Employee was loosening a bolt on a brake caliper of a pickup truck, when EE felt a pain in EE's back. EE was diagnosed with a lumbar strain.
Employee was removing old bits from the Trencor Surface Miner with a large chisel. While the employee was hitting the chisel with a hammer, the chisel became dislodged from the holder striking the employee in the nose.
Employee was repositioning a jack-stand, and while maneuvering the stand, it shifted and the employee placed EE's right hand on the top of the stand to prevent it from tilting forward, resulting in pinching EE's right thumb in between the stand and the frame of mobile equipment EE was working on. The tip of the right thumb was fractured.
Dust had built up on a beam used for hoisting. When utilizing the equipment, the dust was liberated and was able to get behind the worker's safety glasses and into eye.
A mine mechanic was attempting to drive out the front pivot pin from a roof bolter using a 20# sledgehammer and a T-handle drift pin. The employee missed a swing, which resulted in the employee's right index finger getting caught between the handle of the sledgehammer and the drift pin. The finger was lacerated along the top and required 7 stitches.
Employee was replacing production loader push plate hose at south break room. As employee pushed on wrench they felt a pop in the left shoulder. Employee was evaluated and diagnosed with a superior labral tear.
Employee was changing seat compressor on a front end loader, the porta-power jack slipped causing the blocking to come loose allowing the seat to fall on employee's hand smashing the middle finger of the employee's left hand resulting in an open fracture.
Employee was using metal bar to try and get low purity fines material to flow through the feeder when EE felt a pop/twinge followed by pain in left wrist. Employee was diagnosed with a left wrist sprain.
Employee was loosening a screen bolt with a 1 1/4" ratchet wrench when the nut came loose, the employee struck the tip of his finger between the wrench and the frame work.
Employee was in the process of removing a final drive sprocket from a Komatsu PC600 excavator by striking the sprocket with a metal hammer. He hit the sprocket twice and on the third swing a small piece of metal broke off the sprocket striking and embedding in employee's left inside forearm.
EE was repeatedly swing sledge hammers to help unplug feed chute. He was sore that night, but woke up next morning feeling worse & reported injury when he came to work 12/22. Initially he felt it was a slight pull & he worked that day. 2 doc visits 12/23 diagnosis: strain-rotator cuff, release no restrictions, 12/27 prescribed Naproxin, 2 wks PT, again no restrictions.
An employee was using a hammer to free a loader bucket tooth that was stuck. Pieces of metal broke off the tooth when struck by the hammer and struck nearby employees. The employee using the hammer had a 3/16 inch metal shard lodge in his upper left arm, and a nearby observing employee had a 3/8 inch metal shard lodge in his right hand.
An employee was using a hammer to free a loader bucket tooth that was stuck. Pieces of metal broke off the tooth when struck by the hammer and struck nearby employees. The employee using the hammer had a 3/16 inch metal shard lodge in his upper left arm, and a nearby observing employee had a 3/8 inch metal shard lodge in his right hand.
Employee was clearing a rock jam out of #2 Belt Transition with a steel pole and experienced an AC Joint strain to the right shoulder.
EE was using a ratchet with a 12" extension to remove a bolt from a converter on a loader. He exerted force to try and break the bolt loose. When it came free, the EE's finger struck the end of a screw clamp on a hose.
Employee was lifting the #2 screen spider with a pry bar on the #1 Mill. The screen spider was set on the I-Beam frame. As the employee proceeded to lift the screen with the pry bar he began to feel for the bolt hole underneath the frame. The screen spider started to slide along the pry bar and pinched the pad of his right thumb against the frame.
Was hand scaling when a large rock was knocked loose. The rock fell onto the scaling bar, causing the scaling bar to hit the employee on the lower right leg.
After hand scaling for approximately 4.5 hours and task was completed, employee felt sore in right forearm. Employee was examined by company physician on 10/1/04. Intersection syndrome was diagnosis.
EMPLOYEE WAS ATTEMPTING TO BREAK A GYPSUM ROCK WITH A SLEDGEHAMMER AND FELT PAIN IN HIS LOW BACK.
