Employee was assisting in re-installing the cutter head on the dredge. While threading the cutter head onto the shaft, the chain that was around the cutter head slipped/tightened allowing a chain link to roll and pinch EE's left pinky finger in the chain links. This resulted in a degloving/open tuft facture to the left pinky finger requiring medical treatment.

While EE was attempting to remove rocks from around a gate inside a chute that had stopped up, EE struck one of the rocks with a hammer that allowed the gate to fall. The gate struck the EE on the left thumb resulting in a minor fracture to the tip of the left thumb requiring only a small splint applied by the doctor.

Employee was attempting to drive a sprocket onto the shaft of a chain drive at the head pulley of the tunnel conveyor. As EE struck the sprocket with a sledge hammer, a sliver of metal came off of the sprocket and struck the employee in the right knee.

Employee was using a hand winch to lower a small water pump at the primary crusher. While lowering the pump, the winch handle slipped out of employee's hand allowing the handle to come around and strike the top of employee's left hand resulting in a contusion. Restrictions on use of left hand.

Employee was using channel locks to attempt to pull a bad lace from the conveyor. As the lace broke free, the momentum caused the employee to fall forward and employee struck mouth on the conveyor frame damaging six teeth. Dental work required to repair the teeth.

While attempting to puncture a 1-1/2" rubber hose with EE's non-locking pocket knife, the EE had the blade close on index finger causing a laceration across the first knuckle above the finger nail of gloved right hand. At the emergency room 5 stitches were needed to close the wound.

Employee was using a pipe wrench to loosen the curtain bolts on the Stedman crusher. As they were pulling on the wrench they felt a pull or strain in right inner thigh/groin area. Employee immediately stopped performing the task. Employee refused medical treatment at the time; however went to the ER later that night where they were diagnosed with a strain and given 1 day off.
