While removing a wheel unit from shuttle car, employee had a breaker bar on a wrench. He stood on the breaker bar which slipped off the wrench and struck him on the leg.
Cutting ties off plastic lid of 5 gal. bucket of plaster with utility knife and cut hand in thumb area of left hand.
Injured was preparing to rock dust an area with a co-worker and was holding a bag of rock dust and co-worker was slashing bags open with utility knife and cut injured person on right hand.
Injured was using blade knife cutting off shuttle car cable to make splice when knife slipped cutting left index finger-had 5 ctitches.
Was splicing trailing cable and cut right index finger with blade knife.
Was taking hex nuts off south section scoop panel board. Was pulling in up motion on extra tight hex nut when socket slipped off nut causing injured to pull back muscles.
Was on belt moving crew using come-a-long to pull belt and tail piece. Got hand & fingers caught between chain hoist & tail piece.
Employee was prying on the tail gate of the Mack truck with a slate bar. Tail gate was frozen. He experienced pain in his abdominal muscle. On 3/9/07 he informed me he had a lump in his abdomen and was scheduled for an MRI on 3/13/07. A hernia was detected. He was scheduled for surgery on 4/9/07. His 1st day missed was 4/9/07.
Injured employee was threading a peice of 1-1/4" conduit with a pony threader. He was holding the threader in his hands. The torque of the threader caused it to slip out of his hands and strike him in the leftf knee.
Had quill in 3" yellow mine water line greased by channel locks and was hitting with hammer. Missed channel locks and hit left hand with hammer on back of hand.
Individual was trying to reform a piece of the frame of a SC21 shuttle car, prying on it a hammer flew out and hit him in the hand.
EE pryed against bottom structure with a slate bar. Lost footing, slipped and bar came down against his collar bone. Ended up bruised.
WHEN LOADING ROOF BOLTER, THE OPERATOR WAS RE-ADJUSTING GUARDS WHEN HE SAW THAT HE NEEDED A NEW HOLE CUT IN THE RUBBER MAT. WHILE TRYING TO CUT THE HOLE WITH A RAZOR KNIFE THE KNIFE SLIPPED & SLICED HIS PALM OPEN.
WHILE SAWING POST SAW BLADE JUMPED OUT OF NOTCH.CUT LEFT INDEX FINGER.
EE WAS PUNCHING HOLE,1/2",IN RUBBER CONVEYOR BELTING USING AN ARCH PUNCH. EE STRUCK HIS LEFT HAND WITH A 3 LB SHOP HAMMER WHILE HOLDING THE ARCH PUNCH. HE STRUCK HIMSELF ON THE FIRST FINGER BE HIND THE FIRST KNUCKLE AND THE REAR AREA OF HIS THUMB, CAUSING SWELLING AND TENDERNESS.
INJURED EMPLOYEE WAS REPLACING AN END ON AN ELECTRIC HEATER. WHILE USING AN ELECTRICIAN'S KNIFE,THE KNIFE BLADE DID NOT LOCK AND CLOSED ON INDEXFINGER OF LEFT HAND.
WHILE REMOVING BOLTS FROM A DEWATERING SCREEN, THE WRENCH BEING USED SLIPPED AND CAUSED EMPLOYEE TO FALL BACKWARDS AND INJURED HIS BACK AND/OR NECK.
VICTIM WAS REMOVING PIN FROM A CHAIN WHILE STRIKING PIN WITH HAMMER A PIECE OF HAMMER BROKE OFF AND PIERCED THE BACK OF HIS HAND.
