While EE was attempting to remove rocks from around a gate inside a chute that had stopped up, EE struck one of the rocks with a hammer that allowed the gate to fall. The gate struck the EE on the left thumb resulting in a minor fracture to the tip of the left thumb requiring only a small splint applied by the doctor.
While lifting a spool of wire into the bed of a pickup the employee felt a pop/burn feeling in left forearm.
EE sustained a laceration to middle finger of right hand when they grabbed the hedge trimmer they were using to dislodge the blade which was hung up in a bush.
Working at bottom of feeder pan, chip down to bare steel to find plug hole; burn out the weld to move the plate-rust & fine dust on plate-immediately tried to wipe from eye and flushed eye-trouble with eye on way home; awoke to bad pain at 1:30 am; 5:00 am eye swollen shut. Medical attention sought 1/26/2017.
Loading holes, unrolling & moving the dewatering hose, hurt low back
Turning primary impact pulley, the index, middle and ring fingers of left hand smashed between v-belt and pulley. Fracture middle finger, contusion, abrasion index and ring fingers. Medical treatment. Restricted work activity.
End of shift, washing out skid-steer bucket and pins; when employee pulled out pin the bucket fell on his left foot. Fracture left big toe bone. Employee remains off work
EE was putting belts back on primary impact: mashed nail of left ring finger between V belt and thieve on impact motor. 2/13/11 sought medical attention at ER. 2/14/11 sought medical attention at ER. 2/14/11 return to modified work, Rt hand only. Note - 2/14/11 - 2/18/11 schedule vacation taken.
Employee was exiting truck in pit, lost balance and fell approximately eight (8) feet to loose stone in pit. Possible concussion, skinned hands, treated a University Medical Center ER. 7/19/07 unable to return to work until follow up with physician. 7/23/2007 restricted work activity.
WORKING AT THE PLANE, CLIMBING DOWN STIP LADDER, SLIPPED AND FELL APPROX 4 TO 5 FEET ONTO ROCK AFTER CONVEYOR HAD BEEN UNCHOKED. CUT LIP, LEFT HAND, LEFT FOREARM, RIGHT FACIAL CHEEK.
GOING TO GREASE TOP OF BOOM ON THE LOADER - FOOT SLIPPED OFF FENDER AND FELL APPROX. 10 TO 12 FEET TO GROUND - INJURED LEFT LEG MUSCLE, FACE SCRATCHED, SCRAPED LOWER BACK ON FENDER.
EE CLEANING I-BEAMS ON SCREEN TOWER, WHILE CLIMBING LADDER, HE SLIPPED & FELL HITTING CONCRETE.
EE MOVING FROM HER WORK AREA TO COWROKER'S WORK AREA WHEN SHE FELL OVER COWORKER'S TRASH CAN ONTO HER LEFT ARM AND SHOULDER.
REMOVING KELLY BAR (DRILL STEEL) FROM DM25 DRILL. KELLY BAR CAME OUT OF STABILIZER, WAS FALLING TO GROUND, CAUGHT LEG BETWEEN BAR AND BOOM. LEFT LEG JUST ABOVE THE ANKLE, BROKEN.
