Fall from machine

While EE was exiting crusher booth EE fell (due to personal medical issue of low blood sugar/BP). EE fainted and does not recall the fall. EMS was called and confirmed the low blood sugar/low BP at the time of the fall and additional ankle pain. EE refused further treatment that day and was cleared by doctor the next day for low blood sugar/BP, but was kept off work for ankle.

Employee felt pain in lower back while using a water hose to wash out spilled material during cleanup.

Struck by flying object

Employee was collecting tools and rolling up welding leads onto the maintenance truck when work was completed. The automatic reel began to speed up and the employee instead of letting go of the cable, grabbed the lead near the end causing the quick connect end to whip back and strike EE on the right wrist. It was confirmed that a bone in EE's right wrist had been fractured.

Struck by falling object

EE WAS ASSISTING IN THE MOVING OF A METAL SIGN POST EMBEDDED INTO A 2'X 2'X 2' CONCRETE BLOCK WHEN IT WAS TIPPED FORWARD FROM A SKID STEER LOADER BUCKET STRIKING THE EE IN THE FACE CAUSING A LACERATION/AVULSION TO NOSE.

Over-exertion in pulling or pushing objects

Employee was using a pipe wrench to loosen the curtain bolts on the Stedman crusher. As they were pulling on the wrench they felt a pull or strain in right inner thigh/groin area. Employee immediately stopped performing the task. Employee refused medical treatment at the time; however went to the ER later that night where they were diagnosed with a strain and given 1 day off.

Over-exertion in lifting objects

Employee states while shoveling rock they hurt their shoulder. Employee notified the supervisor on Friday 5/5/2017 that the injury occurred on 4/27/2017. Employee currently off work due to restricted activity.

Extension cord caught on truck was throwing off from truck, stepped off truck, landed on both feet, right foot tibia fracture.

Fall onto or against objects

On Monday (9/28/15) employee slipped and hit right leg while cleaning retaining wall and tanks. Right leg continued to get worse, left work early on Thursday (10/1/15). Hospitalized 10/2/15 for infection. Remains off work.

Over-exertion in pulling or pushing objects

Employee was washing off concrete pad. The employee went to pull the water hose with his left arm when the hose got caught on something and jerked his arm back. The employee heard and felt a ""Pop"" in his elbow and had immediate pain. The employee was treated at the Emergency Room for his pain and was instructed to follow up with an Orthopedic doctor.

Struck against a moving object

While in transit on a company errand, truck employee was riding in came to a stopped position and was rear ended. Employee was treated and released at ER.

Employee airing up tire on wheel barrow, put too much air in the tire exploded. Tire parts hit employee on side of head. Treated and released from ER.
