Employee was struck near the left eye (under his safety glasses) with a high pressure plug from the main manifold block after removing the retaining staple from the plug. Employee received 6 stitches under his left eye.

Employee was using a 30# sledge hammer to dislodge the rock teeth from a loader bucket. A piece of metal chipped off the tooth and struck the employee in the arm. The metal was removed and the wound closed with 2 stitches.

EE became fatigued using his right hand to cut strips in lagging on pulley. Switched to his left hand and shortly after slipped and poked himself in the left thigh just above the knee. He sustained a 3/8" cut that was managed with first aid. After his shift he went to the dr for a Tetanus shot and was convinced that he need stitches to prevent infection. Reportable injury.

Employee was prying up a tail gate on a pick-up truck with a screw driver. The screw driver slipped out of his hand and hit employee in the mouth chipping a lower tooth. Employee continue to work and did not report the event until the end of shift the following day (4-6-11).

The wrong tool for the task, a long handled sledge hammer (10lb) was used to set railroad spikes for the spike driving crew following. The primary hazard was not identified during the SLAM Process. The employee also got complacent and lost focus of hand placement. The result, the employee lost the index finger nail and recieved 3 sutures.

Employee was removing a steering cylinder from a Letourneau loader. Both pins were removed and had a come-a-long installed to assist in lowering the component down. The lifting strap was installed somewhere in the middle of the str cylinder barrel. When the steering was rocked loose from position the cylinder swung and crushed the left hand of the employee.

While troubleshooting a hydraulic, employee was trying to loosen a fitting by pushing down on the wrench. When the fitting broke loose, he hit his chin on a piece of belting that was covering the power pack which chipped his tooth. Employee was released full duty.

EE was in process of using a come-along to move a piece of metal plate associated with the #1 crusher screen. As he reached under the screen to unhook the come-along the metal plate slid and pinched two of his left fingers, which resulted in 3 stitches on his left index finger. The extra slack in the chain stopped the metal from falling fully onto this fingers.

WHILE TIGHTENING A BOLT NEAR THE HYDRAULIC LINE ON TRD5321 DOZER, EE HIT THE COOLANT HOSE CAUSING THE ANTI-FREEZE FROM THE HOSE TO DRIP INTO HIS LEFT EYE. HE SUSTAINED CORNEAL ABRASION IN HIS EYE. HE RECEIVED FURTHER MEDICAL CARE AND RETURNED TO FULL DUTY ON NOVEMBER 5, 2002.

THE EE WAS LIFTING UP THE WIRE ROPE ON THE DRUM WITH A DEF ECTIVE PRYBAR WHEN THE END OF THE BAR WAS FORCEDINTO HIS PALM CUTTING THROUGH HIS RUBBER GLOVES WHICH WERE DEEMED AS INADEQUATE PERSONAL PROTECTIVE EQUIPMENT.
