Employee was attempting to remove a ceased impellor from a pump casing when a small fragment of metal sheered and punctured the employees left hand.

Employee was using a pry bar to clear a plugged chute. When the rock broke free, the employee hit their hand on the side of the chute, sustaining a fractured finger.

Employee was using a poker bar during a gypsum starvation when material shifted pinching left hand between poker bar and bin chute. Employee went to the doctor on 9/15 was released to full work duty but was later prescribed an anti inflammatory on 9/22.

Employee got finger pinched by lever arm of shotgun. On 7/24 it was confirmed that the employee fractured the tip of their lt ring finger resulting in modified work.

PINS IN LOADER BUCKET NEEDED REPAIR. INJURED WAS ATTEMPTING TO REMOVE THESE PINS WHEN PORT-A-POWER BROKE FREE FROM BRACKETS WHILE PUMPING JACK TO CREATE PREASURE ON PINS. THE TOOL STRUCK EMPL OYED IN FACE. RESULTING IN INJURY.
