While using a pry bar to perform repairs on a bed of a pickup truck, the pry bar slipped and struck the employees lip resulting in an injury.

Mechanic was performing a routine PM on 571 OB truck. EE was changing a fuel filter on the right side of the engine block. With arm extended and pushing on the filter wrench EE felt a pop in shoulder.

A maintenance technician was working on the latch bar mechanism on the dipper of an electric rope shovel. A small chunk of dirt fell from the dipper door striking the individual which caused the individual to lose their balance and striking EE's mouth on the end of the crowbar. The impact caused a cut lip which required stitches.

On October 9th, 2024, an employee was in the process of putting on a new poly end. During the process the employee noticed it was going on crooked. Employee reached down to adjust the end at the same time another employee was swinging a sledgehammer. The hammer hit the employee's right index finger, resulting in a fractured finger.

The employee stated EE was making a homemade part to repair a sink that was spraying water on the wall. EE was cutting a safety cone with a utility knife to make this part. While cutting EE reached EE's left hand inside the cone and stabbed EE's left thumb. The cut required four stitches.

Changing out carrier roller on bulldozer. Employee struck the roller with a ball pin hammer, causing a piece of metal from the roller to be a projectile which then caused a laceration to the employee's left forearm. The laceration required stitches.

Two employees were removing left side bull gear carrier pins from an 8750 dragline using a "canister" tool. While using this tool and attempting to line up all thread, the pipe slipped, putting the employee's ring finger in a pinch point.

Changing out bucket teeth on 6040 Excavator. Used a sledge hammer to hit tooth, and a piece of the hammer chipped off. This caused a small laceration on right side of stomach.

Employee was flipping cutting edges on a D-11 bulldozer. The employee was using a 24-inch pry bar to rotate the cutting edge. When the cutting edge unexpectedly shifted employee got right pinky finger pinched between the pry bar and the shop floor. Employee was wearing leather gloves.

Employee was replacing the wheel studs on 605 fuel/lube truck, using a punch and hammer. Employee was holding the punch with gloved right hand when the hammer glanced off the punch striking EE's thumb.

EE was replacing the idler arms on a D10 dozer. EE was using a RAD torquing wrench on inside bolt. The reaction arm was pointing upward. EE placed right hand on reaction arm & left hand on the trigger of the wrench. When EE applied torque pressure to the wrench, the reaction arm rotated causing right hand to be caught/pinched between the idler & the reaction arm.

Using a pry bar to remove Becket from Dump Rope Socket. Pry bar slipped, smashing employee's thumb between pry bar and dump rope sheave. An x-ray showed a fracture of the tip of employees left thumb. The employee was wearing gloves at the time of this incident.

Employee was positioning a floor jack under a loader, while pushing jack forward the jack handle was unlocked, accidentally. Which caused the employee to fall forward, placing left hand on loader and the long jack handle smashed/broke left index finger tip.

Employee placing sidewall density logging tool into test borehole felt a hitch in back. Continued to work through the week with back pain going from bad to good. On Sunday back pain became so severe that Cardno nurse case management was called and Cardno became aware of the injury.

Tech was using hydraulic ram to push new studs into the hub. Tech went to release pressure off the ram, when doing so before ee could grab it, it slipped out and struck ee in the right side of the face.

Employee was installing a pin in the roller frame of a D11 when a chip came off the pin. The chip struck the employee in the back of the right thigh resulting in a laceration that required stitches. The employee thought they were using a soft face hammer, however the hammer was found to be a hardened hammer. The manufacturer was not clear in the marking of the different hammers.

Employee was tapping a hole in a piece of plate using a starter tap. The tap broke causing the employees hand to hit the broken piece of tap still in the hole resulting in; cutting through the welding glove and into the outer edge of the palm on the right hand. The cut required stitches. The tap has a microscopic crack appearing to have been faulty.

While assembling a retainer on 104 dragline on left propel case. Employee was using a pry bar to adjust alignment of retainer. The bar was too long to get in a comfortable position and was being used overhead. The bar slipped smashing right thumb between bar and bull gear cover.

A welder was die grinding a weld on a dragline mast repair and a particle of metal got in his eye. The particle/irritation was not noticed until later that evening at home. Welder was examined the next day by an eye doctor who identified the particle and removed it.

Employees were pulling center pin on dozer frame hard bar and the pulling ram could not break the pin loose. Employee decided to hit the pin with a hammer. When he did a piece broke off the pin and hit employee in left cheek. the employee thought he was using a soft head hammer, but it was not. A physician was required to remove the steel chip and insert seven stitches.

While trying to install a bushing into a right track frame on a Komatsu D475 track dozer a mechanic hit his thumb on his left hand with a soft faced hammer. This caused his thumb to smash against the bushing making a cut on the inside of his thumb.

A mechanic was installing a pin on a scraper hitch. The employee was using a sledge hammer to pound the pin into place behind him. When the hammer contacted the pin a piece of metal from the pin broke off and embedded into the mechanic's right leg. Employee was taken to ER but had to have surgery to remove the piece of metal from his leg.

Employee state he was loosening air line fittings from pipe fittings with two wrenches. Upon squeezing he wrenches to loosen the fittings he heard a tearing sound and burn in his left shoulder. Strength was weakened in left arm, thought it would be better. By Friday evening, bruise had formed on left bicep and pain persisted. DID NOT REPORT INCIDENT UNTIL 9/2/13 late evening.

One employee was holding a B&O tool in order to remove a wedge from a socket. Another employee hit the impact area on the B&O tool with a sledge hammer when a piece of metal chipped off of the B&O hit the employee, holding the tool, in the forehead. The employee was transported to ER where doctor removed the piece of metal.

Employee was attempting remove a pin between a dozer push arm and tilt cylinder. The push arm was supported by the crane with the cylinder still pinned. When the pin was driven out the cylinder dropped approximately 2" to the dozer push arm. As it dropped it forced the tool being used to drive the pin out down pinching employee's hand between it and cylinder bore.

An employee was cutting zip ties with a utility knife. He cut through the last zip tie and the knife blade contacted the palm of his left hand causing a laceration. The employee was transported to the ER where he received 2 stitches.

EE was tightening bolts on the strut of a coal hauler. When swinging the hammer it glanced off the truck frame and hit EE's left hand. The hand was pinched between the hammer and a wrench, which lacerated the hand. The laceration required five stitches.

An employee was pounding an 8 inch pin with a sledge hammer on an 8750 dragline when a piece of metal from the hammer broke off and hit another employee standing 10 feet away. The piece of metal lacerated the left chest/shoulder area on the employee.

Mechanic was knocking out a bearing race from a hub on the ground using a brass drift and a hammer. The brass drift edge slipped off the surface of the race as he hit the drift. This caused his left hand to be smashed against a stud on the hub.

Employee was removing an equalizer seal from a dozer frame with a hammer and chisel. Upon striking the chisel, it recoiled in the employee's hand, underneath the face shield, striking employee's tooth. All appropriate PPE was being utilized.

While drilling a water well, the injured floor hand was making a drillpipe connection. The injured employee did not wait for the other floor hand to get tongs completely around the pipe, he put his hand in a pinch point, the tongs were then latched. Injured employee had the tip of his left ring finger amputated.
