Employee was removing old bits from the Trencor Surface Miner with a large chisel. While the employee was hitting the chisel with a hammer, the chisel became dislodged from the holder striking the employee in the nose.

Employee was entering Volvo haul truck and felt a pop in their right knee on 3/12. Employee informed operator on 3/25 of the injury. Employee was seen by company doctor on 3/27 and released with no restrictions. Restrictions were instituted after employee consulted with their personal doctor and had a follow-up exam with the company physician on 4/10.

While employee was clearing snow from a small pile of fines that had been left by a loader operator by the grinding room earlier for sampling, the employee moved to the left while doing so left foot slipped out from underneath employee on some snow covered ice that employee did not realize was there. Employee twisted knee and fell into the fines pile.

Employee was loosening a screen bolt with a 1 1/4"" ratchet wrench when the nut came loose, the employee struck the tip of his finger between the wrench and the frame work.
Employee was repairing hose on Cat 992 end loader. He was in an awkward position for an extended period of time while making the repair. When he stood up to stretch he felt something was wrong.

The affected employee and another checked the main cutting drum of the Surface Miner. The drum would not turn. The jog wheel was then jogged to free the main cutting drum. While one employee was operating the jog controls the affected employee placed his left hand on the sheave getting three fingers caught in the nip between the jog wheel and the sheave.

AS MAINTENANCE, A TEMPORARY HOISTING MECHANISM WAS BEING USED TO LIFT AN I-BEAM, APPROX 20'L ANDWEIGHING 860 LBS. INJURED EE WAS STANDING ON THE LAUNCH, HOLDING A TAGLINE WHEN THE I-BEAM AND T HE HOISTING EQUIP FELL TO THE BOAT AND INTO THE WATER. WHILE THE INJURED CANNOT REMEMBER THE ACCIDENT, THE INJURY APPEARS TO HAVE RESULTED FROM THE FALL, THE EE STRUCK HIS BACK ON THE LAUNCH.
WHILE INSTALLING A RETURN ROLLER ON THE TUBE BELT THE ROLLER SLIPPED OUT OF THE HANGERS & PINCHED THE EE'S LEFT INDEX FINGER CAUSING A FRACTURE AT THE TIP
THE EE WAS WORKING ON THE BOTTOM FLOOR OF THE CRUSHED ROCK BUILDING. ANOTHER EE WAS WELDING ON THE SECOND FLOOR WHEN HOT SLAG FELL FROM THE UPPER FLOOR AND SPLATTERED UNDER THE EE'S SAFETY GLA SSES CAUSING A BURN TO THE EYE.
THE EE WAS OPERATING A CATERPILLAR 777 HAULAGE TRUCK AND WAS HAULING OVERBURDEN. WHILE TURNING AROUND AT THE DUMP SITE, THE TRUCK BROKE THROUGH THE FROST CAUSING THE TIRES TO SINK INTO THE GRO UND AND TIPPING THE TRUCK ON ITS SIDE. THE IMPACT CAUSED A SPRAIN TO THE EE'S LEFT SHOULDER REQUIRING PRESCRIPTION MEDICATION.
