a silo. While repositioning the cover, one of the employees pulled up on handle forcibly causing the handle on the opposite side to go down rapidly pinching the end of the employee's middle finger between the cover's handle and the silo roof. This caused a laceration to the finger and the loss of the fingernail.

EE was reaching out welding. A piece of brick fell off the wall and hit EE's right arm and broke it.

Employee was working at belt 460 with a dingo. When employee had the dingo in reverse, EE bumped into the belt structure which caused a laceration to the tip of the middle finger.

Employee was looking for a pry bar and a Lab tech running the 4x4 mule struck the employee.

Pancake style safety tee was opened to remove vacuum from a hose being used to dust but clean the upper building of a silo. Cover plate did not have a locking mechanism to hold open. Employee 1 was cutting hose while employee 2 was dragging hose back in. Hose hit safety tee closing plate 3/4 engaging vacuum. Employee 1 had arm caught in hose approximately 45 seconds to one minute.

While covering a pallet with a plastic wrap bag, the employee waved the bag in the air similar to the way one opens a trash bag. The bag caught the edge of the employee's headlamp that was mounted to EE's hardhat via elastic band. The waving process pulled the flashlight away and then snapped back in EE's face, resulting in EE's tooth cutting all the way through EE's lip.

Leg slid on some metal grate causing an approx 10CM wound requiring stitches.

Employee was cleaning top of rail car with jackhammer when EE felt a tweak in their back. Finished shift and went home as back was not hurting at that time. Was feeling some tightness, employee was sent for an evaluation and waiting to see doctor.

Employee was positioning himself to clean off a loading spout screen at silo17. The employee tweaked their back when EE was moving between the air slide and the handrail. EE left shift but later reported that their back was tweaked. EE later reported on 10/13/21 that their back was hurting. Then saw saw a doctor.

While working to remove a piece of old vac hose, a production tech cut EE's thumb just to the left of the knuckle.

A weathered rope located on an exterior catwalk, exposed to the elements, was being untangled by an employee. The rope began to fray and splinter into small pieces. The wind caught the rope fragments. One of the fragments went into the employee's right eye.

Technician was tightening a screw in the electrical compartment of an hvac unit when technician's hand slipped and was cut on the housing of the cabinet.

While unrigging bag EE fell off the trailer and injured arm.

Mechanic was cutting a conveyor belt using a razor knife when the blade slipped and cut left wrist. Bleeding was severe and mechanic was transported to emergency department where they received three sutures and a tetanus shot. Employee placed on light duty.

Driver suffered a major health condition at cement load out, driver was transported to local hospital where driver was pronounced deceased by medical staff.

The employee was walking up a set of open ended steps when EE's left boot got caught on the step causing EE to fall forward. When EE fell forward, EE put all of EE's weight on the left shin. A large contusion formed on EE's left shin.

Contractor was preparing glassware for air sampling when it broke cutting right hand through gloves. Contractor received 15 stitches but there is no impairment.

Mechanic had to enter a conveyor chute to cut a section of chain from a sprocket. While exiting the chute, mechanic dislocated right shoulder and had to be transported to hospital to relocate.

Process Attendant was completing inspections of the CKD Silo dust collector. As EE was climbing down the ladder EE felt and heard a pop in EE's right wrist. PA reported to EE's supervisor immediately. PA reported issues closing EE's pointer finger and gripping but no pain outside of that.

Mechanic stuck in preheat tower passenger elevator for over 30 minutes when elevator stopped moving between 1st and 2nd floor. Elevator repair contractor brought on site to move elevator to floor where employee could safely exit. Maintained constant communication with employee during event.

2 employees were stuck in passenger elevator between 4th and 5th floor. Neither employee was in a life threatening situation. MSHA notified after 30 minutes. Employees able to safely exit elevator at approximately 7 a.m. Elevator contractor able to service and repair elevator to full function on 03/06/2021.

Employee had a recordable shift on EE's annuel hearing test greater than 25db in the 2k, 3k and 4k ranges. Employee was sent to audiologist for retest and evaluation.

Plant electrician stepped into the roller mill on the grinding table. EE slipped and fell landing on left shoulder and leg. EE thought EE might be ok at first but later the next morning EE's shoulder was still hurting. EE went to the doctor the next and was put on restricted duty.

Mechanic was working in Raw Mill tapping out bolt holes. While doing this EE felt a pain in the upper groin area. EE did not go to the doctor at first--thought it might go away. On 2/1/21 EE saw a doctor that said EE has a strain in EE's upper groin area. Dr. prescribed antibiotics and told EE to return to work with weight restrictions.

Mechanic replacing chain on primary crusher and while swinging sledge to join connecting link plate hammer hit pad eye causing hammer handle to impact hand caused bruising. Mechanic did not report injury until 01/19/21. After self-care did not help, placed on restrictions 01/25/21 resulting in medical incident.
