Over-exertion in pulling or pushing objects

Employee stated that he was climbing up ladder onto the drill. As he was pulling himself up the ladder, he felt a sharp pain in his left shoulder. Finished working this shift but missed the 11/11/14 work shift due to going to the doctor and having shoulder checked out.

Struck by falling object

Employee stated that he was hanging a temporary curtain in rear window of loader so as to block glare from the light plant lights. His hand hit the overhead liner on the cab causing dust/dirt particle to fall into left eye.

driver got stuck on ice, got out to get chains and fell due to ice, hurt shoulder.

Fall onto or against objects

Employee stated that he was getting out of his mechanic's truck. As he put his feet on the ground, they slid out from under him causing him to fall and strike his head against the floor of the truck. This resulted in a cervical strain and headache.

Employee stated that he was coming down the steps on the loader when his foot slipped off the step. He grabbed hold of the hand hold on the side of the engine compartment to prevent him from falling. As a result, he pulled a muscle in his left shoulder.

Employee stated that he was working on a D10 dozer. As he reached to get tools, his foot slipped/stumbled on some small rocks laying on ground causing him to twist his right knee. Not reportable until he lost work on 9/15/10 due to doctor visit and starting 9/24/10 due to orthoscopic surgery on his right knee.

Employee stated that he was training for the position with the lube crew. While helping service a loader, he sat down on top of the engine hood attempting to take his coveralls off. He said he fell backwards off the loader and down between the rear tire and loader striking the axle.

Employee stated he was operating hauler on road from pit to dump area. He hit a dip in road, causing hauler to bounce up and down onto road. Jarred employee. Not reportable till lost work on 12-3-2007.

Employee stated that he was disconnecting fuel hose from fuel truck. As he did, diesel fuel sprayed out on him. He stepped back to get out of the way. As he did, he tripped on some rocks, fell onto ground striking left elbow on ground causing laceration.

EMPLOYEE STATED THAT HE WAS COMING DOWN THE STEPS AT THE PARTS TRAILER. THE BOOT ON HIS RIGHT FOOT GOT CAUGHT ON THE SECOND STEP, CAUSING HIM TO FALL FORWARD TOWARD THE GROUND. HE LANDED ON HI S LEFT FOOT CAUSING IT TO CREEL UNDER RESULTING IN FRACTURE ABOVE THE ANKLE ON HIS LEFT FOOT. REPORTABLE BUT NO LOST TIME INCURRED.

EE STATES HE WAS WORKING ON DRILL BENCH AND IT WAS RAINY AND MUDDY. HE GOT OUT OF THE TRUCK AND WALKED TOWARD THE TRUCK CONTROLS ON THE BACK OF THE TRUCK. HE TURNED TO REACH FOR THE CTONROLS A ND FELT HIS BACK PULL. HE THOUGHT THE PAIN WOULDGO AWAY HE WENT TO THE DR THE NEXT DAY BACK SRAIN.

THE EE WAS MOVING AN ENGINE VIBRATION DAMPER TO ALLOW ACCESS TO THE FRONT CRANKSHAFT SEAL WHEN HE FELT SOMETHING POP IN THE LOWER MIDDLE PORTION OF HIS LOWER BACK.

EE STATED THAT HE WAS OPERATING A DOZER PUSHING SHOT ROCK DIRT. HE SHIFTED THE DOZER TRANSMISSION INTO REVERSE AND TURNED HIS HEAD TO LOOK BACK HE STATED THAT WHEN HE TURNED HIS HEAD, HE FELT A ""POP"" IN HIS NECK AND HAD PAIN IN HIS NECK.

EMPLOYEE STATES HE WAS LOADING A SHOT AND LOOSE MATERIAL BEGAN TO FELL OF WELL. WHILE TRYING TO ESCAPE, HE FELL ON ROCKS CAUSING A CONTUSION TO HIS RIGHT ARM AND RIB CAGE.

EE STATED THAT HE WAS GETTING INTO THE LOADER CAB. HE HAD OPENED THE DOOR AND WAS REACHING IN TO GET A BROOM SO AS TO SWEEPOUT THE CAB. AS HE WAS STRETCHING/TWISTING, HE FELT A PULLING SENSATI ON IN HIS LOWER BACK/LEFT SIDE AREA. REPORTABLE DUE TO MISSING ONE WORK SHIFT ON 8-9-03.

EE STATED THAT HE WAS TRYING TO ALIGN THE BOLT HOLES ON A PUMP DRIVE ON THE HAULER. HE WAS USING A PRY BAR IN HIS LEFT HAND & WAS PUSHING ON THE PUMP DRIVE WITH HIS RIGHT LEG WHEN HE FELT A ST INGING SENSATION IN HIS RIGHT GROIN AREA. BECAME REPORTABLE WHEN LOST WORK TIME WAS INCURRED STARTING ON 8-23-2002.

EE STATED THAT HE WAS OPERATING A DOZER PUSHING SHOT MATERIAL OFF OF THE BENCH WHEN A SEVERE THUNDERSTORM OCCURRED IN THE AREA. THE DOZER WAS STRUCK BY LIGHTNING DISABLING IT. HE FELT A LITTLE TINGLING IN HIS LEFT ARM & WENT TO HOSPITAL TO BE CHECKED. NO APPARENT INJURIES NOTED BUT KEPT FOR OBSERVATION/MONITORING.

EE STATES: CLIMBING UP LADDER ON TANKER WHEN FOOT SLIPPED OFF REAR BUMPER CAUSING TOES TO BEND AND HEAD CAME FORWARD CAUSING NECK PAIN.

EE RECEIVED ON OR ABOUT 4-16-02 A NOTICE DATED 4-15-02 WHERE THE EE HAS FILED A CLAIM FOR BLACK LUNG BENEFITS. THE EE ALLEGES COAL WORKERS PNEUMOCONIOSIS. NO LOST TIME HAS BEEN INCURRED.

EE RAISED THE ENGINE HOOD ON THE HAULER TO CONNECT AN ELECTRICAL WIRE ONTO THE AC COMPRESSOR. HE ACCIDENTLY HIT AN ANTIFREEZE LINE WITH HIS FOOT CAUSING IT TO RUPTURE AND SPRAY HOT ANTIFREEZE ONTO HIS RIGHT SIDE/HIP AREA. HE WENT TO DOCTOR DUE TO BURNS AND SOME BLISTERING ON SIDE/HIP. NO LOST TIME INCURRED.

EE WAS AT THE BASE OF THE HIGHWALL IN THE PROCESS OF MEASURING THE DISTANCE TO THE FIRST AND SECOND ROWS OF DRILL HOLES WHICH WERE TO BE DRILLED. AS HE WAS WALKING AWAY FROM THE WALL, A ROCK F ELL OFF OF THE HIGHWALL AND STRUCK THE EE IN THEBACK AREA CAUSING MULTIPLE INJURIES.

EMPLOYEE STATED THAT HE HAD FINISHED HIS WORK SHIFT AND WAS EXITING HIS HAULER. HE STATED THAT HE HAD NOT HIT OR TWISTED HIS KNEE IN THE PROCESS. HOWEVER, AS HE WAS CLIMBING DOWN THE HAULER LA DDER HE FELT A BURNING SENSATION IN HIS RIGHT KNEE. AFTER REACHING THE GROUND, HE STATED THAT THE BURNING EASED OFF. ON 8-23 HE MISSED WORK DUE TO KNEE AND WENT TO DOCTOR.