EE WAS ATTEMPTING TO REMOVE THE LID FROM A PLASTIC FIVE GALLON BUCKET SO IT COULD BE USED FOR TRANSPORTING TOOLS. HE CUT INTERLOCKING TABS WITH UTILITY KNIFE. AS HE APPLIED DOWNWARD PRESSURE, THRYING TO CUT THROUGH ONE OF THE TABS, THE BUCKET SHIFTED. HE REACHED TO GRAB IT AND INADVERTANTLY STRUCK HIS LEFT WRIST WITH UTLITY KNIFE.
THE EE WAS HOLDING UP A BUSHING WITH A TIRE SPOON AT THE REAR OF THE MECHANICAL SCALER BOOM WHILE ANOTHER EE WAS INSTALLING A PIN FROM THE OPPOSITE SIDE OF THE SCALER. THE OTHER EE DROVE IN TH E PIN, WHICH HIT THE TIRE SPOON, WHICH HIT THE EE'S CHIN. THE EE NEEDED 9 STITCHES TO TIE UP THE LACERATION TO THE CHIN.
EE WAS PUTTING NEW PINS INTO THE FEEDER BREAKER DRAG CHAIN. WHILE SITTING IN AN AKWARD POSITION,HE USED HIS LEFT HAND FOR SUPPORT. HE PLACED HISHAND APPROXIMATELY 2" AWAY FROM A PIN HE WAS ATT EMPTING TO DRIVE DOWN WITH A HAMMER. WHEN HE SWUNG THE HAMMER HE HIT HIS LEFT INDEX FINGER & FRACTURED THE TIP.
EE WAS IN THE PROCESS OF REMOVING THE FRONT BELLY PAN ON 739 DOZER TO REPAIR A COOLANT LEAK. THE BELLY PAN JACK WAS HANGING UP AND NOT COMING DOWN. EE TRIED USING A PRYBAR TO ASSIST. THE BAR S LIPPED & HIS FORWARD MOMENTUM CAUSED HIM TO STRIKE HIS MOUTH ON THE BLADE STABILIZER BAR.
CONTRACTOR WAS WORKING IN THE MINE REPAIRING A BELT. THE CONTRACTOR WAS CUTTING A PIECE OF BELTWITH A UTILITY KNIFE AND A STRAIGHT EDGE. WHILE CUTTNG THE BELT, HIS HAND SLIPPED CUTTING HIS RIGHT THIGH JUST ABOVE THE KNEE
ON 5/25/00, EMPLOYEE REPORTS HE BUMPED HIS ELBOW ON A SCRAPER SEAT. NO TREATMENT WAS RENDERED. THEN ON 7/26/00, HE REPORTS AGGRAVATING SAME ELBOW INJURY WHEN USING A SLEDGEHAMMER & ASKED TO SEE A DOCTOR. HE WAS TAKEN TO CLINIC ON 7/28; X-RAYS TAKEN WERE NEGATIVE. ANTI-INFLAMMATORY WAS PRESCRIBED AND THE EMPLOYEE WAS RELEASED TO RETURN TO WORK.
2 EE'S WERE ATTEMPTING TO REMOVE A GEAR FROM A SHAFT. AFTER CUTTING GEAR WITH TORCH, EE TRIED TOFREE GEAR BY STRIKINGIT WITH A 3LB SHOP HAMMER INJ EE WAS STRUCK BY A PIECE OF GEAR TOOTH THAT CAUSED LACERATION TO RIGHT FOREARM FOREIGN BODY WAS REMOVED & 4 STITCHES WERE USED TO CLOSE WOUND
EE WAS SCALING CORNER RIB, PRYING A SMALLER ROCK WITH LEVERAGE POINT AGAINST A LARGER ROCK SHARING THE SAME FRACTURE SYSTEM. BOTH ROCKS FELL AND THE LARGER LANDED ON EE'S FOOT. FRACTURED LEFT GREAT TOE.
EE WAS RODDING OUT #3 LAND PLASTER BIN. A LONG HEAVY ROD WITH A BEND AT THE END OF IT WAS USED TO DISLODGE HARDENED MATERIAL. WHILE RODDING ON THE BIN W/FORCE, PART OF THE MATERIAL GAVE WAY. T HE ROD PROCEDDED TO GO DOWN INTO THE BIN, PINCHING THE EE'S FINGER BETWEEN THE ROD AND THE HAND RAIL.
