Employee was helping remove a discharge chute out of the plant. Using chain hoists to move toward doorway. Chain slipped causing the chute to shift/swing toward employee. It struck the right side of employee knocking them onto the floor resulting in pain to right shoulder and right knee.

Employee stated that ee was standing on ladder changing the hydraulic filters on a hauler. As ee was pulling on the ratchet, the ladder shifted sideways causing ee to strain back/hip area. Went to doctor on 3/22/17 to have checked. First day of lost work started on 3/23/17.

Employee stated that he was tightening the bolts on the fan belt pulley on a hauler. As he was pulling on the ratchet, he stated that he felt pain in his left side area. The pain eased up later but never went away and was worse on Tuesday morning. Missed work and went to doctor. Returned on 09/16/15 on restrictions.

Employee stated that he was using a come along to repair the hand rail on a dozer. As he was pulling on the come along, it slipped causing him to strike the hand rail with his left elbow. Went to doctor on 8/11/15 to have checked due to swelling and pain. No lost time, some restrictions, and removed fluid from his elbow.

Employee stated that he was removing the bit ring from the drill bit/sub with a hammer. As he was striking with the hammer, a piece of metal flew off, struck and imbedded in top of right hand behind thumb. Became reportable on 5/16/13 when he decided to have it surgically removed and lost 2 days work.

Employee stated that he was removing the track adjuster cylinder off of the track adjuster rod with a sledge hammer. The hammer glanced off the side of the cylinder and struck his left leg below the knee. Went to doctor and missed one shift work.

Employee stated that he was using a pry bar to lift up on the dozer track so as to put it back together. When he let loose of the bar, he felt pain in between the shoulder blades. Went to doctor on 8/20/12. No lost time but is on work restrictions.

Employee stated that he was using a utility knife to cut the fan belts off of a loader. As he was cutting, the knife slipped causing him to lacerate his little finger (5th digit) on his right hand. Reportable due to requiring sutures.

Employee stated that he was holding the vibrator screen upright with his left hand while using a utility knife to cut the rubber tips off the side of the screen with his right hand. The screen slipped causing his left hand to move to the right and the knife came down and lacerated his left ring and small fingers. Reportable due to sutures.

Employee stated that he was cranking the handle on the leveling jack on the utility trailer, so as to unhook it from the dump truck, when he felt pain in his lower back area. He went to doctor on 8-13-2010 and incurred lost workdays starting with that date.

Employee stated that he was using a chain hoist to reposition the flop gate on the main conveyor belt. As he was pulling on the chain hoist, he felt a pop in his right shoulder. Went to doctor on 3/17/10. Continued working but became reportable when lost time was incurred on 6/9/10 due to having surgical repair on right rotator cuff.

Employee stated that he was helping remove a bogie pin from a dozer roller frame. He was holding a bar while another mechanic was using a hammer to knock out the pin. The bar bounced off and caught his left little finger between the bar and the roller frame, resulting in a fracture to the tip of his little finger.

Employee stated that he was helping mechanic to change the teeth on a loader bucket. Mechanic cut teeth off with cutting torch and employee was knocking teeth off with a hammer. A piece of metal slag flew off, struck and embedded in his upper left arm.

Employee stated that he was trying to dislodge rocks in the crusher so as to unclog the crusher. He placed his left hand on the top of the chute to the crusher for balance. A large rock came loose and struck his left thumb as it laid on the metal chute, severing the tip of his left thumb.
